14 Example kindly provided with permission of West Hertfordshire Hospital NHS Trust Risk Assessment X Risks which were considered serious and/or needed further investigation urgently. Medical Devices -Risk REPORT Adequate records Technical Leadership Medical Devices policy and monitoring. Inventory control Inconsistent application of processes led to inappropriate equipment and support services purchases on some occasions Working environment for Clinical Engineering Disposal of Equipment - Users retain and continue to use equipment past its supported lifespan CAS alerts did not have appropriate Medical Devices technical appraisal and actions. Datix reports - delays in appropriate reviews and responses.

Medical Devices -Risk REPORT Service records did not adequately reflect service activity External service provider management required Work processes need review Equipment Replacement Planning Equipment utilisation and availability Clinical Engineering COSHH management not fully compliant Information Governance on equipment library record keeping Service availability (enhanced support for medical equipment to meet extended hours and efficiency objectives) and Out of Hours support

16 Example kindly provided with permission of West Hertfordshire Hospital NHS Trust Risk Assessment Medical Devices -Risk REPORT Risks that have been mitigated but must be monitored. Technical competency issues Information Governance linked to the recording of pressure relieving mattress use. Maintenance of Patient Trolleys, Couches and Exam Chairs. Helpdesk process antiquated and manual User support interface / process for reporting defective equipment was antiquated. Condition of labels on equipment leading to confusion when users report faults. Datix reporting categorisation (Medical Devices) not continually kept up to date.
