For example, those patients who are on staff-assisted intermittent peritoneal dialysis or performing incenter selfperitoneal dialysis would be counted in this field.

20 Understanding the Status Change Updates

21 Understanding the Statuses CMS-2744 Status Descriptions DRAFT FINAL REJECTED ACCEPTED FINALIZED REOPENED Indicates that a new CMS Annual Facility Survey has been generated. This status includes any errors or warnings when generated from facility data in CROWNWeb. Indicates the facility has generated the survey and submitted the survey to the Network for review and acceptance/ rejection. Indicates the Network has found uncorrected errors or -ORunaddressed warnings on the survey and returned the survey back to the facility for correction. Indicates the Network has reviewed the survey s data and accepted the survey in its current state. Indicates that the Network has accepted the submitted survey from the facility, and has clicked FINALIZED to complete the survey process. Indicates that an error was found on the CMS-2744 form after it was FINALIZED, and that the form has been reopened via a request to the QualityNet Help Desk. The reopened form must be corrected by the facility and resubmitted to the Network for acceptance.

23 Form 2744 Search To locate your 2744 form, click on Form 2744 from the Home page.

24 Form 2744 Search Enter 2014 in the Survey Year field, and click Search to bring up a search list.

25 Form 2744 Search Results Click on the ID number for your facility from the search list.

26 View Facility 2744 The Facility 2744 displays on the screen.

27 Submit for Acceptance If all errors have been resolved, scroll down and click Submit for Acceptance.

30 Review Accepted Status Review the Form 2744 Status screen to view when the Network accepts the CMS-2744; it will display in the Accepted Status section.

32 Checking for Finalized Status After submitting your CMS-2744 for acceptance, regularly check the Status Change Updates screen to see when your CMS-2744 has been finalized (or approved) by the Network. 1. Click on Form 2744 from the Home page. The 2744 Search screen displays.

40 Causes of Error Messages CMS-2744 data is pulled from the following screens in CROWNWeb: Facility Details Admit/Discharge Treatment Summary Patient Attributes Errors display due to invalid or missing data. Error messages are removed after users correct the data, and re-generate their CMS-2744 forms. Errors must be resolved before a form can be accepted.
