I have reviewed all documents including the Supplement to Bidffender Form (Vendor Questionnaire) and after careful evaluation, I concur with awards to: Choice Environmental Services of Broward, Inc. (Groups 1 and 2), Waste Management, Inc. of Florida (Group 3) and Waste Services of Florida, Inc. (Group 4) in the initial three-year amount of $405, D I have reviewed the Vendor's financial background/d&b Report and am satisfied with the Vendor's rating and payment performance. D Not applicable- I have reviewed the response to the Vendor Questionnaire in regards to litigation history and there is no issue of concern.
