7 State HIE Initiative State HIE Initiatives Arkansas has SHARE (State Health Alliance for Record Exchange) SHARE is a functioning HIE Direct Mail / Secure Mail ADH Registries Immunizations, ELR, SS & Cancer Patient Alerts for hospitals & certain PCMH practices Adoption of SHARE has been good Hospitals 56+ participating Practices / Clinics participating Patient Records over 2.0M unique patient records

11 The Sequoia Project National Initiatives Multiple Initiates NwHIN 2007 Nationwide Health Information Network is a set of standards, services, & policies that enable the secure exchange of health information. Not a physical Network. ehealth Exchange 2012 Transitioned/rebranded from ONC to Sequoia, has grown to become the county's largest HIE Network & includes data from VA, DOD, SS & CMS. Carequality 2014 Interoperability Framework, is a publicprivate collaborative, building consensus among health IT data exchanges for enabling data exchange among networks. RSNA Image Share Network - is a conformity that tests the compliance of vendors systems using quality standards for accurate & efficient exchange of medical images.

12 CommonWell Health Alliance National Initiatives Founded in 2013 Is a not-for-profit trade association of health IT companies Vendor led by Allscripts, AthenaHealth, Cerner, Evident/CPSI, Greenway, McKesson, eclinical, Healthland, MedHost, Meditech, CV & others The underlying solution is RelayHealth Core services are patient identity/linking, secure data access/exchange across company's w/disparate systems CommonWell members and Carequality participants represent more than 90 percent of the acute HER market and nearly 60 percent of the ambulatory EHR market

13 The Direct Project National Initiatives The Direct Project launched in 2010 as part of NwHIN DirectTrust is a collaborative non-profit association Mission is to support Secure & Interoperable HIE Adoption of The Direct Project trust framework ; 94,000 health care organizations 1,428,032 Direct addresses/accounts provider-to-provider exchange bi-directional exchange between patients & their providers Over 300 EHR and PHR vendors & over 50 HIEs SHARE is part of the DirectTrust

17 Blockchain What is Blockchain? Developed in 2008 Designed for digital cryptocurrency known as Bitcoin Securely allows the Transfer of Assets in a network Forget, for a moment, big data ; but think long data Long data is the lifetime history of data related to a person This is what blockchains do exceedingly well It is know for its Shared Ledger Capability

18 Interoperability with Blockchain Blockchain Crucial to understand that each block cannot be changed, deleted or otherwise modified Many are excited about the potential for data security Rather than a central database, the blockchain record can be distributed and shared across networks Its open & decentralized nature lend itself to managing health records & proving identity Blockchain has both interoperability & security potential & could enable the Holy Grail of a longitudinal health record by securing data as it s exchanged among organizations in a format that is usable for various clinicians across the care continuum.

19 Healthcare with Blockchain Blockchain Deloite & IBM reported 16 % of payer and provider executives expect to have a commercial blockchain application at scale in 2017 ONC requested white papers and over 60 were submitted Hyperledger HealthCare (HLHC) Working Group Cross industry group supporting open source; Accenture, AE, IBM, Intel, SAP, JP Morgan & others Possible applications in Healthcare include Clinical Trials Compliance Medical Records Master Patient Index

21 Interoperability & HIEs Interoperability & HIEs State & National HIE Initiatives will drive Interoperability Population Health & Precision Medicine will not happen w/o Interoperability Patients expect providers to have their longitudinal data Interoperability will connect owned & non-owned clinics & hospitals Interoperability between Behavioral Health and Primary Care will allow providers to treat the whole patient Interoperability will be the glue that holds it all together Remember HIEs are for the Patient

23 Predicting the Future I do not own a crystal ball Predicting the Future State & Federal regulations are still in process National Initiatives; The Sequoia Project, Carequality & CommonWell Health Alliance will play a role Population Health will be driven by Interoperability Blockchain technology will be under the covers HIEs will survive because they are for the Patients True interoperability has always been the goal, HHS Secretary Tom Price, Health Datapalooza, May 2017, in Washington D.C.
