The original Bid Due Date of 2/4/2016 has been extended to 3:00 PM Local Time. 2. Exhibit A, SOS has been revised to specify TPO thickness and define warranty period and to clarify that approved equivalents may be accepted for the proposed roof material. 3. Bid Sheet has been revised to allow for documentation of roofing system to be bid. Note: If you have already submitted a bid, but will be submitting a revised bid due to this Addendum 1 (to be opened Feb. 3 PM), please notify Greg Hill via greg.hill@dot.state.fl.us or Please acknowledge receipt of this Addendum by completing and including in your bid submittal, by no later than the time and date of the bid/proposal opening. Failure to do so may subject the bidder/proposer to disqualification. 3 EXHIBIT A Revised 2/2/2016 Scope of Services Chiller Plant Painting and Roof Repair DOT Haydon Burns Building, 605 Suwannee Street, Tallahassee, FL The contractor shall furnish all paint, painting materials, roofing materials, and provide all labor related to finishing the exterior and interior surfaces of the Haydon Burns chiller plant as described herein. The contractor will be responsible for cleaning off any loose paint or debris prior to application of new paint by washing and scraping the exterior, roof top area, and doors. The interior walls shall only be prepped by scraping due to the delicate service equipment located within the chiller plant. All cracks will be rout out that cannot be bridged with exterior acrylic coating paint and filled with a urethane sealant. All cracks will have texture applied so as to blend in with surrounding texture of walls. ONE coat of masonry conditioner will be applied to all masonry surfaces and TWO coats of acrylic exterior paint will be applied to same masonry surfaces. Prepare and paint interior and exterior sides of doors with acrylic paint (color to be approved by Department project manager). At rooftop, all open mortar joints and holes will be sealed with urethane sealant and apply TWO coats of acrylic paint to all masonry surfaces. In interior, prepare all water damaged areas for acceptance of one coat of masonry conditioner and TWO coats of acrylic paint to all concrete block areas that are accessible. All unpainted surfaces are to be protected and covered at all times during project. Areas not covered by this project include: containment area, canopy/overhangs, rehanging of signage, or pruning of vegetation around chiller plant. Install new Thermoplastic Olefin (TPO) (60 mil membrane with a 20 year warranty) fleece back roof system (or approved equivalent/alternate brand) over existing roof and replace all flashing. Bidders submitting a bid that is based on an alternate brand of roofing material must submit, with their bid, complete specifications and descriptive technical literature for the proposed material and application. 4 EXHIBIT A Revised 2/2/2016 To provide extra care, the contractor must practice safe working habits and use extreme caution when outdoors of the Haydon Burns Building. Drop cloths must be used in and around areas in which painting occurs. The contractor will be responsible in ensuring all measurements are correct, by a site visit prior to the beginning of the project, in order to purchase an accurate amount of supplies. The contractor shall be held liable for any HVAC equipment within or around the chiller plant that may become damaged due to negligence on their part during the project as deemed appropriate by the Department. The project shall be done during normal working hours of 7:00 a.m. - 5:00 p.m. Monday- Friday. Once the dates have been chosen, contractor must notify the Department one week prior to commencement of project. The contractor must use care in cleaning up any paint spills or splatters that may occur immediately. The contractor shall warranty the project against water penetrations and workmanship for a period of 5 years. All material cut sheets shall be provided to the Department prior to project and ordering for approval by the Department s project manager. Before ordering paint, the contractor shall present to the project manager color samples to match up the colors for approval. A thorough final walk through with the project manager and job foreman will be conducted the Tuesday after the project has been completed for final acceptance by the Department. Invoices will be paid in full after completion of project. Invoice will be submitted electronically to the Department s project manager upon final walkthrough. MFMP Transaction Fee: All payment(s) to the vendor resulting from this competitive solicitation WILL be subject to the MFMP Transaction Fee in accordance with the attached Form PUR 1000 General Contract Condition #14. However, all vendors should be aware that effective November 1 st, 2015 through July 1 st, 2016, in accordance with Senate Bill 2502-A, the Transaction Fee will change from one percent (1%) to seven-tenths of one percent (.70%) of the payment issued. The Transaction Fees imposed shall be based upon the date of issuance of the payment.
