OBJECTIVE RFP-DOT-13/ TB-RE-ADVERTISEMENT EXHIBIT "A" SCOPE OF SERVICES DISTRICTWIDE CONTAMINATION ASSESSMENT and REMEDIATION SERVICES The Florida Department of Transportation (Department) District Four, requests environmental consultant services on an as-needed basis for the response to, and support of, all phases of Department business with an emphasis on the identification, handling, and remediation of contaminated and/or hazardous materials and substances and in-house staffing support on National Environmental Policy Act (NEPA) project management. Work will involve conducting roadway plan and project reviews, contaminated materials and environmental project management, Phase I and Phase II site assessments, sampling and analytical testing, implementation of various forms of soil and groundwater remediation as required, managing contaminated dewatering effluent, construction of transportation structures and features in contaminated areas, and identifying, removing and managing contaminated materials, buried drums, tanks, or other unknown objects and/or materials determined by the Department to represent an immediate threat to the human or natural environment, lands, or waters of the State of Florida, or for situations deemed necessary by the Department. B. SERVICES REQUIRED OF THE CONSULTANT The Vendor shall provide any of the following elements contained herein, if and when required by the Department: 1. Conduct contamination assessment and sampling; site investigations; pilot tests; database, file, report, various plans and other document reviews, review, modify and prepare remedial designs and submittals, and otherwise execute any work associated with the preparation of the following deliverables: Contamination Assessment Plans (CAP), Contamination Assessment Report (CAR), Contaminated Area Management Plans (CAMP), Remedial Action Plans (RAP), Contamination Impact Assessment Reports (CIAR), Contamination Screening Evaluation Reports (CSER) and/or CSER Addendum, Remediation and Remedial Construction schedules and any other Taskspecific deliverable at the direction of the Department. Be prepared to implement, discuss and define each Deliverable element with the Department, and any other interested party, at the direction of the Department. The required services shall be in accordance with and follow Chapter 376, Florida Statutes, Chapter 62 Florida Administrative Code, and/or current applicable Florida Department of Environmental Protection (FDEP) or United Stated Environmental Protection Agency (USEPA) regulations and with the guidelines found in the Project Development and Environment (PD&E) Manual, Volumes I & II, and as directed by the Department. 2. Prepare and/or review Contamination Screening Evaluation Reports (CSER) and/or CSER Addendum in accordance with the guidelines found in the Project Development and Environment (PD&E) Manual, Volumes I & II, and as directed by the Department. 3. Monitor, sample, analyze, neutralize, assess, delineate, decontaminate, document, store, containerize, transport, manage and/or cleanup any material, spill, leak, release, or condition caused by any material, hazardous waste, contaminated and/or hazardous material and/or substance, regulated substance, or unknown substance, including petroleum products, as such need arises upon request by the Department. 2 4. Excavate, handle, store, manage, transport and dispose of any contaminated and/or hazardous material in accordance with all Local, State and Federal ordinances, rules, regulation and laws. Vendor should perform all analytical testing required to identify the materials as well as facilitate the consequent disposal of the material. In the event a temporary storage area is required to "store" the material, the Vendor shall arrange for the temporary storage area until final disposal arrangements are made and the final disposal of the material is accomplished. The use of Department s right of way as a temporary storage area will be evaluated on a case-by-case basis. 5. Comply with current Local, State and Federal laws, rules, regulations and codes whenever Department work is being performed. All permits and licenses required for this Contract will be obtained by the Vendor and maintained for the duration of the Contract. Prepare all documentation, where required by applicable Local, State and Federal laws, rules, regulations and codes and as directed by the Department. 6. The Vendor shall be available to provide response to a given situation with adequate equipment, personnel and materials within a period of time as specified by the Department upon notice to the Vendor and commensurate with the gravity of the situation. When directed by the Department, either orally or in writing, the Vendor may be required to respond to, or support construction activities where unknown contaminated and/or hazardous materials have been exposed or found which could interfere with construction activities. The Vendor shall be capable of providing services on a twenty-four (24) hour a day, seven (7) days a week, fifty-two (52) weeks a year basis. 7. Own, subcontract or have immediate access to all equipment listed in Exhibit C - Price Proposal. 8. Utilize personnel trained and experienced in the following fields and sciences: civil, chemical, environmental, and mechanical engineering; geology; chemistry; biology; toxicology; microbiology; hydrology; heavy equipment operation; roadway construction; and contaminated and/or hazardous materials safety. Experience in contaminated and/or hazardous materials sampling, analysis, management, transportation and disposal; contaminated and/or hazardous materials and petroleum spill cleanup, site restoration, environmental audits, soil and groundwater remediation, and underground and aboveground tanks (UST/AST) services is required. 9. Provide, as required, part time staffing at the District Four headquarters to support the Planning and Environmental Management work efforts. Specific procedural guidance will be provided by the Department upon assignment of each task. The Vendor will provide support for the following: a) Project Contaminated Materials support activities. This will include, but is not limited to; reviewing plan submittals at each design phase for projects in FDOT s Work Program, including projects submitted by local municipalities and agencies. i. Assessing the potential for project impacts to physical and natural, and resources utilizing database research, including the Efficient Transportation Decision Making (ETDM) Environmental Screening Tool (EST). The Vendor must be able to identify potential impacts to the project from a variety of potential contamination sources as well as identifying impacts to: wetlands, threatened and endangered species, wildlife and habitat, noise, air quality and other environmental features as they relate to the project and determine the need for further coordination with Department staff or regulatory agencies. Performing electronic systems updates such as inputting comments into the Department s Electronic Review Comments (ERC) program, updating the Department s Project Suite system with documents, comments, schedules and timeframes, and updating the Prima Vera program and associated project activities. b) Assisting with contamination assessments and developing language for NEPA reevaluations on previously completed PD&E studies, including the completion of PD&E commitments; c) Reviewing right of way transfer requests for minor projects, including projects submitted by local municipalities and other agencies or entities; d) Developing and/or updating informational or procedural manuals to be utilized by Department s staff, design vendors, construction staff, vendor or subcontractors to facilitate the Department s projects and processes. 10. Furnish the Department with records and reports covering each assignment. Such records and reports shall be furnished in the type, frequency, and form directed by the Department and, where applicable, will also be acceptable to the appropriate regulatory agency or authority. Electronic reports shall be provided in the current District Four software format and be compatible with the current GIS infrastructure and software as needed. The current Department software is: Microstation, Microsoft Office and ESRI products. 11. All analytical laboratory services must be conducted in accordance with the FDEP s Standard Operating Procedures and the all Quality Assurance/Quality Control (QA/QC) will be in accordance with the current FDEP Quality Manual. 12. Standard turnaround time for analytical results shall be ten calendar days (maximum). Rush turnaround time for analytical results shall be no more than 72 hours (maximum). 13. Perform as required, those activities associated with the site preparation, clearing and grubbing, removal of USTs/ASTs, pressure testing of USTs/ASTs, removal of hydraulic lifts, conduct asbestos and/or lead surveys and abatement for facilities located within District Four, demolition incidental to environmental activities, construction or installation of transportation related features such as removal and/or replacement of existing asphalt or concrete pavement; removal, relocation, or replacement of underground utilities, (i.e.: storm drainage systems, water mains, sewer mains, etc.:); the installation and operation of dewatering and effluent treatment systems to facilitate installation of such underground utilities; installation of sheet pile or other barriers to create cofferdams or barriers for the installation of certain construction features in dry conditions; or any other activity required in a construction project that must be accomplished when either contaminated and/or hazardous soil and/or groundwater has been identified in the immediate area and must be remediated whether prior to or concurrent with the construction project. Work on USTs/ASTs may include but is not limited to, abandoned tanks, tanks with unknown contents, propane tanks, used-oil tanks, grease traps, chlorine tanks, and petroleum tanks. Any work performed as stated above will be governed by the applicable version of the Florida Department of Transportation Standard Specifications for Road and Bridge Construction and all applicable construction contract documents including but not limited to Plans, Roadway and Traffic Design Standards, Technical Specifications, and any additional requirements governing such work. 4 Replacement of the Project Manager or Contract Manager from the project will require the Department's prior written approval of whomever the Consultant intends to substitute. Without this prior written approval, the Consultant will be considered in default. The Vendor may utilize company personnel or subcontractors to complete required construction work. The Vendor personnel or subcontractor(s) designated to complete the required construction shall possess a working knowledge of Department specification requirements and a demonstrated record of successfully completed construction work of similar type(s) and description(s). Requests to sublet any portion of the work must be submitted to the Department and a subcontractor may perform no work without prior authorization from the Department. C. LABOR CLASSIFICATIONS The following job classifications have been defined by the DEPARTMENT for this contract: 1. Project Manager/Contract Manager Bachelors or Master s Degree with a minimum of five years of contract management experience. At least five years of experience with FDOT transportation improvement CAR projects, or similar roadway project experience. Experience should include: conducting contaminated and/or hazardous materials assessments and management; conducting and managing environmental studies and resource reviews; managing stakeholder relationships; analyzing project budgets and accounting; planning and scheduling projects; reviewing and editing technical reports; overseeing sub-consultant management; handling documents, reporting, and data QA/QC. 2. Senior - Engineer P.E. / Geologist P.G. Bachelor s or Master s Degree, registered professional with over ten years of postregistration experience and a minimum of five years experience with FDOT transportation CAR projects, or similar roadway project experience. Additional experience with transportation improvement projects with respect to plans review and contaminated and/or hazardous materials avoidance, management and construction is required. 3. Senior Scientist Bachelor s or Master s Degree, with over ten years of discipline specialized experience, and a minimum of five years experience with FDOT transportation CAR projects, or similar roadway project experience. Additional experience with contaminated and/or hazardous materials avoidance, management and construction as related to FDOT transportation improvement CAR projects, or similar roadway project experience is required. 4. Project - Engineer P.E. / Geologist P.G. Bachelor s or Master s Degree, with two or more years of post-registration experience, including a minimum of two additional years of experience with contaminated and/or hazardous materials avoidance, management and construction as related to FDOT transportation CAR projects, or similar roadway project experience is required. Project Scientist Bachelor s or Master s Degree, with six or more years of discipline specialized experience, including a minimum of two additional years of experience with contaminated and/or hazardous materials avoidance, management and construction as related to FDOT transportation improvement CAR projects, or similar roadway project experience is required. 6. Staff Engineer / Geologist / Scientist Bachelor s or Master s Degree, with post graduation experience on FDOT transportation improvement projects or similar roadway project experience. Experience with contaminated and/or hazardous materials avoidance, management and construction as related to roadway CAR projects is preferred, and/or experience surveying for environmental features such as wetlands, listed flora and fauna, historical structures, socio-cultural issues and contamination issues. Must have a basic understanding of construction plans, Geographic Information Systems (GIS)/Global Positioning Systems (GPS). 7. Certified Industrial Hygienist (CIH) Bachelor s or Master s Degree, American Board of Industrial Hygiene (ABIH) registration (or equivalent), with four or more years of post-registration experience and experience with FDOT transportation improvement CAR projects or similar roadway project experience. 8. Administrative/Clerical High School diploma with a minimum of two (2) years experience with administrative writing and reporting and Microsoft Office software 9. Field Tech/ CAD Tech / GIS Tech Technical Certificate or Associates Degree, with at least one (1) year of post graduation experience on FDOT transportation improvement projects or similar roadway project experience. Must have a basic understanding of roadway construction plans. GIS Tech requires knowledge and experience in Geographic Information Systems (GIS)/Global Positioning Systems (GPS) interface and mapping experience. Field Tech requires at least one year experience in contaminated and/or hazardous materials assessment and testing, as related to FDOT transportation improvement or similar roadway projects and experience surveying and sampling for contamination issues and other environmental features such as wetlands, listed flora and fauna, historical structures, socio-cultural issues. OSHA 40-hour HAZWOPR, is required and any other associated environmental health and safety certifications. 10. Field Foreman Five or more years experience in contaminated and/or hazardous materials management and supervising and coordinating CAR and construction work crews on construction projects as related to FDOT transportation improvement projects, or similar roadway project experience. OSHA 40-hour HAZWOPR is required, 8-hr HAZWOPER Supervisor training is preferred, and any other associated environmental health and safety certifications. Must have a basic understanding of roadway construction plans. A-5

Equipment Operator Two or more years experience with operation of equipment in contaminated and/or hazardous materials areas on construction projects as related to FDOT transportation improvement projects or similar roadway project experience is required. Performs a variety of semi-skilled and skilled work in the operation of light, medium and some heavy construction equipment and light to heavy duty trucks. OSHA 40-hour HAZWOPR is required and any other associated environmental health and safety certifications. A-6
