SUITE 299 WINSTON-SALEM, NC June 10, 2015 Re: Detailed Description of Project 1215 Brookstown Avenue Winston-Salem, NC Scope of the Project ILLUSTRATION 1 The 1,412 s.f. home at 1215 Brookstown Avenue in Winston-Salem was constructed in the Craftsman, residential style. A previous owner sub-divided the home s interior and added a second entrance door on the Brookstown elevation of the home. A masonry addition was also constructed at the rear of the original house along with a utility shed. Mr. Caldwell intends to repair and restore the home s exterior, demolish the masonry addition, and add two dormers positioned near the rear of the house. The dormers are designed to increase the usable area in the home s attic space. Also, the new construction includes an L shaped terrace that provides an out-of-doors gathering area/automobile parking, new retaining walls and landscaping, and a new storage building. Specifically, the renovation will include the following items: 1. Landscaping/Site Alterations a. Shrubs and Trees ILLUSTRATION 1+2 Evaluate the health of existing trees and shrubs. The tree depicted in illustration 2 appears to be diseased and dying. If that is the case, it will be removed and replace with a sapling of the same species. Other trees/landscaping to be evaluated and maintained as required. b. Walls, Walks and Steps from Brookstown Ave. to the front porch No change at this time c. Wall parallel to the alley ILLUSTRATION Repair low retaining wall at the edge of the alley and replace the missing portion of the wall. The new wall will be approximately 36 high above the finished elevation of the new terrace. 2. Major Demolition ILLUSTRATION 4, 6,+9 Remove building addition, storage building, and retaining wall at the north side of the original house. See new design images for proposed alterations and new construction.

6 3. Exterior Alterations ILLUSTRATION 8,10, a. Doors i) Remove existing doors located on the south elevation and replace with a single door/sidelight that is consistent with the Craftsman architectural style. ii) Add new door on the north elevation (from the new kitchen to the exterior terrace) that is consistent with the Craftsman architectural style. iii) Remove two existing doors on the north elevation and replace with a single door that is consistent with the Craftsman architectural style. b. Windows The exterior window locations and sizes were altered one or more times during the course of the home s history. The design proposal includes the removal of all replacement windows, the reinstallation of simulated divided lights (SDL) windows throughout whose mullion patterns are consistent with the Craftsmen architectural style. Restore i) Replace exterior windows on the south elevation and restore the original sizes to the extent possible. ii) Replace the pair of windows on the east elevation at the M. Bedroom with a single window and reposition the window. iii) Reposition window locations on the north elevation at the M. Bedroom and Bathroom. iv) Replace one window on the east elevation and reposition three new windows on the east elevation. 4. New Construction or Addition 5. ILLUSTRATION 8,10,11+12 a. Site b. House i) Add new terrace on the east and north elevations of the home for outside seating and parking. ii) Add new brick retaining wall(s) and steps to replace the existing retaining walls and create a landscaped area on the north elevation. iii) Add a new storage shed at the north west corner of the existing home i) Add new dormer at the north segment of the existing hip roof. The dormers roof pitch will match the existing roof, Craftsman style windows, and the exterior finishes of the dormer will match the existing house. ii) Remove hazardous chimney and relocate to the center of the new living room space
