Wherein this Addendum conflicts with the original Specifications and/or Drawings, this Addendum shall govern. PROJECT MANUAL ITEM NO. 1. SECTION REQUEST FOR PROPOSAL - RECOMMENDED PRE-BID CONFERENCE DRAWINGS a. Attached to this addendum is copy of the Pre-Bid Conference Agenda and Roster Sheet. ITEM NO. 2. DRAWING DME 2.1 MECHANICAL/ELECTRICAL DEMOLITION PLAN a. Photos of existing cooling tower on roof are attached for Bidder's reference since no access was provided during Pre-Bid Conference. b. Concord Elementary School Contractor to remove and reinstall Owner's Helitech waterproofing system that currently is located on three sides of existing sump tank. Owner preference is reinstall drainage panels between sump and concrete wall but relocation to existing condition is acceptable. ATTACHMENTS 1. Pre-Bid Meeting Agenda 2. Project Information Recommended Pre-Bid Meeting Agenda 1. Bid Date is Thursday, April 21, at 2:30 p.m., CDT at the McCracken County District Central Office, 5347 Benton Road, Paducah, Kentucky at which time all bids will be publicly opened and read aloud. 2. Copies of bid documents may be obtained from Paducah Blueprint & Supply Co., Inc., Paducah, Kentucky for a refundable amount of $40.00 and a non-refundable amount of $ A shipping charge of $25.00 shall be payable to Paducah Blueprint & Supply Co., Inc Addenda items need to be submitted to the Architect no later than 5:00 p.m. CDT on Thursday, April 14, Introduction of attending a. District Superintendent: Quin Sutton b. Facility Director: Todd Jackson c. School Principal: Marc Mavigliano d. Architect: Paul King, PFGW Inc. 5. Inform Bidders of all addenda to-date. 6. No changes are binding unless in written form. 7. Reminder that conditional or qualified bids are not acceptable. 8. The official bid document must be submitted in a sealed envelope clearly indicating Sealed Bid Proposal. It is the bidder s responsibility to submit bid on time. Late bids will not be recognized. 9. Make sure all blanks on all pages are completed signed and notarized if necessary including the authentication of bid page. Failure to do so invalidates the bid.

3 April 12, 2016 Pre-Bid Meeting Agenda Page 2 of A 5% bid bond for the bid amount presented in the proposal. Only a satisfactory Bid Bond will be accepted by the Owner. 11. This project is a lump sum bid for demolition and construction for TOTAL SCOPE of work. 12. Wage Rates do apply as the total project scope is greater than $250, The List of Proposed Subcontractors shall be completed in its entirety and submitted with the bid. A bidder cannot change subcontractors during or after a bid opening or review. 14. Bidder hereby agrees to commence work under this contract on or before a date to be specified in the Notice to Proceed and to be substantially complete within ninety (90) calendar days for the Work. Bidder further agrees to pay as liquidated damages as scheduled below for each calendar days thereafter until project completion as provided in accordance with Specifications. 15. Liquidated Damages Schedule: a. Substantial Completion occurring after 90 calendar days shall be $250 per day. b. Final Completion occurring after 15 calendar days shall be $250 per day. 16. The apparent, successful low bidder will receive a Notice of Award. Bonds are required on Project Manual forms or comparable form supplied by insurance carrier. The contractor has 7 days to submit this paperwork to the Architect. Work shall not be started until contract has been executed. 17. The following bonds, forms and insurance shall be required in order to award a contract: a. Performance Bond. b. Labor and Material Payment Bond. c. Insurance Certificate. 18. Insurance requirements for the project are listed in the General Conditions Amendment. B. Scope, Contract Time and Tour 1. Brief review of the scope of work and Contract Time. 2. Special Note of Work Restrictions in Summary Section. 3. Site tour and questions. C. All questions answered and/or clarifications provided during the Pre-Bid Meeting shall be included in a subsequently issued addendum.
