GENERAL NOTES 1. ALL CONSTRUCTION INDICATED ON THESE PLANS SHALL BE IN ACCORDANCE WITH: THE STANDARD SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION (RI STANDARD SPECIFICATIONS), AMENDED 2010, WITH ALL REVISIONS. THE AMERICAN ASSOCIATION OF STATE HIGHWAY AND TRANSPORTATION OFFICIALS (AASHTO) LRFD BRIDGE CONSTRUCTION SPECIFICATIONS, 3rd, EDITION, 2010, INCLUDING THE LATEST INTERIM REVISIONS. 2. EXISTING DETAILS, DIMENSIONS, AND ELEVATIONS SHOWN ON THESE PLANS HAVE BEEN OBTAINED FROM EXISTING DRAWINGS AND ARE NOT GUARANTEED. THE CONTRACTOR SHALL BE RESPONSIBLE FOR VERIFYING THE EXISTING CONDITIONS PRIOR TO CONSTRUCTION. ANY DISCREPANCIES REQUIRING MODIFICATIONS TO THESE PLANS SHALL BE BROUGHT TO THE ATTENTION OF THE ENGINEER. INFORMATION NOT SHOWN ON THE PLANS SHALL BE OBTAINED FROM THE EXISTING PLANS AND/OR FIELD VERIFICATION. 3. THE EXISTING UTILITIES SHOWN ON THE PLANS ARE APPROXIMATE AND MAY BE INCOMPLETE. ABRASIVE BLAST CLEANING, FIELD PAINTING, AND DISPOSAL OF LEAD DEBRIS AS APPLICABLE SHALL BE IN ACCORDANCE WITH ALL FEDERAL, STATE, AND LOCAL REGULATIONS. 6. NO LIVE LOADS SHALL BE PRESENT ON THE BRIDGE DIRECTLY OVER THE AFFECTED GIRDERS DURING GIRDER HEAT STRAIGHTENING AND STEEL REPAIR OPERATIONS. 7. THE CONTRACTOR SHALL TAKE ALL NECESSARY PRECAUTIONS RELATED TO TEMPERATURE SENSITIVE WORK. NO EXTRA PAYMENT OR TIME EXTENSION WILL BE GRANTED FOR ADHERENCE TO THIS REQUIREMENT. 8. BASED ON THE AGE OF THE BRIDGE, IT IS MORE THAN LIKELY TO CONTAIN LEAD, CHROMIUM, OR OTHER HAZARDOUS MATERIALS WHICH SHALL BE CONSIDERED DURING THE CONTRACTORS MEANS AND METHODS FOR PERFORMING THE WORK REQUIRED UNDER THIS CONTRACT. DESIGN DATA 1. DESIGN SPECIFICATIONS THE AASHTO LRFD BRIDGE DESIGN SPECIFICATIONS, 6th EDITION, 2012, INCLUDING ALL INTERIM REVISIONS TO DATE. THE LRFD BRIDGE DESIGN MANUAL 2007 EDITION INCLUDING ALL REVISIONS TO DATE. ALL OTHER APPLICABLE DESIGN SPECIFICATIONS ARE REFERENCED IN SECTION 1 OF THE LRFD BRIDGE DESIGN MANUAL DATED THE STANDARD SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION (RI STANDARD SPECIFICATION), AMENDED 2010, WITH ALL REVISIONS. IN CASE OF CONFLICT, THE LRFD BRIDGE DESIGN MANUAL SHALL GOVERN. 2. LIVE LOADS THE DESIGN VEHICULAR LIVE LOAD SHALL BE THE HL-93 DESIGNATION ADJUSTED FOR DYNAMIC LOAD ALLOWANCE AND MULTIPLE PRESENCE FACTOR. THE DESIGN PEDESTRIAN LIVE LOAD SHALL BE 75 PSF. MATERIALS STRUCTURAL STEEL: STRUCTURAL STEEL NOTES (CONTINUED) 5. ALL AASHTO M 270 (ASTM A 709) STRUCTURAL STEEL USED FOR STEEL REPAIRS, SHALL MEET THE ZONE 2 LONGITUDINAL CHARPY V-NOTCH TEST OF 15 FT.-LBS. AT 40 F AS SPECIFIED IN TABLE S1 "TOUGHNESS REQUIREMENTS" AS PER AASHTO DESIGNATION M 223 (ASTM DESIGNATION A 572) OF THE AASHTO STANDARD SPECIFICATIONS FOR TRANSPORTATION MATERIALS AND METHODS OF SAMPLING AND TESTING. SAMPLING AND TESTING PROCEDURES SHALL BE IN ACCORDANCE WITH AASHTO T 243 (ASTM DESIGNATION A 673). THE (H) FREQUENCY OF HEAT TESTING SHALL BE USED. 6. STRUCTURAL STEEL SHALL BE PREPARED AND PAINTED IN ACCORDANCE WITH THE RI STANDARD SPECIFICATIONS. THE COLOR OF TOP COAT SHALL MATCH EXISTING PAINT TO REMAIN. 7. ALL HIGH STRENGTH BOLTS SHALL CONFORM TO AASHTO DESIGNATION M 164 (ASTM DESIGNATION A 325). 8. NUTS MEETING AASHTO STANDARDS M 291 (ASTM DESIGNATION A 563) ARE TO BE USED. 9. BOLTS, NUTS AND WASHERS SHALL BE GALVANIZED IN ACCORDANCE WITH AASHTO STANDARDS M 232 (ASTM DESIGNATION A 153). 10. WASHERS MEETING AASHTO STANDARDS M 293 (ASTM DESIGNATION F 436) ARE TO BE USED OVER ALL HOLES THAT ARE MORE THAN 1/16" IN DIAMETER GREATER THAN THE BOLT DIAMETER AND UNDER ALL PARTS TURNED DURING ASSEMBLY. 11. PRIOR TO FABRICATION, ALL MATERIALS SHALL BE BLAST-CLEANED TO AT LEAST SSPC-SP6 TO REMOVE ALL OIL, DIRT, GREASE, MILL SCALE AND OTHER DELETERIOUS MATERIALS FROM THE SURFACES OF THE STEEL TO BE FABRICATED. 12. PRIOR TO SHOP COATING AS SPECIFIED IN SECTION 825 OF THE RI STANDARD SPECIFICATIONS, ALL CORNERS AND EDGES OF STEEL WHICH HAVE BEEN FLAME CUT OR OTHERWISE HARDENED SHALL BE SOFTENED BY GRINDING OR BLAST-CLEANING TO PROVIDE A SURFACE SUITABLE FOR APPLICATION OF THE SPECIFIED PAINT SYSTEM. 13. WELDING OF ATTACHMENTS TO GIRDER FLANGES OR WEBS FOR CONSTRUCTION PURPOSES IS NOT PERMITTED EXCEPT WHEN APPROVED BY THE ENGINEER. HEAT STRAIGHTENING NOTES 1. REFER TO SPECIAL PROVISION ITEM CODE , "HEAT STRAIGHTENING OF COLLISION DAMAGED BEAM/GIRDER - ", FOR GIRDER REPAIR REQUIREMENTS. 2. THE CONTRACTOR SHALL SUBMIT DRAWINGS, DESIGN CALCULATIONS, AND A CONSTRUCTION SEQUENCE DETERMINING THE APPROXIMATE NUMBER AND SIZE OF HEAT PATTERNS AND MAGNITUDE AND LOCATION OF EXTERNALLY APPLIED AUXILIARY FORCES REQUIRED FOR HEAT STRAIGHTENING FOR REVIEW AND APPROVAL BY THE ENGINEER, PRIOR TO COMMENCING CONSTRUCTION. 3. THE CONTRACTOR SHALL RETAIN THE SERVICES OF A CERTIFIED TESTING AGENCY TO CONDUCT VISUAL AND NON-DESTRUCTIVE TESTING OF EXISTING WELDS AND CRACKS. MAGNETIC PARTICLE OR ULTRASONIC TESTING WILL BE REQUIRED TO ACCURATELY DETERMINE THE LIMITS OF STEEL AND WELD REPAIR AREAS. 4. APPLICATION OF TEMPORARY COMPRESSIVE STRESSES DUE TO EXTERNALLY APPLIED AUXILIARY FORCES, SHALL NOT EXCEED THAT WHICH WOULD RESULT FROM A MOMENT EQUIVALENT TO ONE HALF THE PLASTIC MOMENT CAPACITY OF THE ELEMENT, AT THE MAXIMUM TEMPERATURE PRODUCED BY HEATING, TO WHICH THE LOAD IS APPLIED. 5. HEATING SHALL BE CONFINED TO LINE, VEE, AND RECTANGULAR (STRIP) HEAT PATTERNS TO WITHIN THE LIMITS OF BEAM/GIRDER MISALIGNMENT (REPAIR AREA) AS SHOWN ON THESE DRAWINGS AND SHALL BE CONDUCTED SO AS TO BRING THE STEEL TO A TEMPERATURE BETWEEN 1,000 AND 1,150 DEGREES FAHRENHEIT AS RAPIDLY AS POSSIBLE WITHOUT OVERHEATING THE STEEL. 6. THE STEEL TEMPERATURE SHALL BE MONITORED DURING THE HEAT STRAIGHTENING PROCESS WITH TEMPERATURE INDICATING CRAYONS MANUFACTURED FOR 600 DEGREES FAHRENHEIT, 1,000 DEGREES FAHRENHEIT, 1,150 DEGREES FAHRENHEIT, AND 1,250 DEGREES FAHRENHEIT. 7. ANY HEATING PROCEDURE WHICH CAUSES A PORTION OF THE STEEL TO BE HEATED TO A TEMPERATURE GREATER THAN 1,250 DEGREES FAHRENHEIT SHALL BE CONSIDERED DESTRUCTIVE HEATING. DESTRUCTIVE HEATING SHALL AUTOMATICALLY CAUSE THE REJECTION OF WORK. REPAINTING OF EXISTING STRUCTURAL STEEL (CONTINUED) 4. THE WORK ASSOCIATED WITH SETTING UP, MAINTAINING AND REMOVING THE CONTAINMENT SYSTEM FOR THE SURFACE PREPARATION AND THE REPAINTING OF EXISTING STRUCTURAL STEEL, MUST BE CONDUCTED IN ACCORDANCE WITH THE PROPOSED MAINTENANCE AND PROTECTION OF TRAFFIC PLANS AND THE TIME RESTRICTIONS SPECIFIED IN THE CONTRACT DOCUMENTS. 5. ALL EXISTING STRUCTURAL STEEL DESIGNATED TO BE REPAINTED SHALL BE CLEANED WITH ABRASIVE BLAST CLEANING. 6. ANY COMPONENT NOT DESIGNATED TO BE REPAINTED SHALL BE ADEQUATELY PROTECTED FROM CLEANING OPERATIONS. 7. ANY DAMAGE RESULTING FROM THE CONTRACTOR'S CLEANING OR REPAINTING OPERATIONS, SHALL BE REPAIRED BY THE CONTRACTOR TO THE SATISFACTION OF THE ENGINEER AT NO ADDITIONAL COST TO THE STATE. 8. THE REPAINTING OF THE GIRDER SHALL NOT COMMENCE UNTIL AFTER THE COMPLETION OF HEAT STRAIGHTENING AND THE INSTALLATION OF THE NEW STEEL REPAIRS. 9. BASED ON THE AGE OF THE BRIDGE IT IS MORE THAN LIKELY TO CONTAIN LEAD, CHROMIUM OR OTHER HAZARDOUS MATERIALS WHICH SHALL BE CONSIDERED DURING THE CONTRACTORS MEANS AND METHODS FOR PERFORMING THE WORK REQUIRED UNDER THIS CONTRACT. NO ADDITIONAL TIME OR PAYMENT WILL BE MADE TO THE CONTRACTOR FOR REMOVING, HANDLING AND DISPOSING OF THESE MATERIALS. PAINTING OF NEW STRUCTURAL STEEL 1. THE NEW STRUCTURAL STEEL FOR REPAIRS SHALL BE PAINTED TO THE LIMITS INDICATED ON THE PLANS. NEW STRUCTURAL STEEL SHALL BE PREPARED AND PRIMED IN THE SHOP AND THE TOP COATS OF PAINT APPLIED IN THE FIELD AFTER INSTALLATION, ALL IN ACCORDANCE WITH SECTION 825 OF THE RI STANDARD SPECIFICATIONS. THE COLOR OF THE TOP COAT SHALL MATCH THE EXISTING COLOR OF THE BRIDGE. THE PAINT CHOSEN FOR THE TOP COAT SHALL HAVE A PAINT CHIP AND SPECIFICATIONS SUBMITTED TO THE ENGINEER IN SUFFICIENT TIME TO PERMIT CAREFUL CHECKING PRIOR TO PAINTING. UTILITIES 1. LOCATION OF EXISTING UTILITIES SHOWN ARE APPROXIMATE. THE CONTRACTOR SHALL VERIFY LOCATIONS OF ALL UTILITIES, SHOULD THEY AFFECT ANY CONSTRUCTION OPERATIONS. THE "DIG SAFE" POLICY SHALL ALSO BE FOLLOWED. ANY DAMAGE TO EXISTING UTILITIES SHOWN ON THE PLANS OR LOCATED BY DIG SAFE SHALL BE THE CONTRACTORS RESPONSIBILITY. COST OF DAMAGE TO THESE UTILITIES SHALL BE BORNE BY THE CONTRACTOR. 2. CONTRACTOR SHALL ENSURE THAT ALL EXISTING UTILITIES ARE ADEQUATELY SHIELDED AND PROTECTED FROM DAMAGE AND TEMPORARILY SUPPORTED AS REQUIRED. 3. TO PROVIDE WATER FROM FIRE HYDRANTS FOR CONSTRUCTION PURPOSES, PROVIDENCE WATER SUPPLY ISSUES FIRE HYDRANT METER PERMITS TO QUALIFIED APPLICATIONS NEEDING WATER FOR CONSTRUCTION PROJECTS. SUGGESTED SEQUENCE OF CONSTRUCTION 1. REMOVE PAINT WITHIN THE LIMITS OF REPAIR (REFER TO PAINTING RELATED SPECIAL PROVISION). 2. IF REQUIRED, REMOVE DIAPHRAGMS FOR HEAT STRAIGHTENING. 3. HEAT STRAIGHTEN GIRDER AND WEB STIFFENERS. 4. CONDUCT NON-DESTRUCTIVE TESTING TO ENSURE THE STEEL HAS NOT BEEN PERMANENTLY DISTORTED, FRACTURED, OR OVERHEATED (REFER TO SPECIAL PROVISION ITEM CODE ). 5. INSTALL STRUCTURAL STEEL REPAIRS AT THE LOCATIONS SPECIFIED IN THESE PLANS. 6. REPAINT NEW AND EXISTING STRUCTURAL STEEL WITHIN THE LIMITS OF REPAIR (REFER TO PAINTING RELATED SPECIAL PROVISIONS). AASHTO DESIGNATION M 270 (ASTM DESIGNATION A 709), GRADE 36 (M270 GRADE 50 IS AN ACCEPTABLE SUBSTITUTE). STRUCTURAL STEEL NOTES 8. STEEL REJECTED DUE TO DESTRUCTIVE HEATING AND/OR EXTERNALLY APPLIED AUXILIARY FORCES SHALL BE SUBJECTED TO THE SAME RESTRICTIONS AND PROCEDURES AS NOTED IN THE RIDOT STANDARD SPECIFICATIONS FOR CONTRACTOR'S RESPONSIBILITY FOR WORK. ALL REPLACEMENT, REPAIRS, TESTING, AND DELAY COSTS SHALL BE BORNE BY THE CONTRACTOR. 1. FRAMING DIMENSIONS ARE GIVEN ALONG CENTERLINES OF GIRDERS AND ALONG CENTERLINES OF BEARINGS ON ABUTMENTS AND PIERS. 2. THE SHOPS FABRICATING THE STRUCTURAL STEEL FOR GIRDER REPAIRS MUST BE CERTIFIED FOR "SIMPLE STEEL BRIDGES (SBR)" IN ACCORDANCE WITH THE AISC QUALITY CERTIFICATION PROGRAM OR EQUIVALENT. THE SHOPS SHALL ALSO BE CERTIFIED UNDER THE AISC "SOPHISTICATED PAINT ENDORSEMENT (SPE)" QUALITY PROGRAM OR THE SSPC-QP3 PAINT CERTIFICATION PROGRAM. THE FABRICATOR MUST SUBMIT PROOF OF CURRENT CERTIFICATION AS SPECIFIED. 3. STRUCTURAL STEEL SHAPES AND PLATES SHALL CONFORM TO THE LATEST PROVISIONS OF AASHTO DESIGNATION M 270 (ASTM DESIGNATION A 709) GRADE 36 (M270 GRADE 50 IS AN ACCEPTABLE SUBSTITUTE). 4. SHOP DRAWINGS FOR ALL FABRICATED STEEL SHALL BE SUBMITTED TO THE ENGINEER IN SUFFICIENT TIME TO PERMIT CAREFUL CHECKING PRIOR TO FABRICATION. ALL NEW STEEL REPAIRS SHALL BE CAREFULLY AND THOROUGHLY FIELD MEASURED PRIOR TO FABRICATION TO ENSURE SNUG FIT AGAINST ALL SURFACES AND PROPER ALIGNMENT AS SHOWN ON THE PLANS. REPAINTING OF EXISTING STRUCTURAL STEEL 1. THE EXISTING STRUCTURAL STEEL SHALL BE REPAINTED TO THE LIMITS SHOWN ON THE PLANS IN ACCORDANCE WITH THE RI STANDARD SPECIFICATIONS, SPECIAL PROVISION ITEM CODE THE COLOR OF THE TOP COAT SHALL MATCH THE EXISTING COLOR OF THE BRIDGE. THE PAINT CHOSEN FOR THE TOP COAT SHALL HAVE A PAINT CHIP AND SPECIFICATIONS SUBMITTED TO THE ENGINEER IN SUFFICIENT TIME TO PERMIT CAREFUL CHECKING PRIOR TO PAINTING IN ACCORDANCE WITH RI STANDARD SPECIFICATIONS SECTION M.06 PAINT. 2. THE EXISTING STRUCTURAL STEEL TO BE REPAINTED SHALL INCLUDE, BUT NOT BE LIMITED TO, GIRDERS, STIFFENERS, AND FASTENERS. 3. PERSONNEL AND ENVIRONMENTAL PROTECTION, AND CONTAINMENT, COLLECTION, STORAGE AND DISPOSAL OF DEBRIS AND SPENT MATERIALS, SHALL CONFORM TO SECTION 826 OF THE RI STANDARD SPECIFICATIONS, EXCEPT PAYMENT.
