9 FORM OF PROPOSAL 1.01 Submitted by:, 20 (Contracting Firm) (Date) 1.02 Deliver To: The Unified Government of Wyandotte County Purchasing Division 701 North 7 th Street, Room 532 Kansas City, Kansas Telephone: (913) Having read the Specifications and examined the Drawings for the Minnesota Avenue Lot C Walkway Repair and having also received, read and taken into account Addenda through number and likewise having inspected the site and the conditions affecting and governing said Project, the undersigned hereby proposes to furnish all materials and to perform all labor, as specified and described in the said Specifications and/or as shown on the said Drawings for all work necessary to complete the Project on a timely basis and in accordance with the Contract Documents, regardless of whether expressly provided for in such Specifications and Drawings Before completing the Form of Proposal, the Undersigned represents that it has carefully reviewed the Notice to Bidders, Instructions to Bidders, Owner-Contractor Agreement, General Conditions, Supplementary Conditions, and Specifications, and understands these documents. Failure to comply with provisions of the Contract Documents may be cause for disqualification of bid Bonds and Contract: If the Undersigned is notified of bid acceptance, it agrees to furnish required bonds as indicated in Instructions to Bidders Completion of Work: In submitting a bid, the undersigned agrees to execute and complete the Owner- Contractor Agreement in the form included in the Contract Documents and to complete its work within 100 consecutive calendar days after the receipt of Notice to Proceed. 13 DIVISION 1 - GENERAL REQUIREMENTS These documents are intended to support the standard front end requirements as written by the Unified Government of Wyandotte County (UGWYCO). In the case of any conflict or incongruence, the UGWYCO documents shall govern. Reference to General Conditions means reference to the UGWYCO general conditions that are part of the contract documents SUMMARY OF WORK 1.01 GENERAL PROJECT DESCRIPTION A. Base Bid - Demolition of the existing pedestrian bridge and bridge enclosure over the alley, removal of the overhead canopy structure and all existing handrails and guardrails on the site. New construction includes a new replacement bridge with galvanized steel guardrails and handrails that will span the alley. Lighting for the bridge and replacement of stairway lighting in the existing lamp locations along the upper and lower ramp runs and beneath new benches shall be included. The planter box at the south end and bollards at the south and north ends are to be included in the base bid. Concrete saw cutting and replacement new concrete topping as specified. Replace exist door frame and hardware. Additionally the goal is to be able to construct the project in a manner that provides for an efficient addition of an overhead structure that will cover both the upper and lower walks at a later date. Alternate A Add Alternate to provide removal and replacement of guardrail for length indicated on drawings in lieu of painting existing guardrail. Alternate B Add Alternate to construct a new steel canopy, roofing, and lighting as indicated on drawings in addition to footings and base plates for canopy structure. Alternate C Add Alternate to provide polycarbonate canopy material and hardware in lieu of corrugated metal canopy and hardware. Alternate D.1 Add Alternate to provide painted steel handrails and guardrails for the entire site in lieu of galvanized rails. Alternate D.2 Add Alternate to provide stainless steel handrails and guardrails for the entire site in lieu of galvanized rails. B. TESTING 1. All other testing and inspections shall be performed by testing firms hired by and paid for by the Unified Government of Wyandotte County. See the various specification sections for testing requirements DESCRIPTION OF WORK A. The following is a brief description of work. Unified Government of Wyandotte County DIVISION 1-1 GENERAL REQUIREMENTS

14 1. Demolition and concrete cleaning 2. Rough In electrical and lighting 3. Replacement concrete paving and concrete surface treatments 4. New steel handrails and guardrails 1.03 PROJECT SCHEDULE A. Prepare a detailed construction procedure and schedule and submit it to the Architect for approval. Such procedure and schedule must be approved in writing by the Architect prior to the start of construction work. See General Conditions. B. Completion of the work within the time frame allotted is critical to the project and the schedule will be strictly adhered to. Contractor shall be responsible for the expediting of the fabrication and delivery of materials and equipment and shall coordinate delivery of same with the approved construction schedule to allow for completion within the time period specified in the Form of Proposal. 1. It is recognized that the work can be unavoidably affected or influenced by governing regulations, natural phenomena including weather conditions and other forces outside the Contract Documents. However, every effort must be made to keep the project on schedule due to the firm deadline established by the Owner for this particular phase of the work MISCELLANEOUS PROVISIONS A. Performance Requirements for Completed Work: Provide the final and completed project complete and ready for use in every respect by the completion date specified herein. 1. Compliance with all applicable governing regulations, codes and standards is intended and required for the work and for the Owner's occupancy and utilization. 2. In addition to the requirement that every element of the work comply with applicable requirements of the Contract Documents, it is also required that the work as a whole comply with all applicable industry standards and governing codes and regulations ALLOWANCES Allowances for Products 1. Include in the Contract Sum all allowances stated in the Contract Documents. 2. The amount of each allowance includes: a. The cost of the products to the Contractor, less any applicable trade discounts. b. Delivery to the site. c. Applicable Taxes. 15 3. In addition to the amount of each allowance, include in the Contract Sum the Contractor's cost for: a. Handling at the site. b. Protection from the elements and from damage. c. Labor for installation and finishing. d. Other expenses required to complete the installation. e. Contractor's and Subcontractor's overhead and profit. 4. Architect's Duties: a. Consult with Contractor in consideration of products and suppliers. b. Make selection in consultation with Owner designating: 1) Product model and finish. 2) Accessories and attachments. 3) Supplier. 4) Cost to Contractor delivered to the site. 5) Manufacturer's warranties. 5. Contractor's Duties a. Assist Architect and Owner in determining qualified supplier. b. Obtain proposals from suppliers when requested by Architect. c. Make appropriate recommendations for consideration of Architect. d. Notify Architect Promptly of: 1) Any reasonable objections Contractor may have against any supplier under consideration. 2) Any effect on Construction Schedule anticipated by selections under consideration. 6. Contractor Responsibility for Purchase, Delivery and Installation. a. On notification of selection, execute purchase agreement with designated supplier. b. Arrange for and process shop drawings, product data, and samples as required. c. Make all arrangements for delivery. d. Upon delivery, promptly inspect products for damage or defects. e. Submit claims for transportation damage. f. Install and finish products in compliance with requirements of referenced specification sections. 7. Adjustment of Costs a. Should the net cost be more or less than the specified amount of allowance, Contract Sum will be adjusted accordingly by change order. 1) The amount of the Change Order will recognize any changes in handling costs at the site, labor, installation costs, overhead, profit and other expenses caused by the selection under the allowance. If no allowance dollars are approved for use by the Owner, a deductive change order will be executed for the unused amounts. 2) For products specified under a unit cost allowance, the unit cost shall apply to quantities actually used with a nominal allowance for waste as determined by receipted invoices or by field measurement. b. Submit claims for anticipated additional costs at the site or other expenses caused by the selection under the allowance, prior to Unified Government of Wyandotte County DIVISION 1-3 GENERAL REQUIREMENTS

16 execution of the work. c. Failure to submit claims will constitute a waiver of claims for additional costs. d. At the contract closeout, reflect approved changes in contract amounts in the final statement of accounting. 8. Schedule a. Section , Door Hardware: Provide $1, lump sum allowance for hardware materials ALTERNATES A. Definitions and Explanations: "Alternates" are defined as alternate products, materials, equipment, systems, methods, units of work or major elements of the construction, which may, at Owner's option and under terms established by Instructions to Bidders and in the Contract or Agreement, be selected for the work in lieu of corresponding requirements of Contract Documents. Selection may occur prior to Contract date, or may, by the Agreement, be deferred for possible selection at a subsequent date. Alternates may or may not change scope and general character of the work substantially. Requirements of this section may be related to, but must not be confused with, requirements of Contract Documents related to "allowances", "unit prices", "change orders", "substitutions" and similar provisions. 1. Refer to the Contract, and subsequent modifications thereof, for determination of which several scheduled "Alternates" herein have been accepted, and, therefore, are in full force and effect as though included originally in the contract documents for the base bid. 2. The Owner reserves the right to accept or reject any Alternate at the time of awarding the Contract. If, during the progress of the Work, it should become desirable to reinstate any Alternate not included in the Contract, the Owner reserves the right to reinstate the Alternate at the price bid by the Contractor provided that such actions taken in sufficient time as not to delay the progress of the work. B. Notification: Immediately following the award of the Contract, prepare and distribute to each entity to be involved in performance of the work, a notification of the status of each alternate scheduled herein and including those subsequently added by notification during bidding. Indicate which alternates have been: 1) accepted, 2) rejected, and, 3) deferred for consideration at a later date as indicated. Include full description of negotiated modifications to alternates, if any. C. General: The descriptions herein for each alternate are recognized to be incomplete and abbreviated, but imply that each change must be complete for the scope of work affected. Refer to all other applicable specification sections and to applicable drawings, for specific requirements of the work, regardless of whether references are so noted in the description of each alternate. It is recognized that descriptions of alternates are primarily scope definitions, and do not necessarily detail full range of materials and processes needed to complete the work as required. 17 D. Schedule of Alternates Alternate A Add Alternate to provide removal and replacement of guardrail for length indicated on drawings in lieu of painting existing guardrail. Alternate B Add Alternate to construct a new steel canopy, roofing, and lighting as indicated on drawings in addition to footings and base plates for canopy structure. Alternate C Add Alternate to provide polycarbonate canopy material and hardware in lieu of corrugated metal canopy and hardware. Alternate D.1 Add Alternate to provide painted steel handrails and guardrails for the entire site in lieu of galvanized rails. Alternate D.2 Add Alternate to provide stainless steel handrails and guardrails for the entire site in lieu of galvanized rails PROJECT MEETINGS A. Contractor shall schedule and administer weekly progress meetings and periodically called meetings throughout the progress of the work. 1. Contractors Responsibilities Include: a. Prepare agenda for meetings. b. Make physical arrangements for meetings. c. Record minutes; include all significant proceedings and decisions. d. Reproduce and distribute copies of minutes; to all participants in meeting, to all parties affected by decisions made in meeting, to Architect. 2. Suggested Agenda a. Review, approval of minutes of previous meeting. b. Review of work progress since previous meeting. c. Field observations, problems, conflicts. d. Problems which impede Construction Schedule. e. Review of off-site fabrication, delivery schedules. f. Corrective measures and procedures to regain projected schedule. g. Revisions to Construction Schedule. h. Plan progress, schedule, during succeeding work period. i. Coordination of schedules. j. Review submittal schedules; expedite as required. k. Maintenance of quality standards. l. Review proposed changes for effect on Construction Schedule and on completion date CONSTRUCTION SCHEDULES A. Promptly after award of the Contract, prepare and submit an estimated Unified Government of Wyandotte County DIVISION 1-5 GENERAL REQUIREMENTS

18 construction progress schedule for the Work, with sub schedules of related activities which are essential to its progress. This Section supplements paragraph 4.10 of the General Conditions. B. Prepare schedules in the form of a horizontal bar chart. 1. Provide separate horizontal bar for each trade or operation. 2. Horizontal Time Scale: Identify the first work day of each week. 3. Scale and Spacing: To allow space for notations and future revisions. 4. Sheet Size: 11 x 17". C. Construction Progress Schedule 1. Show the complete sequence of construction by activity SHOP DRAWINGS, PRODUCT DATA, SAMPLES A. This Section includes procedures for processing: 1. Shop drawings. 2. Product data. 3. Office samples. 4. Mock-up samples. 5. Certificate of compliance. B. Submittals as approved do not constitute a change order. C. Submit all submittals under Contractor's Transmittal letter. Include the following: 1. Project by title. 2. Contractor's contract number. 3. Work and products by Specification Section and Article number. D. Re-submittals: When Architect requires that a submittal be resubmitted, comply with requirements of this section. 1. Identify changes made since the previous submittal. E. Notify Architect in writing at time of submittal of deviations from the requirements of the Contract Documents. F. Make all submittals far enough in advance of scheduled dates for installation to provide sufficient time for reviews, for securing necessary approvals, for possible revisions and re-submittals, and for placing orders and securing delivery. 1. Architect's Review Time: In scheduling, allow at least 10 working days for Architect's review. 2. Delays caused by the tardiness of the Contractor in preparing and forwarding submittals will not be an acceptable basis for an extension of the Contract completion date or for consideration of alternate products which Unified Government of Wyandotte County DIVISION 1-6 GENERAL REQUIREMENTS

19 G. Shop Drawings do not meet the specified requirements of this Project Manual. 1. Each sheet of Shop Drawings shall be stamped and signed by Contractor before submitting to Architect. Certify compliance with requirements of Contract Documents. 2. Review by the Architect shall not relieve Contractor fron his responsibility in preparing and submitting proper Shop Drawings in accordance with his current obligations. 3. All submissions which, in the opinion of the Architect, are incomplete, contain errors or have not been checked or only superficially checked, will be returned unchecked by the Architect for resubmission. 4. Fabrication of products or start of work before required Shop Drawings are approved by Architect and returned to Contractor shall be at Contractor's risk. 5. Contractor shall be required to prepare coordination drawings and fully coordinate drawings with other trades and contiguous work. Working drawings for coordination shall be submitted as directed by the Architect. Architect reserves the right to require the submittal and/or approval of composite coordination drawings prior to the release of an individual Contractor's Shop Drawing and/or fabrication drawing. H. Product Data 1. Submit only pages which are pertinent. a. Mark each copy of standard printed data to identify pertinent products, referenced to Specification Section and Article number. b. Show reference standards, performance characteristics, and capacities; wiring and piping diagrams and controls; component parts; finishes; dimensions; and required clearances. 2. Modify manufacturer's standard schematic drawings and diagrams to supplement standard information and to provide information specifically applicable to the work. Delete information not applicable. 3. Stamp and sign each set of manufacturer's product data before submitting to Architect to certify compliance with Contract Documents. 4. Number of Copies Required: Submit the number of copies which are required to be returned plus two copies which will be retained by the Architect, but not less than six in any case. 5. Upon approval, one copy of the approved data will be returned to the Contractor for his file. Contractor should submit additional sets above the minimum if he desires more than one copy with the Architect's comments. I. Samples 1. Submit full range of manufacturer's standard finishes except when more restrictive requirements are specified, indicating colors, textures and patterns for Architect's selection. 2. Submit samples to illustrate functional characteristics of products, including Unified Government of Wyandotte County DIVISION 1-7 GENERAL REQUIREMENTS

20 parts and attachments. J. Mock-Up Samples: Where mock-up samples and similar samples are indicated in the individual specifications sections, comply with requirements for "Concrete Samples", and process transmittal forms for mock-ups to provide a record of activity. Mock up samples will be required for the concrete treatments; 2 - for the vertical concrete and 2 - for the flatwork to show blending of color and texture to the existing concrete. K. Certificates of Compliance: Contractor shall submit "Certificates of Compliance" certifying that all materials used in the work comply with all specified provisions thereof. 1. Submit in the form of a letter or company standard forms. 2. If test reports are submitted with "Certificates of Compliance". test reports shall include data or dates of testing and results of testing. L. Test Reports: Test reports certified by an independent testing laboratory must be made available upon request by Architect MATERIAL AND EQUIPMENT A. Products 1. Products include materials, equipment and systems. 2. Products incorporated into the work: a. Comply with specifications and referenced standards as minimum requirements. b. Comply with size, make, type, and quality specified, or as specifically accepted in writing by the Architect. B. Transportation and Handling 1. Arrange deliveries of products in accordance with construction schedules, coordinate to avoid conflict with work and conditions at the site. 2. Transport products by methods to avoid product damage. 3. Deliver products in undamaged condition, in manufacturer's original containers or packaging, with identifying labels intact and legible. 4. Immediately on delivery, inspect shipments to assure compliance with requirements of Contract Documents and accepted submittals, and that products are properly protected and undamaged. C. Storage and Protection 1. Store products in accordance with manufacturer's instructions with seals and labels intact and legible. a. Store products subject to damage by the elements in weathertight enclosures. b. Maintain temperature and humidity within the ranges required by Unified Government of Wyandotte County DIVISION 1-8 GENERAL REQUIREMENTS

21 manufacturer's instructions. 2. Exterior Storage a. Store fabricated products above the ground, on blocking or skids, to prevent soiling or staining. Cover products which are subject to deterioration with impervious coverings. Provide adequate ventilation to avoid condensation. b. Store loose granular materials in a well-drained area on solid surfaces to prevent mixing with foreign materials. 3. Arrange storage in a manner to provide access for inspection. Make periodic inspections of stored products to assure that products are maintained under specified conditions, and free from damage. 4. Protection After Installation: Provide coverings as necessary to protect installed products from damage from traffic and subsequent construction operations. Remove when no longer needed. D. Material Safety Data Sheets: Contractor to maintain Material Safety Data Sheets at the project site. SECTION SELECTIVE DEMOLITION PART 1 - GENERAL 1.1 RELATED DOCUMENTS A. Drawings and general provisions of the Contract, including General and Supplementary Conditions and other Division 1 Specification Sections, apply to this Section. 1.2 SUMMARY A. This Section includes the following: 1. Demolition and removal of selected portions of a building or structure. 2. Demolition and removal of selected site elements. 3. Repair procedures for selective demolition operations. B. Related Sections include the following: 1. Division 1 Section "Summary" for use of the premises and phasing requirements. 2. Division 1 Section "Construction Progress Documentation" for preconstruction photographs taken before selective demolition. 3. Division 1 Section "Photographic Documentation" for preconstruction photographs taken before selective demolition. 4. Division 16 Sections for demolishing, cutting, patching, or relocating electrical items. 1.3 DEFINITIONS A. Remove: Detach items from existing construction and legally dispose of them offsite, unless indicated to be removed and salvaged or removed and reinstalled. B. Remove and Salvage: Detach items from existing construction and deliver them to Owner. Owner has the first right of refusal on items to be disposed of. C. Remove and Reinstall: Detach items from existing construction, prepare them for reuse, and reinstall them where indicated. D. Existing to Remain: (See 1.3 B above) Existing items of construction that are not to be removed and that are not otherwise indicated to be removed, removed and salvaged, or removed and reinstalled. 1.4 MATERIALS OWNERSHIP A. Except for items or materials indicated to be reused, salvaged, reinstalled, or otherwise indicated to remain Owner's property, demolished materials shall become Contractor's property and shall be removed from Project site. 1.5 SUBMITTALS A. Proposed Dust and Nose Control Measures: Submit statement or drawing that indicates the measures proposed for use, proposed locations, and proposed time frame for their operation. Identify options if proposed measures are later determined to be inadequate. B. Schedule of Selective Demolition Activities: Indicate the following: 1. Detailed sequence of selective demolition and removal work, with starting and ending dates for each activity. Ensure Owner's functions are uninterrupted. 2. Interruption of utility services. 3. Coordination for shutoff, capping, and continuation of utility services. C. Inventory: After selective demolition is complete, submit a list of items that have been removed and salvaged. D. 23 E. Landfill Records: Indicate receipt and acceptance of hazardous wastes by a landfill facility licensed to accept hazardous wastes. 1.6 QUALITY ASSURANCE A. Demolition Firm Qualifications: An experienced firm that has specialized in demolition work similar in material and extent to that indicated for this Project. B. Professional Engineer Qualifications: Comply with Division 1 Section "Quality Requirements." C. Regulatory Requirements: Comply with governing EPA notification regulations before beginning selective demolition. Comply with hauling and disposal regulations of authorities having jurisdiction. D. Standards: Comply with ANSI A10.6 and NFPA 241. E. Pre-demolition Conference: Conduct conference at Project site for Project Management and Coordination. Retain paragraph above if requirements in other Division 1 Sections are adequate for Project. Retain paragraph below if additional requirements are necessary; include information about conference. Identify specific participants not mentioned in Division 1. F. Review methods and procedures related to selective demolition including, but not limited to, the following: 1. Inspect and discuss condition of construction to be selectively demolished. 2. Review structural load limitations of existing structure. 3. Review and finalize selective demolition schedule and verify availability of materials, demolition personnel, equipment, and facilities needed to make progress and avoid delays. 4. Review requirements of work performed by other trades that rely on substrates exposed by selective demolition operations. 5. Identify coordination and scheduling of demo and construction work that will affect the traffic in the alley with the UGWYCO. Provide signage and notices as directed by UGWYCO. 1.7 PROJECT CONDITIONS A. Conduct selective demolition so Owner's operations or neighboring operations will not be disrupted. Provide not less than 48 hours' notice to Owner and neighboring business owners of activities that will affect their operations. B. Maintain access to existing walkways, circulation, and other adjacent occupied or used facilities. 1. Do not close or obstruct walkways, circulation, or other occupied or used facilities without written permission from authorities having jurisdiction. 2. Provide and maintain necessary guardrails, barricades and signage for the Unified Government of Wyandotte County DIVISION 1-11 GENERAL REQUIREMENTS

24 safety of those in the adjacent garage and the grounds surrounding the site. A. Owner assumes no responsibility for condition of areas to be selectively demolished. 1. Conditions existing at time of inspection for bidding purpose will be maintained by Owner as far as practical. 2. Before selective demolition, Owner will remove the following items: a. To be determined at construction kick-off meeting B. Hazardous Materials: It is not expected that hazardous materials will be encountered in the Work. 1. Hazardous materials will be removed by Owner before start of the Work. 2. If materials suspected of containing hazardous materials are encountered, do not disturb; immediately notify Architect and Owner. Hazardous materials will be removed by Owner under a separate contract. C. Hazardous Materials: Hazardous materials are present in building to be selectively demolished. A report on the presence of hazardous materials is on file for review and use. Examine report to become aware of locations where hazardous materials are present. 1. Hazardous material remediation is specified elsewhere in the Contract Documents. 2. Do not disturb hazardous materials or items suspected of containing hazardous materials except under procedures specified elsewhere in the Contract Documents. D. Storage or sale of removed items or materials on-site will not be permitted. E. Utility Service: Maintain existing utilities indicated to remain in service and protect them against damage during selective demolition operations. 1. Maintain fire-protection facilities in service during selective demolition operations. PART 2 - PRODUCTS 4.1 REPAIR MATERIALS A. Use repair materials identical to existing materials. 1. If identical materials are unavailable or cannot be used for exposed surfaces, use materials that visually match existing adjacent surfaces to the fullest extent possible. 2. 25 B. Comply with material and installation requirements specified in individual Specification Sections. PART 3 - EXECUTION 5.1 EXAMINATION A. Verify that utilities have been disconnected and capped. B. Survey existing conditions and correlate with requirements indicated to determine extent of selective demolition required. C. Inventory and record the condition of items to be removed and reinstalled and items to be removed and salvaged. D. When unanticipated mechanical, electrical, or structural elements that conflict with intended function or design are encountered, investigate and measure the nature and extent of conflict. Promptly submit a written report to Architect. E. Engage a professional engineer to survey condition of building to determine whether removing any element might result in structural deficiency or unplanned collapse of any portion of structure or adjacent structures during selective demolition operations. F. Perform surveys as the Work progresses to detect hazards resulting from selective demolition activities. 5.2 UTILITY SERVICES A. Existing Utilities: Maintain services indicated to remain and protect them against damage during selective demolition operations. B. Do not interrupt existing utilities serving occupied or operating facilities unless authorized in writing by Owner and authorities having jurisdiction. Provide temporary services during interruptions to existing utilities, as acceptable to Owner and to authorities having jurisdiction. 1. Provide at least 72 hours notice to Owner if shutdown of service is required during changeover. C. Utility Requirements: Locate, identify, disconnect, and seal or cap off indicated utilities serving areas to be selectively demolished. 1. Owner will arrange to shut off indicated utilities when requested by Contractor. 2. If utility services are required to be removed, relocated, or abandoned, before proceeding with selective demolition provide temporary utilities that bypass area of selective demolition and that maintain continuity of service to other parts of building. 26 3. Cut off pipe or conduit in walls or partitions to be removed. Cap, valve, or plug and seal remaining portion of pipe or conduit after bypassing. 5.3 PREPARATION A. Dangerous Materials: Drain, purge, or otherwise remove, collect, and dispose of chemicals, gases, explosives, acids, flammables, or other dangerous materials before proceeding with selective demolition operations. B. Temporary Facilities: Provide temporary barricades and other protection required to prevent injury to people and damage to adjacent buildings and facilities to remain. 1. Provide protection to ensure safe passage of people around selective demolition area and to and from occupied portions of building. 2. Protect walls, ceilings, floors, and other existing finish work that are to remain or that are exposed during selective demolition operations. C. Temporary Partitions: Erect and maintain dustproof partitions and temporary enclosures to limit dust and dirt migration and to separate areas from fumes and noise. 5.4 POLLUTION CONTROLS A. Dust Control: Use temporary enclosures, and other suitable methods to limit spread of dust and dirt. Comply with governing environmental-protection regulations. 1. Do not use water when it may damage existing construction or create hazardous or objectionable conditions, such as ice, flooding, and pollution. 2. Wash down paved areas where construction traffic has entered and exited the site daily. B. Disposal: Remove and transport debris in a manner that will prevent spillage on adjacent surfaces and areas. 1. Remove debris from elevated portions of building by chute, hoist, or other device that will convey debris to grade level in a controlled descent. C. Cleaning: Clean adjacent structures and improvements of dust, dirt, and debris caused by selective demolition operations. Return adjacent areas to condition existing before selective demolition operations began. 5.5 SELECTIVE DEMOLITION A. General: Demolish and remove existing construction only to the extent required by new construction and as indicated. 27 1. Neatly cut openings and holes plumb, square, and true to dimensions required. Use cutting methods least likely to damage construction to remain or adjoining construction. Use hand tools or small power tools designed for sawing or grinding, not hammering and chopping, to minimize disturbance of adjacent surfaces. Temporarily cover openings to remain. 2. Cut or drill from the exposed or finished side into concealed surfaces to avoid marring existing finished surfaces. 3. Do not use cutting torches until work area is cleared of flammable materials. At concealed spaces, such as duct and pipe interiors, verify condition and contents of hidden space before starting flame-cutting operations. Maintain fire watch or portable fire-suppression devices during flame-cutting operations. 4. Maintain adequate ventilation when using cutting torches. 5. Locate selective demolition equipment and remove debris and materials so as not to impose excessive loads on supporting walls, floors, or framing. 6. Dispose of demolished items and materials promptly. 7. Return elements of construction and surfaces that are to remain to condition existing before selective demolition operations began. B. Existing Facilities: Comply with building manager's requirements for using and protecting elevators, stairs, walkways, loading docks, building entries, and other building facilities during selective demolition operations. C. Removed and Reinstalled Items: Comply with the following: 1. Clean and repair items to functional condition adequate for intended reuse. 2. Reinstall items in locations indicated. Comply with installation requirements for new materials and equipment. Provide connections, supports, and miscellaneous materials necessary to make item functional for use indicated. 5.6 PATCHING AND REPAIRS A. General: Promptly repair damage to adjacent construction caused by selective demolition operations. B. Patching: Comply with Division 1 Section "Cutting and Patching." C. Repairs: Where repairs to existing surfaces are required, patch to produce surfaces suitable for new materials. 1. Completely fill holes and depressions in existing masonry walls that are to remain with an approved masonry patching material applied according to manufacturer's written recommendations. D. Finishes: Restore exposed finishes of patched areas and extend restoration into adjoining construction in a manner that eliminates evidence of patching and refinishing. E. Floors and Walls: Where walls or partitions that are demolished extend one finished area into another, patch and repair floor and wall surfaces in the new space. 28 floor and wall coverings and replace with new materials, if necessary, to achieve uniform color and appearance. 1. Patch with durable seams that are as invisible as possible. Provide materials and comply with installation requirements specified in other Sections of these Specifications. 2. Where patching occurs in a painted surface, apply primer and intermediate paint coats over patch and apply final paint coat over entire unbroken surface containing patch. Provide additional coats until patch blends with adjacent surfaces. 3. Where feasible, test and inspect patched areas after completion to demonstrate integrity of installation. F. Ceilings: Not applicable 5.7 DISPOSAL OF DEMOLISHED MATERIALS A. General: Promptly dispose of demolished materials. Do not allow demolished materials to accumulate on-site. B. Disposal: Transport demolished materials off Owner's property and legally dispose of them CUTTING AND PATCHING A. Execute cutting, fitting or patching of Work, required to: 1. Make several parts fit properly. 2. Uncover Work to provide for installation of ill-timed Work. 3. Remove and replace defective Work. 4. Remove and replace Work not conforming to requirements of Contract Documents. 5. Remove samples of installed Work as specified for testing. 6. Install specified Work in existing construction. Patch with durable seams that are as invisible as possible. Provide materials and comply with installation requirements specified in other Sections of these Specifications. 7. Where patching occurs in a painted surface, apply primer and intermediate paint coats over patch and apply final paint coat over entire unbroken surface containing patch. Provide additional coats until patch blends with adjacent surfaces. 8. Where feasible, test and inspect patched areas after completion to demonstrate integrity of installation. B. In addition to contract requirements, upon written instructions of Architect: 1. Uncover Work to provide for Architect's observation of covered Work. 2. 29 C. Each Contractor shall be responsible for and pay all cost involved with cutting and patching that may be required to complete the work in his Contract with the following stipulations: 1. General contractor shall provide cutting and infill work for other trades only when indicated on General Trades Drawings. 2. All cutting and patching shall be done by craftsmen skilled in type of work involved. 3. Each Contractor shall furnish the General Contractor with, and be responsible for, exact location and size of all holes and openings required to be cut or necessary for his work. Cost of cutting the hole shall be borne by the Contractor requiring the hole or opening. D. Do not endanger any work by cutting or altering work or any part of it. E. Do not cut or alter Work of another Contractor without written consent of Architect CLOSEOUT PROCEDURES PART 1 - GENERAL 6.1 RELATED DOCUMENTS A. Drawings and general provisions of the Contract, including General and Supplementary Conditions and other Division 1 Specification Sections, apply to this Section. 6.2 SUMMARY A. This Section includes administrative and procedural requirements for contract closeout, including, but not limited to, the following: 1. Inspection procedures. 2. Project Record Documents. 3. Operation and maintenance manuals. 4. Warranties. 5. Instruction of Owner's personnel. 6. Final cleaning. B. Related Sections include the following: 1. Division 1 Section "Payment Procedures" for requirements for Applications for Payment for Substantial and Final Completion. 2. Division 1 Section "Construction Progress Documentation" for submitting Final Completion construction photographs and negatives. 30 3. Divisions 2 through 16 Sections for specific closeout and special cleaning requirements for products of those Sections. 6.3 SUBSTANTIAL COMPLETION A. Preliminary Procedures: Before requesting inspection for determining date of Substantial Completion, complete the following. List items below that are incomplete in request. 1. Prepare a list of items to be completed and corrected (punch list), the value of items on the list, and reasons why the Work is not complete. 2. Advise Owner of pending insurance changeover requirements. 3. Submit specific warranties, workmanship bonds, maintenance service agreements, final certifications, and similar documents. 4. Obtain and submit releases permitting Owner unrestricted use of the Work and access to services and utilities. Include occupancy permits, operating certificates, and similar releases. 5. Prepare and submit Project Record Documents, operation and maintenance manuals, Final Completion construction photographs, damage or settlement surveys, property surveys, and similar final record information. 6. Deliver tools, spare parts, extra materials, and similar items to location designated by Owner. Label with manufacturer's name and model number where applicable. 7. Make final changeover of permanent locks and deliver keys to Owner. Advise Owner's personnel of changeover in security provisions. 8. Complete startup testing of systems lighting and confirmation of drainage system operation. 9. Test/adjust/balance records Not applicable. 10. Terminate and remove temporary facilities from Project site, along with mockups, construction tools, and similar elements. 11. Advise Owner of changeover in heat and other utilities. 12. Submit changeover information related to Owner's occupancy, use, operation, and maintenance. 13. Complete final cleaning requirements, including touchup painting. 14. Touch up and otherwise repair and restore marred exposed finishes to eliminate visual defects. B. Inspection: Submit a written request for inspection for Substantial Completion. On receipt of request, Architect will either proceed with inspection or notify Contractor of unfulfilled requirements. Architect will prepare the Certificate of Substantial Completion after inspection or will notify Contractor of items, either on Contractor's list or additional items identified by Architect, that must be completed or corrected before certificate will be issued. 1. Re-inspection: Request re-inspection when the Work identified in previous inspections as incomplete is completed or corrected. 2. 31 6.4 FINAL COMPLETION A. Preliminary Procedures: Before requesting final inspection for determining date of Final Completion, complete the following: 1. Submit a final Application for Payment according to Division 1 Section "Payment Procedures." 2. Submit certified copy of Architect's Substantial Completion inspection list of items to be completed or corrected (punch list), endorsed and dated by Architect. The certified copy of the list shall state that each item has been completed or otherwise resolved for acceptance. 3. Submit evidence of final, continuing insurance coverage complying with insurance requirements. 4. Pest Control Not required. 5. Instruct Owner's personnel in operation, adjustment, and maintenance of products, equipment, and systems. B. Inspection: Submit a written request for final inspection for acceptance. On receipt of request, Architect will either proceed with inspection or notify Contractor of unfulfilled requirements. Architect will prepare a final Certificate for Payment after inspection or will notify Contractor of construction that must be completed or corrected before certificate will be issued. 1. Re-inspection: Request re-inspection when the Work identified in previous inspections as incomplete is completed or corrected. 6.5 PROJECT RECORD DOCUMENTS A. General: Do not use Project Record Documents for construction purposes. Protect Project Record Documents from deterioration and loss. Provide access to Project Record Documents for Architect's reference during normal working hours. B. Record Drawings: Maintain and submit one set of blue- or black-line white prints of Contract Drawings and Shop Drawings. 1. Mark Record Prints to show the actual installation where installation varies from that shown originally. Require individual or entity who obtained record data, whether individual or entity is Installer, subcontractor, or similar entity, to prepare the marked-up Record Prints. a. Give particular attention to information on concealed elements that cannot be readily identified and recorded later. b. Accurately record information in an understandable drawing technique. c. Record data as soon as possible after obtaining it. Record and check the markup before enclosing concealed installations. d. Mark Contract Drawings or Shop Drawings, whichever is most capable of showing actual physical conditions, completely and accurately.
