Page 1 of 2 To control stormwater from new development and redevelopment projects throughout the Borough of Roselle (including projects we operate) we will do the following: We currently ensure that all new residential development and redevelopment projects are subject to the Residential Site Improvement Standards for stormwater management (including the NJDEP Stormwater Management Rules, N.J.A.C. 7:8, referenced in those standards) are in compliance with those standards. Our planning and zoning boards ensure such compliance before issuing preliminary or final subdivision or site plan approvals under the Municipal Land Use Law. Since the EDPS, the Borough of Roselle has not constructed any new development or redevelopment projects on Borough property. We ensure adequate long-term operation and maintenance of BMPs for developer's projects by requiring a project maintenance plan similar to the maintenance plan described in our stormwater control ordinance, and by requiring and funding the implementation of that plan. We also require any storm drain inlets that we install to comply with the design standard in Attachement C of our permits. A new ordinance is under consideration by the Roselle Borough Council regarding Private Developments and is anticipated to be adopted in the summer of We ensure such operation and maintenance for any new development or redevelopment projects on our property by complying with the maintenance requirements in ordinance. In addition, any storm drain inlets we install comply with that ordinance's standard for such inlets. Our municipal planning and legal officials have reviewed and incorporated the Municipal Stormwater Management Plan and Stormwater Control Ordinance in the NJ Stormwater BMP Manual, into the municipal stormwater management plan and municipal stormwater control ordinance for the Borough. We have coordinated with the County Planning agency staff regarding revisons to said documents.

Page 2 of 2 The stormwater control ordinance, which is administered by our planning and zoning boards, Borough Engineer and Code Enforcement Office, controls stormwater from non-residential development and redevelopment projects. Where it is necessary to implement the municipal stormwater management plan, the ordinance also controls aspects of residential development and redevelopment projects that are not subject to the Residentail Site Improvements Standards. For any BMP that is installed in order to comply with the requirements of our post-construction program, the Borough of Roselle ensures adequate long-term operation as well as preventative and corrective maintenance (including replacement) of BMPs. For BMPs on private propety that we do not own or operate, the Borough of Roselle enforces a provision in the municipal stormwater control ordinance that requires the private entity to perform the operation and maintenance, with penalties if the private entity does not comply. If, for example, the private entity does not perform the required maintenance, the Borough can perform the maintenance and charge the private entity. The Borough of Roselle also enforces through the municipal stormwater control ordinance, compliance with the desgin standard in Attachmetn C of our permit to control passage of solid and floatable materials through storm drain inlets. Such compliance will be achieved by installation of the NJDOT bicycle safe grate and (if needed) a curb opening with a clear space no bigger than two inches across the smallest dimension.

Be specific on how you will distribute your educational information, and how you will conduct your annual event. The local public education program focuses on providing the public with infomation on the impact of stormwater discharges to surface and ground waters of the Borough and State. As part of the Tier A permit, the Borough of Roselle shall annually conduct education activites that total a minimum of 10 points as stated within Attachment E of the permit. This shall include, but is not limited to the following: The Borough of Roselle is in the process of updating their municipal website. A link shall be added to (1 point) The Borough of Roselle shall present a stormwater display and materials at the Annual Borough Street Fair. In addition, a stormwater display is currently active at the Municipal Building (2 points). The Borough of Roselle currently distributes a municipal produced equivalent of the NJDEP stormwater information for our Annual Distribution by mailto our local residents and businesses. The brochure is distributed with our municipal newletter/paper called the "Round Up". Additional copies are available at the municipal building (210 Chestnut Street) (3 points). The Borough of Roselle is in the process of developing educational material that shall be advertised on their local television channel (Channel 34) specific to stormwater discharge. Multiple advertisements shall utilized from the NJDEP material. We will incorporate the information available from as part of the television advertisements (2 points each/maximum of 4 points per year).

6 SPPP Form 5 Storm Drain Inlet Labeling :Borough of Roselle County Union NJPDES # :NJG PI ID #: Team Member/Title: Lou Williams, DPW Superintendent Date of Completion: February 28, 2005 Date of most recent update: June 1, 2010 Storm Drain Inlet Labeling Describe your storm drain inlet labeling program, including your labeling schedule, the details of your long-term maintenance plan, and plans on coordinating with watershed groups or other volunteer organizations. Our Public Works Department has labeled the storm drain inlets. All storm drain inlets along municipal streets with sidewalks, all inlets within plazas, parking areas and maintenance yards that the Borough of Roselle operates are currently labeled. The plastic labels utilized were from both supplied from Clearwater Technology and Almetek. The labels were applied using adhesive. During our annual catch basin cleaning program, we continue to check these labels to ensure that they are still visible, and if they are not, we replace the labels immediately.

Who will prepare your map (e.g., municipal employees, a consultant, etc.)? The Borough of Roselle previously retained Pennoni Associates, Consulting Engineers, to prepare a Roselle Storm Sewer Map and Outfall Map. All storm sewers, streams and outfalls were mapped and shown on the plan. This work was completed on September 13, 2006 (as delineated on said plans). As per the September 29, 2006 Storm Sewer Inspection Report prepared by Pennoni Associates Inc., data points were collected and provided on GPS/GIS data. The data was collected to allow accurate collection and efficiency while locating storm outfall structures throughout the Borough. Stated within the report, the Borough of Roselle maintains a total of 173 outfalls, of which 99 are located on the Wet Brook and 74 are located on Plum Brook. In addition, as per the report, there are a total of 772 municipal catch basins within the Borough of Roselle. The Borough's Consulting Muncipal Engineer, shall investigate the outfall pipes previously located for general maintenance, illicit connections, dry weather flow, outfall pipe stream scour. These investigations shall be performed as part of the Borough of Roselle's Illicit Connection Elimination Program and Outfall Pipe Stream Scouring Remediation Program during the summer of Should areas of concern be identified, the DPW will be informed for immediate repairs and maintenance.

An initial physical inspection of all of our outfall pipes were performed in The Borough's Consulting Muncipal Engineer, shall investigate the outfall pipes previously located for general maintenance, illicit connections, dry weather flow, outfall pipe stream scour. These investigations shall be performed as part of the Borough of Roselle's Illicit Connection Elimination Program and Outfall Pipe Stream Scouring Remediation Program during the summer of Should areas of concern be identified, the DPW will be informed for immediate repairs and maintenance. We will use the DEP Illicit Connection Inspection Report Form to conduct these inspections, and each of the forms will be kept with our SPPP records. Outfall pipes that are found to have a dry weather flow or evidence of an intermittent non-stormwater flow will be rechecked again, tracked to the source and locate the illicit connection. If we are able to locate the illicit connection (and the connection is within the Borough of Roselle), we will cite the responsible party for being in violation of our Illicit Connection Ordinance, and we will have the connection eliminated immediately. If, after the appropriate amount of investigation, we are unable to locate the source of the illicit connection, we will submit the Closeout Investigation Form with our Annual Inspection and Recertification. If an illicit connection is found to originate from another public entity, Roselle Borough will report the illicit connection to the NJDEP. The Borough of Roselle has a DPW telephone hotline that is currently used for reporting spills and illegal dumping. This hotline will also be made available for reporting illicit connections.

10 SPPP Form 9 Yard Waste Ordinance/Collection Program :Borough of Roselle County Union NJPDES # :NJG PI ID #: Team Member/Title: Lou Williams, Public Works Superintendent Date of Completion: March 29, 2005 Date of most recent update: June 1, 2010 Please describe your yard waste collection program. Be sure to include the collection schedule and how you will notify the residents and businesses of this schedule. Attach additional pages as necessary. We have considered the two options available and have decided to develop a yard waste collection and disposal program instead of just adopting and enforcing an ordinance that prohibits placing non-containerized yard waste in the street. We conduct weekly collections of branches, leaves, garden debris and grass during the months of April through October. Additional leaf collections will occur in November and December. The above materials must be placed in bio-degradable bags or a 30 gallon container and placed behind the curb. During the remainder of the year, the Borough of Roselle may hold additional yard waste collections, but no schedule has been determined for these. During the months when we are having collections, we will post our collection schedule and our ordinance requirements on our telephone hotline. The collection schedule will be mailed to the businesses and residents, adertised on our local cable TV channel and will be posted and available at the Borough Clerk's and Public Work's offices. To develop a collection schedule, we have divided the Borough into five wards, giving ourselves one day each week to do collections in each ward. The five wards are shown on a Borough map. (See attached map.) The Borough of Roselle also has adopted and is enforcing a yard waste ordinance (see SPPP Form 10) that prohibits all yard wastes from being placed at the curb or along the street more than seven days prior to our scheduled collections, unless they are bagged or otherwise containerized. The ordinance will also prohibit the placing of yard waste closer than 10 feet from any storm sewer inlet along the street, unless they are bagged or otherwise containerized.

(NOTE: Attach a street sweeping log containing the following information: date and area swept, # of miles swept and the total amount of materials collected.) The Borough of Roselle has evalued all of its streets to determine which area will need to be swept monthly. The Borough of Roselle intends on maintaining its existing street sweeping program for all streets, which includes sweeping all streets once a week. Each ward is swept on one day per week. Enclosed is a ward map of Roselle. The Borough of Roselle's sweeping log is attached. Road Erosion Control Maintenance Describe your Road Erosion Control Maintenance Program, including inspection schedules. A list of all sites of roadside erosion and the repair technique(s) you will be using for each site should be attached to this form. (NOTE: Attach a road erosion control maintenance log containing the following information: location, repairs, date) The Borough of Roselle utilizes the Public Works Department to monitor their road and streets for erosion problems during normal patrols. All identified road erosion problems will be reported problems will be reported to the DPW Superintendent. During the SPPP Team Meetings, identified areas of erosion will be discussed and repairs prioritized. All maintenance personnel will then be assigned to the areas of concern, and the areas identified to have road erosion problems will be repaired in accordance with the Standards for Soil Erosion and Sediment Control in New Jersey. All maintenance personnel will maintain an inspection log and the DPW Superintendent will maintain a list of all repairs and the dates completed. The status of Road Erosion Control Maintenance Program will be included in the Annual Report and Recertification.

Attach a map/diagram or additional pages as necessary. The Borough of Roselle will continue to perform an annual Catch Basin Cleaning Program in order to maintain catch basin function and efficiency. All catch basins will continue to be inspected and cleaned at a minimum of once per year. If, at the time of the inspection, no sediment, trash or debris is observed in the catch basin, then the catch basin will not be cleaned. All catch basins will be inspected yearly, even if they were found to be "clean" the previous year. At the time of the cleaning, the catch basins will also be inspected for proper function. Maintenance will be scheduled for those catch basins that are in disrepair. Please describe your stormwater facility maintenance program for cleaning and maintenance of all stormwater facilities operated by the municipality. Attach additional pages as necessary. (NOTE: Attach a maintenance log containing information on any repairs/maintenance performed on stormwater facilities to ensure their proper function and operation.) The Borough of Roselle implements a Stormwater Facility Maintenance Program to ensure that all stormwater facilities operated by the Borough function properly. Roselle operates the following: - Catch Basins/Storm Drains - Storm Drains - Swales - Detention basins - Flap gates These stormwater facilities will be inspected annually to ensure that they are functioning properly. In high-risk areas, preventative maintenance will be performed on all stormwater facilities to ensure that they do not begin to fail.

15 SPPP Form 14 - Outfall Pipe Stream Scouring Remediation :Borough of Roselle County: Union NJPDES # :NJG PI ID #: Team Member/Title: Lou Williams, Public Works Superintendnet Date of Completion: March 1, 2005 Date of most recent update: June 1, 2010 Describe your stormwater outfall pipe scouring detection, remediation and maintenance program to detect and control active, localized stream and stream bank scouring. Attach additional pages as necessary. (NOTE: Attach a prioritized list of sites observed to have outfall pipe stream and stream bank scouring, date of anticipated repair, method of repair and date of completion.) When we are doing the Illicit Connection Inspection part of this program, the Borough of Roselle investigates the outfall pipes for signs of scouring and erosion. All sites will be placed on a priority list and repairs will be made in accordance with the Standards for Soil Erosion and Sediment Control in New Jersey. In addition, repairs that do not need NJDEP permits for those repairs may be done first. We follow each repair up with an annual inspection of the site to ensure that scouring has not resumed. Attached is a remediation log form for all sites with outfall pipe stream scouring, the date we plan on repairing the scouring, and the method of repair we will use. When repairs are completed, we will note the date of that repair on this form.

If your current storage practices do not meet the de-icing material storage SBR describe your construction schedule and your seasonal tarping interim measures. If you plan on sharing a storage structure, please include its location, as well as a complete list of all concerned public entities. If you store sand outdoors, describe how it meets the minimum standard. The Borough of Roselle currently stores its de-icing salt in a permanent structure located at its maintenance yard located on Columbus Avenue near Twelfth Avenue. The structure has a permanent foundation floor and is walled and roofed. At the completion of loading and unloading activities we shall inspect for spilled salt. The permanent structure shall have maintenance and inspections performed on a regular basis.

Maintenance Yard Operations Public Works employees (Field training will include the SOPs for fueling, vehicle and equipment maintenance, general good housekeeping also for de-icing materials storage)
