The deck surface was chain dragged in 1999 and was found to have approximately 1700 square feet of delaminated or unsound deck. Approximately 4% of the total deck surface is defective. This amount of unsound concrete would constitute a Condition State 3 rating and an NBI code rating of 5. During the 2004 inspection, it was noted that there is approximately 600 square feet of concrete deck patches and an additional 10 square feet of delaminated concrete located in the west bound lane of span STRIP SEAL JOINT LF N/A N/A 144 LF N/A N/A New waterproof expansion joints were placed at the abutments in The strip seals were full of dirt and debris; however there were no signs of leakage at the bridge seat & 2006 joint opening and inspection notes; West Minnesota End - WBL - the joint opening was 1 1/4 inches wide across the entire length. EBL - the joint opening was 1 1/2 inches wide across the entire length. East Wisconsin End - WBL - the joint opening was 1 1/2 inches wide across the entire length. EBL - the joint opening was 3 inches wide across the entire length. 301 POURED DECK JOINT ,530 LF 6, N/A N/A 6,530 LF 6, N/A N/A Both Wisconsin approach slabs have scattered areas where the joint material has lost adhesion approximately 70 linear feet. There are scattered areas of spalling along the concrete construction joints. There are scattered joints across the entire length of the bridge in both lanes where the joint material has lost adhesion approximately 260 linear feet. Minnesota End E-eight Joints; West Bound Lane - during 2006 inspection the joint opening measured 2 1/2 to 3 inches wide across the entire length and approximately 26 linear feet of the joint material has lost adhesion. During 2005 inspection the joint opening measured 3 inches wide across the entire length and approximately 25 linear feet of joint material has lost adhesion. East Bound Lane - during 2006 inspection the joint opening measured 4 inches wide across the entire length and approximately 50 linear feet of joint material has lost adhesion or was completely gone. During 2005 inspection the joint opening measured 4 inches wide across the entire lane and approximately 50 linear feet of joint material has lost adhesion. Wisconsin End E-eight Joints; West Bound Lane - during 2006 inspection the joint opening measured 1 1/2 inches wide across the entire width of the right hand lane and 1 inch wide across the entire width of the left hand lane. All of the joint material has lost adhesion and 10 feet of the foam material is gone. During 2005 inspection the joint opening measured 1 1/2 inches wide across the entire width of the right hand lane and 1 inch wide across the entire width of the left hand lane. All of the joint material has lost adhesion and 10 feet of the foam material is gone. East Bound Lane - during 2006 inspection the joint opening measured 1 inch wide across the entire width of the right hand lane and 1 inch wide across the entire width of the left hand lane and 6 linear feet of the joint material has lost adhesion. During 2005 inspection the joint opening measured 1 3/4 inches wide across the entire width of the right hand lane and 1 1/2 inches wide across the entire width of the left hand lane. All of the joint material has lost adhesion. 321 CONC APPROACH SLAB Minnesota Approach Slabs WBL - has 1 full length longitudinal crack located near the centerline and there are 2 small spalls along the end block joint. EBL - has 1 full length longitudinal crack and 1 transverse crack that extends from the centerline to the right hand shoulder. The transverse crack has 3 random cracks extending out from the crack. Wisconsin Approach Slabs WBL - has 3 spalled areas along the end block joint, 3 along the construction joints next to both the south and north curbs. The south side curb has settled slightly from 1/2 to 2/4 of an inch. The north side side curb has random cracking, deteriorated concrete and spalled areas. EBL - has 5 full width transverse cracks with some short longitudinal cracks in the end block, 3 small spalls along the end block next to the strip seal extrusion and a small area of deteriorated concrete at the center line next to the end block joint. The west side curbing along the center median near the end block has a small spalled area. 331 CONCRETE RAILING ,980 LF 2,430 2, N/A 4,980 LF 2,430 2, N/A The railing was modified in 1981 to a solid barrier. There are 10 to 15 vertical cracks per concrete rail section with all cracks leaching or staining. The top surfaces of all railing have random longitudinal cracks with staining. There are scattered areas in spans 2, 3, 4, 5, 6, 7, 8 & 10 that have horizontal cracking, deteriorating concrete, small spalls, staining and rebar spalls approximately 100 square feet. See the attached "Snooper Inspection Report" for further detail. 210 CONCRETE PIER WALL LF N/A 414 LF N/A 215 CONCRETE ABUTMENT LF N/A 155 LF N/A West Abutment - has 13 vertical cracks scattered along the front face that extend across the bridge seat. The parapet wall has 14 full height vertical cracks with staining or leaching, diagonal leaching cracks at each end with a small spall near the top at the south end. There is a spalled area in the parapet wall at the center line construction joint with 2 exposed rebar approximately 4 square feet in size. East Abutment - has 13 vertical cracks scattered along the front face that extend across the bridge seat. The parapet wall has 10 full height vertical cracks with staining or leaching, diagonal leaching cracks at each end and at the centerline. The east abutment appears to have some inward movement of the abutment, this movement is noted at the tops of the parapet wall at each end. 2003, 2004, and 2005 measurements were; South End - 0 at the bottom to 2 1/4 inches at the top. North End - 0 at the bottom to 2 inches at the top. During 2006 the measurements noted were; South End - 0 at the bottom to 2 1/2 inches at the top a change of 1/4 of an inch. North End - 0 at the bottom to 2 1/4 inches at the top a change of 1/4 of an inch. 234 CONCRETE CAP LF N/A 766 LF N/A All Pier caps have scattered vertical and random cracks. 387 CONCRETE WINGWALL West Abutment Wingwall's - the southwest wingwall has 3 vertical cracks. The northwest wingwall has scattered areas of minor cracks with some staining. East Abutment Wingwall's - the southeast wingwall has scattered vertical and horizontal cracking with leaching. There is an area of delaminated concrete approximately 4 square feet in size. The northeast wingwall has scattered vertical and horizontal cracks with map cracking. There is an area of delaminated concrete approximately 4 square feet in size. 356 FATIGUE CRACKING CONC DECK CRACKING There are approximately 100,000 linear feet of unsealed cracks in the surface of the deck. 359 CONC DECK UNDERSIDE EA EA There are scattered areas of active corrosion on the stay in place forms. The overhangs on the sides of the deck and the medium have spalls with exposed rebar.

4 08/13/2007 Page 4 of SETTLEMENT East Wisconsin Abutment - appears to have some inward movement movement and measurements taken at the tops of the parapet wall at each end. During inspection in 2003, 2004, and 2005 measurements noted were; South End - 0 at the bottom to 2 1/4 inches at the top. North End - 0 at the bottom to 2 inches at the top. During 2006 the measurements noted were; South End - 0 at the bottom to 2 1/2 inches at the top a change of 1/4 of an inch. North End - 0 at the bottom to 2 1/4 inches at the top a change of 1/4 of an inch. 363 SECTION LOSS CRITICAL FINDING EA 1 0 N/A N/A N/A No critical findings were observed at the time of inspection. 1 EA 1 0 N/A N/A N/A 966 FRACTURE CRITICAL Do Not Remove. See in-depth report for location of F/C members. 981 SIGNING EA All signs were in place at the time of inspection. 1 EA GUARDRAIL There are numerous plate beam sections along the center median across the entire length of the bridge that have minor to moderate amounts of corrosion. There is one section of plate beam on the west bound side in Span 2 that has a 6 inch tear. 983 PLOWSTRAPS The plowstraps are located over the abutment at the Minnesota end of the bridge. WBL - there are 2 missing plowstraps. EBL - there are 6 missing plowstraps. 984 DRAINAGE Wisconsin Approach Slabs - WBL - the catch basin along the north side curbing has 6" X 3' spalled area. 985 SLOPES The slopes at the abutments and wingwall's are in stable condition at the time of inspection. 986 CURB & SIDEWALK There are numerous unsealed longitudinal cracks in the tops and front face of both curbs with some leaching and rust staining. There are numerous rebar spalls scattered along the front face of the curb on the west bound side of the bridge in Spans 8 through MISCELLANEOUS

NOTE: Bridge was inspected using under deck snooper truck in NOTE: Check the attached snooper inspection report for further details. NOTE: Underwater inspection completed in NOTE: River soundings were taken in 11-88, 10-89, 10-90, 11-91, & NOTE: The bridge was originally painted in 196, it was blasted clean and repainted in 1993 and Channel - NBI # 8 All banks are well protected at this time.
