One (1) DATE: October 23, 2018 TO: All Record Holding Contract Documents This Addendum is issued to all registered plan holders pursuant to the Conditions of the Contract. This Addendum serves to clarify, revise, and supersede information in the Project Manual, Drawings, and previously issued Addenda. This addendum and its attachments shall become a part of the plans and specifications and contract documents and shall apply to the bid proposals for the above named project. The bidder(s) shall notify all affected subcontractors, material suppliers, and others to incorporate necessary cost and schedule updates, to the bid proposal and the work changes affected by this Addendum. The Bidder shall acknowledge receipt of this Addendum in the appropriate space on the Bid Form. Bidders must also acknowledge receipt by to wendy@kelleynetwork.com. In the event of conflict between plans and specifications and this addendum, the addendum shall take precedence. Any modifications necessary to incorporate the revisions shall be included in the appropriate bid prices. The bid documents are hereby corrected, modified, and/or amended in the following manner: General: 1. The sign in sheet and agenda from the Pre-Bid meeting on October 15, 2018 at 3:00 P.M. is attached. 2. As a clarification to the bypass pumping location and routing, the Contractor shall setup bypass pumping equipment immediately upstream of the pipe segment under construction and shall pump to the manhole immediately downstream of the pipe segment under construction. 2 Contractor may utilize storm drain pipes beneath driveways to route the bypass pump piping. The bypass pump piping must be removed from storm drain pipes for any rain event. Driveway access to remain uninterrupted at all times. Contractor to utilize temporary crossing ramps if the bypass pump piping blocks access to the drive. Contractor to be responsible for all damage as a result of bypass pumping lines left in storm drain pipe during a rain event.. By: WILL GLENN, P.E. Addendum #1 is 2 total pages. 4 SANITARY SEWER SYSTEM IMPROVEMENTS TOWN OF ROGERSVILLE CWSRF # CS MONDAY, OCTOBER 15, 3:00 P.M. Note: This agenda is complimentary, for the purpose of review during this meeting only. It does not attempt to, nor does it represent a total recapitulation of Project requirements and does not change or alter same in any respect. Changes, if any, will only be made by written Addendum. General Review Items 1 Welcoming remarks. Reminder to sign the attendance list. 2 Introduction of Owner: Rogersville Water Works Shannon Howell, Manager, (256) Introduction of Engineer: Project Engineer Will Glenn, P.E., The Kelley Group, LLC, (256) Scope of Work: Contractor will supply a minimum of five (5) references and validation of performing similar sanitary sewer rehab construction. Contractor must have successfully installed or rehabbed a minimum of 10,000 linear feet of gravity sewer line. Prior to construction, Contractor & Engineer to meet and discuss expected problem areas in regard to the erosion control work. 1400Base bid includes a combination of pipe bursting (4,973 LF) and open cut replacement (607 LF) along with 15 point repairs. Additive Alternative #1 includes pipe bursting (1409 LF) and Additive Alternative #2 includes pipe bursting (1034 LF). Work must begin thirty (30) days after contract execution No work to be performed on Saturday and Sunday. Contractor may request approval from Engineer to work Saturday and Sunday 48 hours prior to weekend. Construction progress schedule Progress Meetings - Progress meeting will be held as needed or at the request of Owner, Engineer, Installing Contractor or ADEM Invoice procedure should be submitted on or before 25 th day of each month & include updated work schedule Change Orders - All change orders will be processed as outlined in the executed contracts. Only formally executed and approved change orders will be recognized. Line items for work to be performed are only estimated quantities. A final change order to resolve actual quantities used is to be expected at project s end. Quantities to not exceed the bid line item without notification to Engineer. KG PROJECT NO Page 1 of 4

5 SRF covered sign - Project sign will be installed by Installation Contractor within 100 yards of Rogersville Water Works office entry door in clear view. American Iron & Steel requirements are listed in the Project Manual. Contractor to maintain sanitary sewer service at all times. Contractor to supply bypass pumping equipment immediately upstream of the work area. Contractor to submit a maintenance of sewer service plan to the Engineer prior to Notice to Proceed. Contractor to submit a construction phasing plan and schedule to Engineer prior to Notice to Proceed. Contractor to submit a traffic control plan to Engineer prior to Notice to Proceed. Contractor to minimize the number of insertion pits and coordinate insertion pit locations with Engineer. All service laterals shall be dye tested to verify active status prior to reconnection to the main. Owner supplied sanitary sewer flow study dated July 24, 2015 to aid in bypass pumping design. Flow study is located in the Project Manual. Owner supplied CCTV data for all sewer lines within the scope of work and is located in the Project Manual. Contractor to supply post-construction CCTV data to verify installation of new pipe meet s contract requirements. Engineer to review and Contractor to fix all issues noted by the Engineer at no additional cost to the Owner. MBE/WBE Contractor is reminded to review and abide by ADEM requirements to solicit bids from minority and women owned businesses should any subcontractors be used on the project. If the Contractor will not be using a subcontractor, written notice to that effect is required on Contractor letterhead. Davis Bacon wages and Certified Payroll Davis Bacon Posters should be posted at a prominent location at the job site. Installing Contractor to complete WH-347 Certified Payroll forms for each and every week during the project and provide to Engineer certified payroll forms with Invoices. Onsite employee interviews will be scheduled two weeks from start of project and again two weeks from end of project. Two people to sign off on interview forms. Engineer to receive notification of rain days- Any rain days to be used by the Installing Contractor shall be communicated by the Installing Contractor to the Engineer in an as documentation and clearly define the dates used as rain days. Claiming of a rain day shall be communicated before the end of the work day.
