1 Mesa Antero Water Association CR 261H, Nathrop, CO The purpose of this document is to provide important instructions to a homeowner/builder of a property in Mesa Antero Subdivision Filings 3 and 4 for connecting to the Central Water System. Please read and follow the instructions carefully to ensure a timely and successful connection. In the event of any conflict between these instructions and Chaffee County, Colorado Building Code, the Code shall control. At least fifteen (15) days prior to proposed connection to the Central Water System: 1. The plans for a new construction must have final approval by the Mesa Antero Architectural Control Committee (MAACC) before a connection may be performed. Instructions and contacts for MAACC are on the Mesa Antero website under MAACC. 2. Submit the tap fee (if not already paid) to Mesa Antero Water Association, P.O. Box 159, Buena Vista, CO The current tap fee is $7000. For some lots, the tap fee has been paid by a previous property owner, or the fee has been frozen at a lesser amount. Contact the Association at MesaAnteroWater@gmail.com to determine the applicable fee. 3. Execute and sign a Water Services Agreement and give the original to an Association Director. The WSA is a fill-in-the-blanks form on the Mesa Antero website under MAWA. 4. Contact the Association to establish the location of the water distribution line to be tapped and the approximate location of the connection. Association contacts are listed on the Mesa Antero website under MAWA. Or by sending an to the Association at MesaAnteroWater@gmail.com. We have posted a Water System Map on the website for planning purposes. 5. Provide the Association with the intended excavator's credentials and experience. All excavation within 30 feet of the water distribution line, and the actual tap-in, must be done by an Association-approved contractor. A homeowner/builder's excavator with appropriate credentials and experience may perform this work. If the homeowner/builder's excavator does not have the necessary qualifications, ample time must be allowed for the Association to schedule a suitable substitute. At least seven (7) days prior to the proposed connection to the Central Water System: 1. Contact the Colorado Utility Notification Center (phone number is 811 or ) prior to digging the trench for the water line. (Contractor's responsibility) 2. If necessary to trench across a County Road or otherwise excavate in the 60-foot County Road right-ofway in order to tap into the System, contact the Chaffee County Road and Bridge Dept. ( or ) for a permit. (Contractor's responsibility). Service Line / Tap-in Materials: In order to standardize installation, no substitution of Service Line materials will be permitted. 2 1. One-inch cross-linked polyethylene (PEX) tubing (or other tubing that is approved by the State of Colorado for underground potable water use) from the water distribution line to the curb stop valve, plus necessary fittings if any. (Selected and provided by homeowner or homeowner s contractor). 2. For installations where the Service Line crosses a county road or is in a zone suspected or known to freeze: PVC Schedule 80 2 pipe sleeve and insulation tarp (e.g., Insul-Tarp) the length of the road cross or freeze zone. (Provided by homeowner or homeowner s contractor). 3. A locator wire for use in finding the Service Line once buried. (Provided by homeowner or homeowner s contractor). 4. One 4-inch brass pipe saddle (or 2-inch saddle in most cul-de-sacs), and one 1-inch corporation stop valve. (Provided by Association). 5. One 1-inch brass curb stop valve in the Service Line, which is used to turn service to the residence off and on. (Provided by Association). 6. One 6-foot curb stop box and internal rod; one box lid to provide access to the curb stop valve. (Provided by Association). 7. One blue water valve marker. (Provided by Association). Note: The Association will bill the homeowner s contractor for tap-in materials it provides (at Association s actual cost) and for installation services performed by the Association s contractor, if any. Payment is due within 30 days following receipt of invoice. Specific Service Line Installation Requirements: 1. The curb stop will be placed on the homeowner's property in the water utilities easement as close as practical to an adjacent county road. If the owner's home is on the opposite side of a county road from the distribution line, the curb stop will be placed on the homeowner's side of the road. The curb stop shall not be placed in the drainage ditch along the side of the county road. 2. The portion of the Service Line from the corporation stop valve to the curb stop valve shall be marked with a locator wire. A locator wire from the curb stop to the house is at owner's discretion but is recommended. 3. The entire Service Line from corporation stop valve to the house is to be buried at least six (6) feet below grade. 4. For installations where the Service Line crosses a county road, run the line through the PVC Schedule 80 2 pipe sleeve, cover with fill material, and cover with insulation tarp the length of the road cross. 5. For installations not crossing a county road but where the Service Line is in a zone suspected or known to freeze, cover with insulation tarp the length of the freeze zone. 6. The Service Line shall be placed upon a bed of sand a minimum of six (6) inches deep, and covered with sand a minimum of six (6) inches deep. 7. The Service Line shall be pressure tested for one (1) hour, showing no visible leaks, prior to backfilling. 8. All valves in the Service Line must be checked and adjusted, if necessary, so that they will turn freely. 9. All connections must be left uncovered for Association inspection. A Director of the Association must inspect and "sign-off" on the completed installation prior to backfilling. Revised: 10/11/2018 Page 2 of 5

The contractor or property owner shall prepare and submit to the Association a "rough sketch of the "as-installed" water Service Line from the curb stop valve to the residence, including approximate distances and identification of other utilities (telephone, electric, propane) buried in the same trench. 11. The Association shall be present and will conduct a required inspection at the time the tap-in to the distribution line is made. All the above requirements must be completed prior to or just following the tap-in. Forty-eight (48) hour notice to the Association is required. 12. When it is necessary to trench across a private or Chaffee County road to connect the water distribution line to the residence, it may be necessary for the contractor to make a "return trip" in four to six weeks after the original installation in order to repair the settling and indentation that may occur in the roadway. That expense is the responsibility of the property owner. Meter Installation Materials: In order to standardize installation, no substitution of the specified meter, check valve, or pressure reducing valve will be permitted. 1. Sensus 5/8" x 3/4" SR-II ECR remote meter and washers. A meter must be installed so that water usage can be monitored and billed each calendar quarter. 2. Inside meter insetter and compression connectors for mounting of the meter. 3. Remote readout touchpad, length of solid 18-2 AWG wire to connect the meter to the touchpad, and MXU to enable reading the meter from the road. 4. Dual check valve to prevent backflow into the Central Water System. 5. Water pressure reducing valve. Note: All the above meter installation materials will be supplied upon request by the Association to the homeowner at the Association's actual cost. You will be billed for payment, which is due within 30 days following receipt of invoice. All meter installation materials are installed by the owner's contractor at owner's expense. Specific Water Meter Installation Requirements: 1. Locate the meter in a place that is heated and out of the wind or weather (like a mechanical room, basement, or crawl space). Mount the meter in the insetter in a horizontal line with the register easily accessible and visible for reading. 2. The remote readout touch pad and MXU shall be mounted on the outside of the home, clearly visible with an unobstructed view by line-of-sight from the nearest county road, and accessible to a meter reader on foot when necessary for leak detection or in the event of a failed remote reader. Allow for a 6" x 6" surface on which to mount the MXU. 3. The in-house pressure reducing valve should be accessible to the owner in or under the residence and set according to State guidelines at 40 to 50 PSI for a residence. 4. An in-house master shutoff valve and sediment filter, not provided by the Association, are recommended. 5. The sequence of meter installation parts (in direction of water flow): (1) in-house master shutoff valve, (2) flow meter, (3) dual check valve, (4) pressure reducing valve, (5) sediment filter. 4 6. The Association will conduct a required inspection of the meter installation upon completion, program the MXU for remote reading, and record the current meter reading as the point of zero usage. Revised: 10/11/2018 Page 4 of 5
