Description The SureSeal Floor Drain Trap Seal: Models SS2009, SS3509 and SS4009 are preassembled inline floor drain trap seal protective devices, which are made of commercial grade ABS plastic and polychloroprene plastics. In general, the SureSeal: Models SS2009, SS3509 and SS4009 are comprised of three pieces: the housing, the diaphragm and the gasket. The SureSeal Floor Drain Trap Seal: Models SS2009, SS3509 and SS4009 must be installed in conjunction with approved area drain fixtures, as area drain fixtures are described in ANSI/ASME A The SureSeal devices may receive liquid from a drain surface and minimize the ingress of sewer gas from the drain into the building. Authorization Requested The Applicant sought authorization for use of the SureSeal Floor Drain Trap Seal: Models SS2009, SS3509 and SS4009 in conjunction with approved area drain fixtures, as area drain fixtures are described in ANSI/ASME A , in lieu of trap seal primers, as required by Sentence (1) of Division B of the Building Code. 5. Assessment Reports and assessments provided by the Applicant show that if the SureSeal Floor Drain Trap Seal: Models SS2009, SS3509 and SS4009 are designed, performance tested, installed, and maintained in accordance with the limitations in the manufacturer's instructions, and the conditions stated in this authorization; the same level of performance will be provided as required by the Building Code. Test Report, SGS Test Report, ASSE Model SS2009, SS3009 and SS3509 report number 654: , dated March 15, Authorization The SureSeal Floor Drain Trap Seal: Models SS2009, SS3509 and SS4009 are authorized for use in lieu of a trap seal primer within the scope of Part 7 Plumbing of Division B of the Building Code, and when installed in accordance with the manufacturer s recommendations, and within the terms and conditions contained in 6.A and 6.B below; all other requirements of the Building Code shall apply: SureSeal, LLC Page 3 of 5

4 A. Specific Terms and Conditions 1. This Authorization is valid only for SureSeal, LLC s, SureSeal Floor Drain Trap Seal: Models SS2009, SS3509 and SS4009 for 50mm (2 ), 89mm (3.5 ) and 102mm (4 ) diameter devices (the SureSeal ); 2. This Authorization expires on July 28, 2016; 3. The SureSeal shall only be installed in conjunction with an ANSI/ASME A area floor drain, as illustrated on the manufacturer's installation instructions on the product packaging; 4. The SureSeal shall have the following permanent and visible markings: (a) SureSeal, (b) ASSE 1072, and (c) Models SS2009, Models SS3509, or Models SS4009; 5. The SureSeal shall not be installed where the liquid discharge may reach or exceed a temperature of 93 0 C (200 0 F); and 6. A final test shall be performed, as required by Subsection Testing of Drainage and Venting Systems of Division B of the Building Code, prior to the floor drain trap(s) being primed with water, to confirm the SureSeal installation. B. General Conditions 1. The use of the SureSeal as described in Section 3. and the Specific Terms and Conditions set out in Section 6.A must comply with: (a) (b) (c) the Building Code Act, 1992, (the Act ) as amended or re-enacted, except as specifically authorized herein, the Building Code as amended or remade, and all other applicable legislation. 2. A copy of this Authorization shall accompany each application for a building permit and shall be maintained on the site of the construction with the building permit. 3. The Applicant specified in Section 1. shall promptly notify the BMEC of: (a) (b) the failure of the Applicant to comply with any of the Specific Terms and Conditions set out in Section 6.A, the failure of the material, system or building design that is the subject matter of this Authorization to (i) (ii) comply with any of the Specific Terms and Conditions set out in Section 6.A, or provide a satisfactory level of performance in situ, or SureSeal, LLC Page 4 of 5

5 (c) the occurrence of any of the events described in General Conditions 6.B.4.(a), (b), (e) or (f). 4. The BMEC may amend or revoke this Authorization at any time on its own initiative, or at the request of the Applicant specified in Section 1. without restricting the foregoing, the BMEC may amend or revoke this Authorization where it determines that: (a) any change has been made to: (i) the name of the Applicant specified in Section 1, (ii) the address or other contact name information of the Applicant specified in Section 1, (iii) the ownership of the Applicant specified in Section 2, (iv) the manufacturing facilities specified in Section 2, (v) the material, system, or building design that is the subject matter of this Authorization, or (vi) a test method relevant to this Authorization, (b) (c) (d) (e) (f) (g) the Applicant has failed to comply with any of the terms and conditions set out in this Authorization, in the opinion of the BMEC, the use of the material, system or building design authorized herein provides an unsatisfactory level of performance in situ, in the opinion of the BMEC, amendment or revocation of the Authorization is appropriate on the basis of potential danger to public health and safety, the Act or Building Code has been amended, re-enacted or remade in a manner relevant to this Authorization, this Authorization was issued on mistaken, false or incorrect information, or a revision of an editorial nature is appropriate.
