2 Description of Work: Removal of a tile and screed floor topping, and concrete plinth from an open area within the basement of Selfridges. The total area to be removed is approximately 2300 metres square. The screed and tile combined depth is approximately 80mm. The raised concrete plinth area is approximately 165 metres square and average 300mm deep. Resources: 6 Labourers 2 Brokk operatives 2 Brokks (Weight 384kg each excluding attachments) 1 Foreman (M. Jervis) Skips provided by others 110v power supply 415v power supply Task lighting units Hand Tools 110v / 415v leads Emergency Procedures and Contacts: MAJ Site Foreman: M. Jervis Principal Contractor First Aiders: S Bennett, J Hunter, R Martin Muster Point: Top of service ramp Fire Points: 4 fire point within basement work area, hand operated gongs means of raising alarm. Access & Egress Pedestrian access is via Markets Street. Alternative access for tools and equipment is via the service ramp. Personal Protective Equipment All contractors shall wear hard hats, steel toe-capped and midsole protective footwear at all times. Gloves will be worn for general manual handling operations, note the gloves should have adequate grip ability and resistances to water ingress. Floor tiles may be damp/wet slippery to handle due to water suppression of dust. Eye protection conforming to BSEN 166/B standard and ear protection shall be worn by Brokk operatives and those carrying out debris clearance works. Note:- Noise levels of Brokk 40 are anticipated to be 82.9dB(A) from each Brokk (85.9dB(A) from 2 Brokk s. Page 2 of 9

3 Respiratory (FFP3 standard) disposable dust masks will be used when there is dust / mist visible in the air. Long sleeves and trousers will be worn at all times, coveralls will be provided. All employees will have been adequately trained to use and maintain any item of PPE in their possession. Requirements from Principal Contractor 415v 3 Phase 32 amp supply x 2 with trip devices 110v 16amp / 32 amp supplies Continuous water supply Confirmation of service terminations to any services that may be located within the screed/tile floor coverings, particular attention to be given to area around escalators, raised plinth and within the walls near the stairs. Information relating to any known live services, such as routes taken when applicable to MAJ works. Confirmation of asbestos type 3 survey, and results of any findings that may affect MAJ operations. Welfare Welfare facilities provided extent and restraints of provisions to be clarified within induction. No eating drinking in work areas. Controls and Sequence of Works Arrive on site, all persons to attend a principal contractor site induction and a method statement briefing prior to commencing, signing the appropriate registers thereafter. Copies to be kept on site and made available when requested. Foreman Mark Jervis to check (if not previously received confirmation) with principal contractor adequate precautions have been taken to prevent accidental contact with live services. In instances such as pictured below where a known live services passes through a formed core hole to the service tray below, the area within 500mm of the services will be left, unless fractures have occurred making removal by hand without the use of tools and equipment a possibility. 4 Leave 500mm around service, unless fractures in screed and good separation from slab allows safe removal by hand. Take care that screed and tiles are not pushed by Brokk resulting in damage to service. Brokk operatives to be competent through both experience and training. Equipment will be brought through the back service entrance onto site. Stlye & Wood will have a forklift truck and competent trained operative on standby to assist in any lifting operations that maybe deemed necessary. The work area will be split into two sections the escalators shall define the two areas. MAJ personnel will work one side of the escalators whilst other contractors work on the opposite side of the escalators. With the escalators set as the demarcations and supervision and instruction this shall be deemed as adequate segregation of work areas. Should the situation on site change then as needs dictate physical demarcations will be added in the form of barriers and/or red, white tape. MAJ commencing area Others work area Page 4 of 9

5 Amour braided cables will be used for the Brokk and they will be routed to avoid interaction with the Brokk and others during works. The leads will be kept clear or water dust suppression and the operative will isolate the power at the mains supply before connecting and unplugging the sockets. The Brokk operative will control which areas labourers can access. Labourers will not access areas within reach of the Brokk arm, all works will be carried out at a safe distance. The Brokk operative must operate the Brokk from a safe distance both for himself and in good sight of labourers. Once an area of floor has been broken into manageable pieces and there is sufficient access for the labourers to gain safe access without risk of contact from the Brokk or flying debris the labourers will access the area. There are several disposal bins on site provided by others that will be utilised. Labourers will load bins within the weight limits and transport them to the rear load bay for emptying into the skips by Styles & Wood trained forklift operatives. This process will be repeated throughout the contract until completion. There are 6 labourers; duties will be rotated from loading to transporting the bins as far as practicable to reduce manual handling. When necessary two persons will transport bins particularly when loaded. The sizes of sections that the Brokk breaks the floor material into will take into account that there will be a lot of bending and picking up from floor level, so with this in mind, broken sections will not generally exceed 10kgs, those that do will be joint lifted. Individuals will only lift within their capabilities and joint lifts will not exceed 2/3rds of the individual s capabilities. Contractors shall adopt good kinetic lifting techniques keeping the back straight bending the knees. The works will be carried out systematically giving consideration to the manual handling operations i.e. it is easier to push the bins on a tile surface, with this in mind when possible a tiled route will remain for the bins. Dust will be controlled using water mist to dampen down. The person dampening down must remain at a safe distance from the Brokk and be located in good view of the Brokk operative. Tiled areas shall be maintained clear of water / slurry deposition. Squeegee the residue onto the concrete areas where necessary to avoid slip hazards, take care to avoid cables. Page 5 of 9

6 The areas around the electrical power supplies will again be left untouched by the Brokk to at least 500mm clearance. Prior to recommencing on the next shift Styles & wood appointed electrical contractors will relocate power supplies to an adjacent prepared area. Leave a minimum of 500mm around power supplies, Style & Wood to arrange relocation to a prepared area. Once relocated removal of remaining area can proceed. The raised plinth area will be broken using the Brokk again into manageable pieces. Should it be discovered that the slab it cast as part of the original slab then 415v diamond saw cutting will be required along with an additional method statement. Left raised plinth to be removed Brokk Some use of smaller handheld breakers maybe required to clean edges up. When selecting the breakers advice will be sought to obtain the lowest practicable noise and vibration levels. Operatives will be given adequate information instruction and training. They will be made aware of safe usage times and should needs dictate be given other non-vibratory tasks to do if these times are a risk of being exceeded. Page 6 of 9

7 Chisels will be frequently changed to keep vibration exposure limits as low as possible. Portable Electrical Equipment will display current evidence of PAT testing. All items of plant and equipment will be subject to pre-user inspections and a record of inspections will be made onto a register at least weekly. Defective plant and equipment will be taken out of service. No combustion engine plant will be operated internal of the premises due to confines. Monitoring & Compliance: All persons involved shall receive a site induction on arrival to site prior to commencing and sign a register thereafter to this effect. All persons will sign in and out each time they attend and leave site in accordance with the Styles & Wood requirements. All person involved shall be briefed and understand the contents of this method statement, associated risk and COSHH assessments prior to commencement and sign the register below. Any specific additional information shall be given in the form of a toolbox talk. All accidents and near misses will be reported. H H Terminate or isolate services whenever possible, Supervisor to cheque prior to commencing. Services that must remain live to be clearly marked and no works within 500mm. Provide information to all on site. High visibility tops to be worn along with safety footwear and hard hats. Vibration Contractors L M Brokk used to reduce exposure. Minimal use of vibratory hand tools, inform employees of safe exposure times, if necessary rotate operatives. Wear gloves. Train in effects of vibration. Noise Anyone in area M M 82.9dB(A) per Brokk 85.9dB(A) from 2 Brokks. Hearing protection to be provided and worn by anyone exposed.
