1 Scope of Work 1) This project consists of DEMO/Excavation of Twenty-Five (25) Apartments and one (1) Mailbox Shed located at Cherokee Heights Apartments (#01) Pryor, Oklahoma. The contractor shall bid to ensure "NOTIFICATION to ODEQ and ODOL" for all units 10 Days before the demolition. Demo: 2) The contractor shall bid to furnish all necessary labor, licenses, material, equipment, tools, permits, and insurance necessary to completed this "Scope of Work" in 56 Calendar Days. 3) The contractor shall bid to remove existing utilities, (Water and Gas line) 25' from Apartments. All existing sewer line will be capped 25' from apartments; all non-asbestos debris will be disposed of at "State Approved Dump site". 4) The contractor shall bid to Demo/Excavation Twenty-Two (22) apartments and One (1) mailbox shed. Apartment's# are: 1,2,4,5,6,7,10,11,12,13,14,15,16,17,18,19,20,21,22,23,24, and to remove all debris and dispose of at "State Approved Dump site". 5) The Contractor Shall bid: ABATEMENT/ASBESTOS/DEMO/EXCAVATION ofthree (3) Apartments #'s 3, 8, and 25. Bid to remove all debris and dispose of at an Asbestos Approved Landfill. 6) Summary of Findings "Asbestos" A) Unit #3 - Wall Texture, 4,900 Sq. Ft. Unit #25 - Ceiling Texture, 1,400 Sq. Ft. NOTE: - The contractor shall bid to treat this solicitation as RACM (Regulated Asbestos Containing Material); therefore the following requirements must be met: The contractor shall bid these apartments considered as a "Class 1 Work", the removal of surfacing materials creates regulated asbestos containing material (RACM). The contractor shall bid to remove Asbestos from the wall texture and ceiling texture prior to demolition on the following apartments: Unit #3 and Unit #25. Supervisor/Contractor needs to be EPA certified and licensed through Oklahoma Department of Labor (5 day EPA supervisor class). Workers need to be EPA certified workers and ODOL licensed (4 day EPA worker class).

2 Air monitoring must occur during the removal and clearance air sampling is needed. All documentation of air sampling must be provided to Cherokee Nation Environmental Dept. and HACN once job is complete. RACM waste must be labeled and disposed of into an asbestos approved landfill. Manifest must be provided to Cherokee Nation Environmental Dept and HACN once disposal occurs. ODOL, ODEQ, OSHA regulations for asbestos removal must be followed. All certifications/licenses should be submitted with bid proposal. B) Unit #8 - Floor Tile Mastic, 1,400 Sq. Ft. The contractor may bid this apartment considered as "Class /I work". The mastic under the floor tile in unit #8 can remain in the building during demolition as long as it does not become a regulated material in the process. It can be disposed of with other construction debris. A wet demolition is recommended during the process. Supervisor/Contractor needs to be an EPA certified and licensed through Oklahoma Department of Labor (5 day EPA supervisor class). Workers need to be EPA certified workers (8 hour class according to 29 CFR (9)(iv)). All certifications/licenses should be submitted with bid proposal. ODOL, ODEQ, OSHA regulations for asbestos removal must be followed If the mastic becomes RACM during any part of the demolition the unit #8 project must default to the Class I requirements listed above. EXCAVATION: 7) The contractor shall bid to remove (25) Apartments and (1) Mailbox shed footings and dispose at a "State Approved Dump Site". 8) The contractor shall bid to cut concrete driveways from project (Asphalt) street. 9) The contractor shall bid to remove all sidewalks and concrete driveways (tinhorns) and dispose at an "Approved State Dump Site". 10) The contractor shall bid to make necessary (Cuts) or excavate ditches to have "Continuous Ditch Line" on this project.

3 FINAL GRADE: 11) The contractor shall bid to make necessary (Excavation) to fill footing, driveways and sidewalks with existing top soil on this project. 12) The contractor shall bid to level and tight blade (all) disturber soil. 13) The contractor shall bid to grass seed (all) disturber soil. 14) The contractor shall bid to mark all existing sewer lines. {With 3/8"x3' Rebar) W' under of finish grade of the yard. 15) Bidder can visit the job site and acquaint themselves with the exact nature of work to be performed. Contract Larry or William to schedule a site visit. 16) The submission of the bid shall indicate the bidder thoroughly understands the specifications, conditions and scope of work. 17) Bidder shall be responsible for determining the amount of labor and materials needed to complete the project being bid. 18) All equipment, material and articles furnish under this contract shall be new and of the most suitable grade for the purpose intended, unless otherwise specifically provided in the contract. 19) All work must be performed in a workmanlike manner. 20) Contractor shall at all times keep the work area clean, including storage areas free from accumulation of waste materials. Contractor shall place a Portable/Johnny and post all proper documents for this project. After completing the work and final inspection the contractor shall remove all equipment, tools and materials that are not property of the HACN. The work area shall be left in a clean, neat and orderly condition satisfactory to the HACN inspector and perform all specified all tests. 21) Three phase payment will be paid on the project; # (1) one payment at 30% completion of this project, # (2) two payment at 60% completion of this project, and the Final payment at 100% completion of the project with the approval of the HACN inspector.

4 Building Demolition Notification Form GENERAL INSTRUCTIONS: The structure to be demolished must first be inspected by an EPA-accredited asbestos inspector and then this Demolition Notification Form must be completed and submitted. NOTE: If the building or structure contains "regulated asbestos containing materials" (RACM), the RACM must be abated and a Standard EPA NESHAP Demolition and/or Renovation Notification Form must be completed and submitted to this Department. This Demolition Notification Form will not be accepted for reporting the removal of regulated asbestos containing materials (in lieu of the NESHAP form) from the building(s) scheduled for demolition. This form must be received by the Department not less than 10 working days (two calendar weeks) before the demolition project is scheduled to start. Any notification that is incomplete or any notification indicating the activities to be in violation of applicable regulations will be considered an invalid notification. Separate notification must be provided for each building or other individual facility that will be demolished. Under some circumstances, the removal of Category I non-friable ACM (e.g., vinyl asbestos tile) may not be required prior to demolition. Category Il non-friable ACM (e.g. "transite") must be removed prior to demolition. Mail the original form, completed and signed, to the DEQ office closest to your job site.
