3 1. SCOPE OF PROJECT The Project will be the renovation and expansion of a new clinical lab (6,900 sq. ft.) located on the ground floor of the existing West Pavilion Memorial City Hospital campus. The Project will also include renovating the existing Morgue (620 sq. ft.), which will be converted to a refrigerated body holding and family viewing area. The overall project size is approximately 10,120, sq. ft. General Contractor will be expected to coordinate with all Project Team Members on their areas of work, including MH personnel. Equipment vendors, and other parties employed by owner, (MH). General Contractor (GC) services will include all work in accordance with the plans and specifications as issued for the Project and the GC will be expected to work with the Project team to help maintain the Guaranteed Maximum Price (GMP). Installation and hook-up of selected equipment and supports as indicated in the Project documents is part of this proposal. DOCUMENTS: Each prime bidder will be provided access to Bidding Documents through an E-Builder bid invitation. Bidder is responsible for the printing the documents from the E-Builder website. All questions and requests for clarification shall be submitted in written via the e-builder Website by November 27, The Memorial Hermann Project Manager will in turn route the questions and requests to the A&E team. Responses will be returned timely. Verbal questions and requests will not be accepted unless exceptional circumstances exist. Such verbal questions and requests shall be followed by written confirmation. Questions and requests for clarification will be clarified or answered by Addendum via the E-Builder website. Verbal responses will not be given under any circumstances. Houston, Texas Page 3

4 2. SCOPE OF SERVICES This Request for Proposal (RFP) seeks to identify a General Contractor (GC) for the Memorial Hermann Memorial City Clinical Lab Renovation Project. The GC will be required to work in conjunction with the Owner s representatives and the design team to accomplish the work outlined. The selected GC will be expected to enter into modified AIA A111 and A201 agreement. The GC will work with the designated Project Manager, Rodney Villafranca, to ensure that the construction efforts adhere to the quality, cost, and schedule objectives of the Project. The GC understands that the Owner will monitor the progress of the work, the project schedule and agreed-to GMP. The following Contractors are Pre-Qualified as Prime Bidders: 1. C.A. 6 4. AGREEMENT BETWEEN OWNER AND CONTRACTOR Memorial Hermann Modified standard AIA documents A111 and A201 will be utilized. Refer to the attached copies. Reference Subparagraph of the modified AIA A111 Agreement: The Liquidated Amount shall be $ per day. Reference Subparagraph of the modified AIA A111 Agreement: The Owner is exempt from the payment of sales and use taxes. The selected GC is expected to review the Modified agreement in detail prior to submitting their response to the Owner. Include any and all proposed qualifications to this agreement. The Owner will evaluate qualifications proposed and will take such qualifications into account when determining a final selection for GC on the Project. Houston, Texas Page 6

7 5. PROPOSAL PROCEDURES The purpose of this Request for Proposal is to select a GC on a competitive basis to provide total construction services to accomplish the successful execution of the work. Each participant is requested to provide through Ebuilder Format: a stipulated Fixed Fee a detailed estimate of General Conditions the Cost of Work The sum of these items will be considered the GMP. Additionally, respondents to this RFP are requested to provide a Project Schedule identifying by calendar days the construction, commissioning, and post-construction closeout phases of the work and to provide any and all critical milestone dates. In addition, a list of the proposed construction team for the duration of the project. The GMP for the project, including the Fixed Fee, General Conditions, and Cost of Work, will be fixed upon the Owner s acceptance of the respondent s GMP proposal to be based upon 100% Construction Documents for the Project. In addition to fees and schedules, the Owner s acceptance of proposals will be based significantly on the bidders past experience and current staffing. I. ALL PROPOSALS MUST INCLUDE THE FOLLOWING: A. A fully executed AIA qualification statement. B. A detailed construction schedule with milestones, and a phasing/logistics plan for the project. C. Bid qualifications, if any. D. The respondent s acceptance or qualifications to the Form of Agreement. E. A detailed estimate of General Conditions as outlined in Section 7. F. A completed Project Cost Worksheet per Ebuilder format. G. A list of Proposed Subcontractors per Ebuilder format. H. Safety Record II. ALL PARTICIPANTS SHALL RECOGNIZE AND CONFORM TO THE FOLLOWING STIPULATIONS REGARDING THE SUBMITTAL OF THIS PROPOSAL: A. Each participant, by submitting a proposal, represents that it has read and understands this Request for Proposal and submitted on December 6, 2:00 P.M. B. Participants shall promptly notify in writing the Architect and copy the Project Manager of any ambiguity, inconsistency or error which they may discover upon examination of the Request for Proposal. C. Any interpretation, correction, or change of the Request for Proposal will be issued by architect in written addendum. D. Each proposal shall state that the participant is a sole proprietor, a partnership, a corporation or a legal entity. Each copy shall be signed by the person or persons legally authorized to bind the participant to a contract. Houston, Texas Page 7

8 E. All proposals shall be submitted through the web-based Ebuilder format. Proposals shall be submitted in accordance with the date and time stipulated. Participants shall assume full responsibility for timely delivery at the location designated for receipt of the proposals. These proposals submitted cannot be withdrawn, modified or canceled by the participant for sixty (60) days after the date of proposal submittal. I. All proposals will be opened privately. The Owner will not issue proposal results. J. The Owner shall have the right to reject any or all proposals that are in any way incomplete, inconsistent or irregular with respect to the Request for Proposal. K. It is the intent of the Owner to award a contract for the Project to a responsible General Contractor that offers the best team for the overall requirements of this Request for Proposal. The Owner shall have the right to waive any informality or irregularity in any proposal received and to accept the proposal which, in its judgment, is in its own best interest. L. Prior to the commencement of work, the successful participant shall furnish evidence of insurance covering the faithful performance of the Contract. III. SELECTION OF THE GC WILL BE BASED ON THE FOLLOWING CRITERIA: A. The Guaranteed Maximum Price (Fixed Fee, General Conditions, Cost of Work. B. The detailed Construction Schedule with milestones and logistics/phasing plan. C. The Qualifications and Experience of the proposed team relating to medical facilities D. Qualifications to AIA agreement or Construction Documents. E. The respondent s Safety Record. F. The respondent s Financial and Bonding resources. IV. RFP SUBMITTAL DUE DATE: Submit via E-Builder 2:00 P.M. CST, November 6, 2012: V. 9 6. OWNER SCHEDULE-MILESTONE REQUIREMENTS Construction will begin at the earliest possible date after the Owner s Notice to Proceed is issued. The following is a schedule of the proposed issuance of Contract Documents and Owner occupancy requirements. GENERAL CONDITIONS DEFINITION Prepare a detailed cost estimate of the General Conditions required to complete the entire Project in accordance with the definition of General Conditions Costs per Paragraph 7.2 of modified AIA Document A111 attached hereto. GENERAL CONDITIONS COST SHOULD BE DETAILED AS FOLLOWS: Contractor on-site project staff costs, including burden and benefits Contractor project-related office support and accounting services Contractor project office costs, including equipment, supplies, utilities and maintenance. Telephone and data service (equipment and monthly utility costs) Project photos and or videos Miscellaneous office expenses / postage / messenger Pre-construction services (including, but not limited to, estimating and purchasing) All temporary job site facilities (including installation & maintenance, etc.) Hoisting / Lift (including temporary elevator / lift protection and operation) Clean-up, including debris removal from site Final cleaning See Project Cost Worksheet Contractor s Liability Insurance Trade association and/or AGC fees Safety programs and expenses (including safety equipment and supplies) Surveying and equipment Security Temporary signage Other consultants (please specify) Job vehicles, staff autos, fuel, maintenance, tires, etc. Miscellaneous printing costs Scheduling Cell phones and radios Other miscellaneous costs Houston, Texas Page 10
