1 ADDENDUM OSU and the A&M Systems Purchasing Department ADDENDUM P MWC ADDENDUM # 3 Issue Date: July 5, 2017 The Purpose of this Addendum # 3 is to provide Questions and Answers, Contractor Requirement Clarifications, A New Bid Form as a result of Addendum 2, and the Pre-Bid Conference Sign-In Sheet. Questions and Answers 1 Page Contractor Requirement Clarifications 1 Page New Bid Form 3 Pages o This new bid form replaces the bid form that was sent out with the original bid packet and should be used in its place, as Add Alternate 1 was removed by Addendum 2 Pre-Bid Conference Sign-In Sheet 1 Page All other specifications remain unchanged. All inquiries during the bid and evaluation process must be directed to the Buyer of Record, OSU and the A&M Systems Purchasing Department, by phone at , fax , purchase@okstate.edu, or if you are responding to an online solicitation please use the associated Question and Answer Board. 2 P MWC ADDENDUM # 3 Questions and Answers 1 NOC Utility Extension Issue Date: July 5, 2017 QUESTION 1: Please confirm request for pricing for Alternate #1. I do not recall it being mentioned at pre-bid meeting. ANSWER 1: Alternate #1 was removed in Addendum #2. QUESTION 2: Project specification starting on page 8 of 247, reference Bid package #1 and Bid Package #2. Drawings only support Bid Package #1. Please confirm this bid is only for Bid Package #1. ANSWER 2: This bid package is for Bid Package #1 ONLY. QUESTION 3: Revised drawings that show Hot water and DCW entry and routing locations were said to be UPDATED and distributed this week. How will those drawings be incorporated into the bid documents and the current drawing(s) excluded? a. Will this drawing also identify complete Alternate #1? ANSWER 3: The drawings are incorporated as Addendum #2. (a) See answer #1. QUESTION 4: Natural Gas Line. Please identify desired separation of tasks between contractor and OSU in regards to piping, valves, valve boxes, bedding material, tracer wire, marking tape, backfill/compaction and as built drawings. ANSWER 4: When excavating the ductbank trench a bench should be excavated to the Natural Gas Line depth on the north side of the ductbank. When the ductbank is installed the OSU personnel will install the Natural Gas Line prior to the contractor backfilling the bench area on the north side of the trench. OSU personnel will install all Natural Gas Line components including the bedding material, marker tape and tracer wire. The OSU surveyor will as-built the Natural Gas Line. The contractor shall backfill and compact the excavated bench and trench. OSU personnel will be readily available to coordinate the work with the contractor. Questions and Answers Page 1 of 1

This addendum consists of 1 page: Contractor Requirement Clarification This project will require working in electric vaults with energized feeders. To be awarded this project, the contractor shall have personnel with the required OSHA training for working in vaults with energized feeders.
