2 Page 1 of 8 RFI QUESTIONS AND ANSWERS to RFP no /ME Renovate Old MRI and Upgrade in RB.2625 Project RFP Addendum 2, Dated: January 28, 2015 RFI No. 1 Forty-Five Questions Question 1: Will there be any scopes of work that will be required to be performed after normal business hours or on weekends? Answer 1: Per the drawings and specifications, there will be types of work that will be restricted to after hours. Refer to General note 20 on sheet A Some work will require after hours and weekend schedule, such as MEP shutdowns, crane placement and delivering the MRI into the building. The GC should consider those actions as after hour activities. Question 2: Will "B.I.M." drawings be required on this project? Answer 2: No. Question 3: Specification section references the OCIP program. Is this to be an OCIP project? Answer 3: This Project is not under an OCIP program. Question 4: Please provide the scope and duration for the demolition/ equipment removal to be completed by GE. Answer 4: Existing MRI Unit and associated components demolition/disassembling is forecasted to be days. Question 5: Please confirm that MDA s rigging contractor will remove and put back the exterior grating for removal of the old equipment and delivery of the new equipment. Answer 5: This task is to be included as part of the scope of work by the GC. GC may contract with the rigging company to accomplish this task.

3 Page 2 of 8 Question 6: Is the contractor to include traffic control for the delivery and removal of equipment? If so, please issue a traffic control plan with UTPD officer requirements. Answer 6: The GC is required to coordinate with UTPD for traffic control. MDACC is assuming that TMC will not be involved, since the crane will be located on MDA interior drive. A Traffic control plan is available and will be provided. Question 7: Please confirm that GE will provide floor protection during removal of the equipment and delivery of the new equipment. Answer 7: GE normally provides mats to allow their dolly to roll. This will have to be coordinated by the GC to ensure floor protection is adequate. Question 8: Will there be any conflicts installing reinforcing steel at the new MRI slab? Should fiber mesh be used in lieu of reinforcing steel? Answer 8: Fiber mesh should be used. Structural drawings were produced based on the understanding that mild reinforced steel would be used. On 5/S the slab specified underneath the MRI per GE specifications is revised to 4"(min) cover. Question 9: Will new trench at MRI require fiberglass or fiber mesh? Answer 9: Fiber mesh should be used. Same comment as above, except that this area is less critical due to the fact that it is not in proximity to the equipment. Question 10: Will it be acceptable to abandon the existing cable trench in place and infill? Answer 10: The Architectural and Structural drawings show this trench to be filled. Question 11: Please specify what hours are acceptable for concrete delivery and pouring. Answer 11: The exact hours will need to be coordinated with MDACC, but this operation will have to occur after normal working hours.

4 Page 3 of 8 Question 12: Is the existing CMU grouted solid, specifically where new channels are meant to brace? If not, should an L be added to the underside of the CMU to the end of the channel for bracing? Answer 12: We do not know the grouting of the CMU cells. We assume you are referring to 4S2-00. No L shape is required. Question 13: Is Thermal Protection the only roofing contractor allowed? Answer 13: Yes. Question 14: Opening 3 at the areaway is being replaced by a 6 wide opening. To better tolerate future equipment and expansion please consider removing CMU to 9 6 and installing an 8 door opening. Answer 14: The project is to be bid as drawn. Question 15: Typically temporary barriers are built up to the ceiling grid however, MDACC inspectors require these barriers to be drywall, taped and floated to deck. Is it acceptable to construct these barriers up to the ceiling only? Answer 15: Bid the project with the temporary wall in the corridor up to the ceiling. Question 16: Temporary construction barriers shown on sheet A-101 show that there are several rooms that will be inaccessible for a time. Is it acceptable to not have access to these rooms, or is it necessary to modify the temporary partition plan? Answer 16: The location and duration of temporary barriers shall be reviewed and finalized once a contractor is selected. It is not the intent of the project to have rooms that are inaccessible. Question 17: Please confirm that the temporary partition plan is not to be followed as shown, and that the intent is that MDACC and contractor develop a temporary partition plan.

5 Page 4 of 8 Answer 17: Yes, it is the intent of the project management team to develop a final temporary partition plan with the contractor and MDACC. Question 18: Please confirm AWP-1 is not used in this project as shown on finish schedule, A-602. Answer 18: AWP-1 is not used. AWP-2 should be applied to all walls inside the MRI Procedure Room. Question 19: Are any of the existing floor finishes carpet; will the carpet glue will need to be removed? Answer 19: No. Question 20: Per finish schedule phase one on sheet A-602, corridors are to receive resilient base. There is an existing terrazzo floor with coved terrazzo base in these corridors. Is the intent to patch the existing terrazzo where it is disturbed/at new openings, or to replace all terrazzo base with a resilient base at these floors? Answer 20: The corridors outside of the MRI suite should have finishes that match existing finishes. Question 21: Please confirm that HVAC TAB and commissioning will be the responsibility of the general contractor. Answer 21: Yes, HVAC TAB and commissioning will be the responsibility of the general contractor. However, MDACC may select a third party to check the TAB/GC work. Question 22: Will a baseline pre-read of existing HVAC components or CFMs be required? Answer 22: Note will be added to M-201 for pre-tab air readings. Question 23: Does MDACC have a preferred TAB firm for this project? Answer 23: Engineered Air Balance is the preferred TAB firm, but other ones can be proposed. Based on previous project work history, MDACC has a good rapport with Engineered Air Balance.

6 Page 5 of 8 Question 24: Please confirm that the contractor will be allowed access to the existing chase on levels above through which existing cryogen vent will be demolished. Answer 24: Access to the chases will be allowed with the proper notice and due diligence during the project. Question 25: Are there any other systems in the chase housing the existing cryogen vent that is to be demolished? Answer 25: This will be determined at the time of demolishing the line. Question 26: What is the purity of the existing water? Answer 26: The purity of the existing water is not known. Question 27: Will GE require assistance from the MEP contractors for the removal of the existing equipment and installation of the new equipment? Answer 27: Per previous installation, GE is requesting assistance by the project electrician at the time of removal of existing equipment and installation of the new equipment. Question 28: Notes 1 and 2 on sheet M-202 state that the existing exhaust fan is to be replaced. Are the existing circuits to be replaced as well? Answer 28: Yes, refer to Notes 21 & 22 on 2E300. Question 29: Please specify which, if any, existing dual duct terminal units are pneumatically controlled. Answer 29: Have shown 3 dual duct boxes to be reused. Assume all are pneumatically controlled. Note 18 on M-201 asks for all dual duct boxes to be modified with DDC controls. Question 30: Please confirm that new and/or additional return air and transfer ducts are not required.

7 Page 6 of 8 Answer 30: All return air and exhaust is ducted. No transfer air ducts are used. Question 31: Please confirm that it is acceptable to surface mount fire alarm devices on plaster walls. Answer 31: Surface mounting fire alarm devices on existing plaster walls is acceptable. Question 32: Will MDACC IT be responsible for removing and reinstalling telecom and data devices? Answer 32: Yes, MDACC IT will be responsible for removing and reinstalling data cabling. Question 33: Will MDACC IT or contractor be responsible for raising and installing supports for data cabling in phase 1 corridors? Answer 33: Yes, MDACC IT will be responsible for raising and reinstalling data cabling supports. MDACC IT will require full cooperation and coordination of the GC. Question 34: Please advise if the contractor will be required to use a third party reviewer for sprinkler and fire alarm submittal reviews. Answer 34: Sprinkler and fire alarm submittals will be reviewed by MDACC. Question 35: Per sheet S-200 notes 4-6, will there be an opportunity to evaluate the site with contractor s selected structural engineer prior to bidding? Does MDACC have these existing calculations; does this note apply? Answer 35: This note does apply to the project. MDACC can confirm access to the site. We are not aware of any existing structural information. Question 36: Please clarify who is to furnish the Ergotron wall mounted patient station. CFCI per note 20 on sheet A-104, and OFCI per note 28 on A-401. Which is correct?

8 Page 7 of 8 Answer 36: This item is to be Contractor furnished and Contractor installed. Question 37: Please label the detail shown at key note 18 (removable RF shielding partition) on sheet A-104. Answer 37: Enlarged plan reference is 2 on sheet A-505. Question 38: Please specify what key note 46 on sheet A-104 should denote. Shown on drawings, but not defined. Answer 38: Note 46 reference on the plan should be deleted. It is not used. Question 39: Please indicate location of key note 45 on sheet A-104. Defined in key notes, but not found on drawings. Answer 39: This note should be a General Note on the sheet and not a Key Note. Question 40: Please specify where the condition shown in detail 4/A-108 occurs. Answer 40: This detail occurs at the fur down in the MRI Procedure room. Question 41: Please provide cut detail 16/A-504. Detail is indicated at 7B/A-401, but does not exist on sheet A-504. Answer 41: Refer to Construction Document Addendum Number 1. Question 42: Note 7 and 8 on sheet MD-201 states to demo existing vent and exhaust piping back to the second floor roof. Please advise on the extent of rework associated on level 1 to accomplish this. Answer 42: The extent of work cannot be determined until the chase is opened. Based on drawing M-102, it appears that the chase stacks so there should not be a lot of rework but without destructive site investigation we cannot be certain.

9 Page 8 of 8 Question 43: What is the floor to deck height in the basement? Answer 43: The floor to floor dimension is 15'. The deck thickness is 6". The beam depth varies. Question 44: Will sound pads be required at electrical back boxes and special systems? Answer 44: Yes, sound pads will be required. Question 45: Will the work in the corridors be able to occur during regular working hours since the corridors are not patient areas? Answer 45: These corridors are 24/7 occupancy. These corridors are not patient areas, but are MDACC maintenance areas. Areas around the project area is occupied by Facilities and Staff. MDACC is requesting GC assistance with constructability at phase one corridors. Some work, especially in front of Facilities Offices and Storage may be required to be after hours work.
