1 January 11, 2013 Addendum #4 Haywood County Auxiliary Services and Haywood County Auxiliary Services: Building Shell Renovation Waynesville, North Carolina The following changes, revisions, additions, and/or clarifications shall be made in the Drawings and Specifications and this addendum and all other addenda shall be a part of the Contract Documents. Please acknowledge receipt of this addendum in the place provided on the Proposal Form. Item #G-1 Proposal Forms with Unit Prices: The revised proposal forms with the required unit prices are attached. Item #G-2 Ceramic Tile: See Section Ceramic Tile page 4 Tile Products: 3 x3 tile is specified, other sizes will be considered. The cove base can be a 6 high x 8 or 12 long. Glazed wall tile is not used. Section Ceramic Tile page 9 Tile floor installation: waterproofing is only at floor wall juncture since this is slab on grade? Item #G-3 Pre-Bid Signup Sheet: See attached signup sheet for the pre-bid conference that was held at 3pm on January 10, 2013 at the project site. Item #G-4 Pre-Patio Material: Reference the site plan on sheet A0.1, the patio material is concrete same construction as the sidewalks. Item #G-5 Down Spouts to Storm Drains: See sheet A4.0 for downspout locations. All downspouts are to tie into a PVC receptor and connect to a 4 schedule 40 PVC pipe that are then tied to a 6 schedule 40 PVC pipe running the length of both sides of the facility near the new sidewalks. Allow for 900 feet of 6 pipe and tie to the existing catch basins. The exiting catch basins are located next to the existing building (3 total) and the existing yard inlet that is shown on sheet A0.1 and A0.3 at the end of the new Swale. Item #G-6 Existing Concrete Floor Prep: Refer to note 21 on sheet A1.0 The existing concrete floor is to be patched at all large cracks and grind out the high spots in the repair areas only. It is not the intent to for the entire existing slab to be grind smooth and leveled. The floor slab is to be sealed in accordance with the floor coverings manufactures recommendations. Item #G-7 Asbestos Testing: See allowance in the specifications page GR-11. The testing allowance can be used for asbestos testing.

2 Item #G-8 Permits and Tap fees: All permits and tap fees including landfill dumping fees are waived on this project. County will provide main water line hookup from the street to the building and shown on fire protection plan sheet FP1.0 and plumbing drawing sheet P2.0 Item #G-9 Roof Trusses: metal Plate connected wood trusses are specified. Refer to sheet A1.0 roof framing plan. The trusses indicated with a solid line are to new trusses as specified. The dashed trusses are exiting to remain and can be repaired, sistered up, or replaced. Item #G-10 New Concrete Block Knee wall: See drawing A1.0 drawing #4. Note 10 indicates the length of new CMU knee wall to be added on top of the existing footings. Fill new block wall cells with pea gravel concrete and provide ½ x 16 ABs 32 O.C. Item #G-11 Fill for Sidewalks: See sheet A1.0, the 8 to 10 of soil overburden to be removed for installation of the new parking lot base and 2 asphalt can be used to fill along the north side of the facility for support of the new sidewalk. The new side walk on the lower side shall have a turn down edge at least 24 in height. In is the intent of the drawing to have the parking lot match the existing slope of the site. The Bidder further declares that: he has examined the site of the Work and informed himself fully in regard to all conditions pertaining to the place where the Work is to be done; he has examined the Specifications for the Work and the Contract Documents relative thereto, and has read all special provisions furnished prior to the opening of bids; and he has satisfied himself relative to the Work to be performed. The undersigned further agrees that in case of failure on his part to execute the said Contract and the required bonds within ten (10) consecutive calendar days after the award of contract, the check, cash or bid bond accompanying his bid shall be paid into the funds of the Owner's account set aside for this Project, as liquidated damages for such failure; otherwise the certified check, cash or bid bond accompanying this proposal shall be returned to the undersigned.
