1 ADDENDUM #001 Date: March 21, 2019 Wayne State College Bowen Hall JJA Project: 3529 Sealant Replacement and Exterior Renovations Drawings & Specifications 1111 Main Street; Wayne, Nebraska Dated: 03/05/19 The Drawings and Specifications for the above referenced project are hereby amended and the following changes shall be made part of the Contract Documents. ITEM #1 ITEM #2 Refer to the attached SignIn sheet from the PreBid Walkthrough that occurred on Tuesday, March 19, Refer to the Project Manual, Procurement Requirements, Section Invitation to Bid and Section Instructions to Bidders. A. Refer to Paragraph 1.17 of the Invitation to Bid and paragraph 1.24, Subparagraphs B and C of the Instruction to Bidders. As indicated in the specifications, full time onsite labor may begin May 6, 2019 and the completion date required by the Owner is June 14, 2019 with Final completion no later than July 31, Below is additional information about the building occupancy use and Owner required Construction Schedule: Construction Schedule: 1. Work can begin May weeks until June 23 LAUNCH camp with creative scheduling for NSR May 2024 and June NO Construction Operations June Creative scheduling from July 1 13 for football camp June 30July 2 and NSR July NO Construction Operations July July must finish up. Wayne State College Camp dates: 1. June 2329 LAUNCH Workshop #1 No work on the building these dates 2. June 30July 2 WSC Football Can work this one out perhaps leave the south elevation unoccupied. May need to begin work day at 7 am on that Mon & Tue 3. July 1420 LAUNCH Workshop #2 No work on the building these dates 4. During the LAUNCH weeks there cannot be any trucks, fencing, clutter or work around or in Bowen. New Student Registration Days with tours only going to Bowen on: 1. Location for NSR is usually in the SC but may be in other locations this year due to work in the SC. 2. These weeks we will have to work this out so one of the entries on the north or south is available for touring students. 3. May 2024 No work on the north or south elevations 4. June 37 No work on the north or south elevations 5. July 1012 No work on the north or south elevations B. The contractor will be allowed to stage in a portion of the drive east of the building and Parking available in the Northeast Corner of Lot 4. Page 1 of 3

A. Delete to Bid Proposal Form in the booklet contract documents and replace with the attached Revised Bid Proposal Form. (Change wording for Unit Price #2 from twopart silyl terminated polyether sealant to silicone sealant) Refer to the Project Manual, Contracting Requirements, Section Supplementary Conditions. A. Refer to Paragraph 1.3.F.3.a. Commercial General Liability Insurance. Change Policy from (ISO CG ) to (ISO CG ). B. Refer to Paragraph 1.3.F.3.b.12. Limits of Liability. The Owner will not invoke the option of the protective liability policy. C. Refer to Paragraph 1.3.F.3.g.1. Installation Floater. Remove (All RiskSpecial Endorsement, CF1004 Ed. 183: including theft of building materials). Refer to the Project Manual, Division 01 General Requirements, Section Unit Prices. A. Refer to paragraph 3.1.B. Unit Price #2. Change to: 1. Provide a Cost per Linear Foot ( Unit Price ) to install additional silicone sealant for use at precast and masonry, per Specifications, as determined by the Owner/Architect and if outside the scope of the contract. ITEM #6 ITEM #7 ITEM #8 ITEM #9 Refer to the Project Manual, Division 01 General Requirements, Section Temporary Facilities and Controls. A. Refer to paragraph 1.5. Temporary Electricity. At the PreBid Walkthough there was a request for 208 Volt 40 amp Power. Per Wayne State College there is spare 208 power on Eighth Floor (8 th ). Contractor would have to run power cords up the stairwell to the roof. Refer to the Drawings, Sheet A1.1 Ground Floor Plan. A. Confirmation: Crack Injection shall be completed at the 30 columns on the First Floor as noted. Refer to the Drawings, Sheets A2.1 through A2.4 Exterior Elevations. A. Refer to the Legend, and the denoted Steel Angle Brick Ledges shown in Green on the drawings. On top of the steel angle brick ledge there is existing sealant. Contractor shall remove and replace the existing sealant as part of the work at the steel angle brick ledge. Refer to the Drawings, Sheet O1.1 Window Precast Elevations and Section. A. Upon request, refer the partial wall section drawing (next page), at the First Floor Level, through the brick wall area from the original 1965 drawings. B. Note per the original drawings and the partial wall section detail below, the cavity wall flashing should be installed at each floor level. Field Verify. Page 2 of 3

6 BID PROPOSAL FORM, page 2 JJA PROJECT NO UNIT PRICES: The quantity of those materials/procedures which are listed below may be modified by a Change Order in accordance with the following Unit Prices: UNIT PRICE #1: Contractor shall remove unacceptable mortar and tuckpointing joints at brick, per Specifications, which may be determined unacceptable by the Owner/Architect for an additional cost of: Dollars ($ )/Linear Foot UNIT PRICE #2: Contractor shall install additional silicone sealant for use at precast concrete and masonry, per Specifications, as determined by the Owner/Architect and if outside the scope of the contract for an additional cost of: Dollars ($ )/Linear Foot The undersigned further acknowledges and agrees: 1. That the Owner reserves the right to reject any or all bids and to waive any informalities in the bidding. 2. That this Bid shall be good and may not be withdrawn for a period of fortyfive (45) days after the scheduled closing time for receiving bids. 3. That if notified of the acceptance of this Bid Proposal within fortyfive (45) days from the scheduled opening of the bids, the undersigned will enter into and execute an "Owner Contractor Agreement" based upon this Bid, in the form of a stipulated sum agreement, to be attached to and made part of the Contract Documents. 4. That if awarded the Contract, to furnish and deliver to the Owner within fifteen (15) days after the signing of the Contract, a satisfactory Performance Bond and Labor and Material Payment Bond in the form currently issued by the AIA in an amount equal to one hundred percent (100%) of the Contract Sum for the Work to be performed. 5. That if awarded the Contract, the work shall be substantially complete by June 14, 2019 (First through Eight Floor) and Ground Level and Penthouse coordinated with Owner. Final Completion no later than July 31, All bidders must comply with the State of Nebraska s Drug Free Workplace. 7. Either (1) The Contractor is required to use a federal immigration verification system to determine the work eligibility status of new employees physically performing services with the State of Nebraska. Or (2) The Contractor is an individual or sole proprietorship. The Contractor must complete the United States Citizenship Attestation Form, available on the Department of Administrative Services website at 8. Contractor understands that Contracts executed as a result of this bid proposal are subject to statutory public disclosure and public website posting requirements. Information submitted with your bid proposal and written responses are subject to disclosure requirements under the Nebraska Public Record statutes. Information may only be withheld from the public under very limited circumstances. Documents submitted as part of the bid proposal containing trade secret or proprietary information must be submitted in a sealed envelope marked "Proprietary".
