2 GENERAL PLAN NOTES PERMANENT SIGNS PROJECT SCOPE THIS PROJECT INCLUDES REMOVAL OF PORTIONS OF CONCRETE CURB & GUTTER AND HMA SURFACE; INSTALLATION OF SANITARY AND STORM SEWER AND UNDERDRAIN; EXCAVATION OF EXISTING GRAVEL ROAD; INSTALLATION OF CONCRETE CURB & GUTTER AND NON-REINFORCED CONCRETE SURFACE ALONG WITH DRIVEWAYS AND RESTORATION WORK BEHIND THE CURB. THE LENGTH OF THE PROJECT IS 0.49 MILES. EXISTING, PERMANENT SIGNS SHALL REMAIN IN PLACE, EXCEPT WHERE NECESSARY TO COMPLETE THE WORK. SUCH SIGNS WILL BE REMOVED, PROTECTED, SET ASIDE AND.REINSTALLED IN PLACE UPON COMPLETION OF THE WORK. PAYMENT WILL BE MADE IN ACCORDANCE WITH CORRESPONDING PAY ITEMS. SPOIL MATERIAL FROM MACHINE GRADING, MODIFED OPERATION PROJECT TIME: THE MDOT IS PROVIDING SOME FUNDING WITHOUT OVERSIGHT. ACCORDINGLY THE CITY IS RESPONSIBLE FOR ALL PROJECT CONTROLS. HOWEVER, CONSTRUCTION CANNOT BEGIN UNTIL MDOT ISSUES A NOTICE TO PROCEED TO THE CITY OF GREENVILLE. IT IS ANTICIPATED TO RECEIVE THIS NOTICE BY AUGUST 7, 2018 AND CONSTRUCTION CAN BEGIN IMMEDIATELY THEREAFTER. HOWEVER, IF THIS NOTICE IS DELAYED IT WILL BE CONSIDERED JUST GROUNDS FOR AN ADJUSTMENT OF TIME TO THE CONTRACT. PROJECT STAGING: EXCESS MATERIAL REMOVED AS PART OF THE GRADING OPERAION MAY BE USED AS EMBANKMENT BEHIND THE CURB. ON-SITE MATERIAL MAY BE USED TO FORM THE REQUIRED GRADE IN FILL SECTIONS WITHIN THE INFLUENCE OF TRAFFIC ONLY IF IT CONFORMS WITH MDOT CLASS 2 OR OTHERWISE APPROVED IN WRITING BY THE ENGINEER. IT IS ESTIMATED THAT THERE WILL BE EXCESS MATERIAL TO BE REMOVED FROM THE SITE. IF IT IS ADVANTAGEOUS TO THE CONTRACTOR, THE EXCESS MATERIAL MAY BE DISPOSED AT THE GREENVILLE MUNICIPAL AIRPORT LOCATED APPROXIMATELY 4 MILES SOUTH OF THE PROJECT SITE. THE DISPOSAL AREA SHALL BE LEVELED AT THE CONCLUSION OF THE PROJECT IF THIS SITE IS USED FOR DISPOSAL.. THIS PROJECT CAN BE DONE UNDER COMPLETE CLOSURE SOUTH OF DICASTAL WAY, WITH ACCESS MAINTAINED TO RESIDENTS. PART-WIDTH METHODS ARE ANTICIPATED FOR THE AREA NORTH OF DICASTAL WAY, BUT MEANS AND METHODS ARE THE RESPONSIBILITY OF THE CONTRACTOR. EROSION CONTROL THE CONTRACTOR SHALL CONFORM TO THE STATE OF MICHIGAN ACT 347 P.A. OF 1979 EROSION AND SEDIMENTATION CONTROL AND THE NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM (NPDES) TO CONTROL RUNOFF DURING CONSTRUCTION. MOBILIZATION PAY ITEM: THE ANTICIPATED SEQUENCE OF EVENTS ARE AS FOLLOWS, BUT THE CONTRACTOR IS RESPONSIBLE FOR ALL MEANS AND METHODS THE MOBILIZATION PAY ITEM SHOULD REFLECT THE TRUE COST OF ITEMS NOT SPECIFICALLY COVERED ELSEWHERE. THERE IS NO MAXIMUM AMOUNT LIMITATION. MAINTAINING ACCESS: THE CONTRACTOR SHALL MAINTAIN ACCESS TO RESIDENTIAL PROPERTIES; DICASTAL, NORTH AMERICA; AND FOREMOST FARMS AT ALL TIMES, UNLESS SUITABLE, TEMPORARY ARRANGEMENTS HAVE BEEN MADE WITH THE PROPERTY OWNERS AND AGREED TO BY THE CITY. THE CONTRACTOR SHALL PROVIDE AND MAINTAIN ADEQUATE SIGNS, CONES, BARRICADES, ETC TO PROTECT TRAFFIC AND FINISHED WORK NORTH OF DICASTAL WAY AND AT THE CORNER OF INDUSTRIAL PARK DRIVE AND VANDEINSE AVENUE IN A MANNER ACCEPTABLE TO THE CITY. PAYMENT FOR MAINTAINING ACCESS SHALL BE COVERED UNDER THE MINOR TRAF DEVICES PAY ITEM. 1. SET UP EROSION CONTROL ITEMS PRIOR TO ANY WORK BEING DONE. 2. REMOVAL OF PAVEMENT. 3. SANITARY SEWER WORK 4. STORM SEWER WORK, INCLUDING UNDERDRAIN. 5. GRADING AND COMPACTION OF SUBGRADE. 6. CURB AND GUTTER. 7. PAVING. 8. DRIVEWAYS. 9. STRIPING. 10. RESTORATION BEHIND CURBS. MISCELLANEOUS QUANTITIES CONTROL POINTS CONTROL POINTS FOR THE PROJECT ARE SHOWN ON THIS SHEET AND SHALL BE PROTECTED BY THE CONTRACTOR FOR THE DURATION OF THE CONSTRUCTION. THE ELEVATIONS SHOWN ARE BASED ON THE USGS DATUM. IF CONTROL POINTS ARE CONFLICTING WITH THE WORK, THE CONTRACTOR SHALL NOTIFY THE ENGINEER TO HAVE THEM RELOCATED. FAILURE TO NOTIFY THE ENGINEER WILL RESULT IN THE CONTRACTOR HAVING TO PAY FOR REESTABLISHMENT OF LOST CONTROL POINTS. THE ITEM "MAINTENANCE GRAVEL" IS ADDED AS A MISCELLANEOUS QUANTITY TO BE USED TO MAINTAIN ACCESS TO RESIDENTS OF. IT WILL BE PAID FOR BY THE TON AS DETERMINED FROM LOAD TICKETS DELIVERED TO THE SITE AND USED FOR THIS PURPOSE. 10 TONS IS THE QUANTITY INCLUDED FOR BIDDING PURPOSES. THE ITEM "SUBGRADE, UNDERCUTTING, TYPE II" IS ADDED AS A MISCELLANEOUS QUANTITY TO BE USED TO REMOVE / REPLACE UNSUITABLE MATERIAL IN THE SUBGRADE AS NECESSARY. 1,500 CYD IS THE QUANTITY INCLUDED FOR BIDDING PURPOSES. UNDERGROUND UTILITIES SANITARY SEWER LATERALS: FOR PROTECTION OF UNDERGROUND UTILITIES AND IN CONFORMANCE WITH PUBLIC ACT 53, 1974, THE CONTRACTOR SHALL CALL MISS DIG IN ACCORDANCE WITH COMMON PRACTICE. DEPTH AND MATERIAL OF EXISTING UTILITIES ARE SHOWN ON THE PROFILE SHEETS, IF KNOWN. THE CONTRACTOR SHALL CONDUCT HIS OPERATIONS IN SUCH A WAY AS TO NOT HARM EXISTING UTILITIES. TRAFFIC CONTROL ITEMS THIS WORK SHALL BE DONE IN ACCORDANCE WITH THE APPLICABLE REQUIREMENTS OF SECTION 812 OF THE 2012 MDOT STANDARD SPECIFICATIONS FOR CONSTRUCTION. THE CONTRACTOR IS ADVISED THAT THE 2011 MICHIGAN MANUAL OF UNIFORM TRAFFIC CONTROL DEVICES IS HEREBY ESTABLISHED AS GOVERNING ALL WORK IN CONNECTION WITH TRAFFIC CONTROL DEVICES, BARRICADE LIGHTING, ETC. REQUIRED ON THIS PROJECT. PROTECTION OF ADJACENT WORK SANITARY SEWER LATERALS ARE SHOWN ON THE PLANS IN APPROXIMATE LOCATIONS AND PAID FOR AS SANITARY SEWER, CL A, 4-INCH, TR DET B ACCORDING TO THE SPECIAL PROVISION. THE ACTUAL LOCATION SHALL BE DETERMINED IN THE FIELD BY THE ENGINEER. PROJECT COORDINATION INFORMATION THIS CONTACT INFORMATION REPRESENTS THE BEST INFORMATION AVAILABLE AT THIS TIME. IT IS PRESENTED TO ASSIST THE CONTRACTOR IN COORDINATION EFFORTS. THE CITY IS NOT RESPONSIBLE FOR FAILURE OF THE CONTRACTOR TO CONTACT THE PROPER PERSON OR REPERCUSSIONS OF THAT FAILURE. NOTES THE NATURE OF THE PROPOSED WORK WILL RESULT IN NEW MATERIALS ABUTTING OR ADJOINING EXISTING MATERIALS. THE CONTRACTOR SHALL PROTECT EXISTING MATERIALS DESIGNATED TO REMAIN IN PLACE. DAMAGE TO EXISTING MATERIALS BY THE CONTRACTORS OPERATIONS SHALL BE REPAIRED TO EQUAL OR BETTER CONDITION AT THE SOLE COST OF THE CONTRACTOR. SIDEWALK RAMPS & DETECTABLE WARNING DEVICES SIDEWALK RAMPS SHALL BE PLACED AS SHOWN ON THE PLANS. ALL RAMPS SHALL CONFORM WITH ADA REQUIREMENTS WITH DETECTABLE WARNING DEVICES. MOST RAMPS WILL BE TYPE R, AS SHOWN ON THE STANDARD PLAN. DETECTABLE WARNING DEVICES SHALL BE DUCTILE IRON WITH BLACK COATING. BACKFILL BEHIND CURB & GUTTER BACKFILLING BEHIND NEW CURB AND GUTTER WILL BE PAID FOR AS PART OF THE MACHINE GRADING, MODIFIED PAY ITEM. THE CALCULATED QUANTITIES OF MATERIAL ARE REFLECTED IN THE TABLE ON SHEET 4. 2
