1 Wisconsin REQUEST FOR BIDS Sealed bids for the City Hall & Library Carpet Replacement & Installation will be received and read at 2:00 pm on Friday May 27, 2016 at the office of City Clerk,, 111 S. Main Street, Waupaca, Wisconsin Bids must be sealed and clearly labeled on the outside of the envelope, the following: Sealed Bid: The reserves the right to reject any and all bids, waive any bidding formalities and otherwise award a bid that is in the best interest of the city of Waupaca. Bid Guaranty: A certified check or bank draft payable to the Treasurer of the, or a satisfactory bid bond, in an amount not less than five percent (5%) but not more than 10 percent (10%) of the estimated bid amount shall accompany each bid as a guaranty that if the bid is accepted the bidder will execute and file the proper contract and bond within fifteen (15) days after the acceptance of the bid. If the successful bidder fails to execute and file the required contract and bond, the amount of the check or the bid bond shall be forfeited to the as liquidated damages. 2 Wisconsin REQUEST FOR BIDS 1. SCOPE OF PROPOSAL This Request for Bids is for qualified contractors who wish to submit bids for the CITY HALL & LIBRARY CARPET REPLACEMENT & as specified in the request for bids and state hereinafter. The Contractor must provide a bid that includes all labor, materials and associated costs to complete the work. It is recommended that all contractors independently measure for actual required square footage. This scope assumes all labor, material, shipping, delivery, disposal and any other necessary work not identified would be included in the bid prices. Contractor shall perform the following: 1) Tear-out & Replacement of approximately 625 SY of City Hall carpeting & 1,022 LF cove base a. Any jacking, lifting, moving, of furnishings shall be incidental to Tear-out & Replacement. 2) Tear-out & Replacement of approximately 1,462 SY of Library carpeting & 1,065 LF cove base a. Any jacking, lifting, moving, of book racks and furnishings shall be incidental to Tear-out & Replacement. 3) Tear-out of approximately 48 SY of stairwell carpet 4) Replacement of stairwell carpet with vinyl stair tread & riser cove Contractor shall submit: 1) All material information on manufacturer, warrantee, and grade. 2) Indicate direct manufacture purchase by Owner on BID FORM. Owner shall provide: 1) Access to facilities 2) Plans, specifications, and drawings as needed 3) Bottom shelving of library racks shall be emptied by Owner to allow access for carpet replacement. 2. 3 3. PRE-QUALIFICATION All bidders must complete a Bidder s Proof of Responsibility as provided by the Owner. PROJECT TIMELINE 2 pm on Friday May 27, 2016 Bids Due Tuesday, June Bid Awarded by Common Council June 8 17, 2016 Contract documents prepared & executed September 16, 2016 Work completed 6. COMPLETION DATE All work on shall be completed by end of working day, September 16, A $ fine will be imposed for each and every day the project is not completed. The above deadline may be extended due to extenuating circumstances. Any extension must be approved in writing by the Owner. It is anticipated the Library will be closed several days for carpet replacement, contractor will make every effort to minimize Library down-time. The Contractor shall provide written notice 15 calendars prior to work in Library. Contractor shall provide a brief narrative and project timeline describing essential features of the work with anticipated completion date. 7. BID AWARD In accordance with the procurement code, the award of the bid will not be based solely on the lowest price. Other factors such as the Contractor s experience, quality of product proposed, compliance with bid specification, quality of reference responses and other related considerations will be evaluated when awarding the bid. 8. INSURANCE The selected contractor shall maintain adequate insurance in an amount sufficient to protect the City of Waupaca from any and all public liability and workers compensation claims at all times during the performance of this contract. A copy of the contractor s insurance with the named as an additional insured will be required prior to signing of the contract. 9. CONTACT PERSON All comments, questions, request a site visit, or request additional information regarding this proposal shall be made to Justin Berrens, Director of Public Works/City Engineer, (715) or Russ Montgomery, Facilities Superintendent, (715) PROTECTION OF EXISTING FACILITY AND EQUIPMENT The contractor shall protect all existing equipment and facilities and shall be responsible for any damaged done to those facilities as a result of this work.

4 11. WARRANTY The contractor shall provide manufacturer warranty and 1 year guarantee on installation. 12. PREVAILING WAGE There is no prevailing wage requirement for this project. 13. OWNER CONTRACT The Contractor must execute a copy of the Owner s contract and sample of which is included with the bid packet. 14. REFERENCES The Contractor shall provide the names and contact information of three projects of similar nature, completed in the last 3 5 years. 15. AS-EQUAL BID SUBMITTALS All variances from the project specifications shall be clearly identified by the bidder in the bid submittal. Justification that the identified variance will provide for an as-equal or comparable product or results must be submitted. Final determination that the variance is acceptable and is considered as-equal to the stated specifications is to be made by the.
