3 23. Field Office Special Provision No. The Field Safety Audit ust be conducted and being satisfied with prior to the coenceent of the project. Field office ust be in place prior to coenceent. Payent at the Contract Price shall be full copensation for all labour, equipent and aterials for the supply and placeent and relocation of the Field Office. 38. Restoration Work Special Provision No. GN121M01 Septeber 2012 Restoration of Driveways, Sidewalks and Private Walkways Driveway thicknesses for residential, industrial and coercial properties shall be according to T Within driveways, sidewalks and private walks, all trenches and excavations shall be restored with a 150 depth of Granular A according to TS 1010 copacted to 100% of axiu dry density as the driveway, sidewalk or walkway base. All driveways shall be saw cut in a straight line. All asphalt or concrete driveways shall be paved fro trench to the curb so that driveways will only have one straight joint. If short services are located within the asphalt driveway, the driveway shall be paved fro the service connection pit to the curb or fro the connection pit to the back of sidewalk, so that driveways will only have one straight joint. The Contractor shall restore any asphalt, concrete and interlocking stone driveways disturbed during construction to equal to or better than existing condition. Driveway concrete curbs, including curb returns, sidewalks and private walkways ipacted by construction shall be reconstructed to original condition or better. All costs associated with this Work shall be included in the Contract Price for sanitary, stor, waterain and water service installation. No separate payent shall be ade. Roadway Asphalt Paveent Within the paved roadway, all trenches and excavations shall be restored with unshrinkable fill to the existing subgrade plus, granular, concrete, and asphalt paveent aterials to atch existing paveent conditions. Teporary paveent surface restoration following waterain/sewer installation shall consist of HL8 HS asphalt placed to atch existing surface elevations. Thickness range of the existing paveent coponents will be as noted within the Addendu No. 1, TCN of 13

4 Geotechnical Reports. The edges of the existing paveent are to be coated with SS-1 eulsified asphalt. Roadway curb and curb and gutter, and sidewalk ipacted by construction shall be peranently repaired with concrete as per TS The Contractor is advised that the City will not consider additional payent for the restoration of any over breaks that ight occur at the edges of the trenches. All costs associated with this Work shall be included in the Contract Price for sanitary, stor or water service installation. No separate payent shall be ade. The peranent restoration of paved areas outside of the trench to follow TS4.60 and shall include cold illing of 40 depth of asphalt and the placeent of 40 of copacted HL1 asphalt in excess of the trench. Length and width of the illed and HL1 asphalt areas shall be as per TS The cold illing and HL1 asphalt ites will be paid under separate ites. Boulevard Sodded Areas Within sodded boulevards, all trenches and excavations backfilled with granular or select native aterial shall be restored with 100 topsoil and sod according to TS 5.10 and TS 5.00, respectively. All other boulevard surfaces such as unit pavers, interlock stone, flag stone, concrete, etc. shall be restored to eet or exceed original conditions. All costs associated with this Work shall be included in the Contract Price for sanitary, stor or water service installation. No separate payent shall be ade. Curb Subdrains The Contractor shall ake peranent repairs to all existing curb subdrains to atch existing conditions or better. Any aterials which cannot be salvaged shall be supplied by the Contractor. All costs associated with this Work shall be included in the Contract Price for sanitary, stor or water service installation. No separate payent shall be ade. The Contractor is strongly advised that the City will not consider additional payent for any trench collapse, additional excavation and / or additional unshrinkable fill due to trench collapse etc or for the restoration of any over breaks that ight occur at the edges of the trenches. The contractor is also advised to pay attention to the fact that the contract will include the peranent repair/restoration of the roadway, affected driveways, walkways, curb, curb & gutter, Addendu No. 1, TCN of 13

5 side walk and boulevard areas. The peranent repair/restoration Work shall be copliant with the City standards and specifications. All trenchwork and cuts related to the contract within project liits in the public right of way including the trenchwork & excavaton in the paved roadway will be peranently repaired. No teporary repair related to the contract to be left for transportion services at the copletion of the contract. All costs associated with the repair/restoration Work shall be included in the Contract Price for water or sewer ainline or service installation and copleted to the satisfaction of the Contract Adinistrator. No separate payent shall be ade except for the approxiately 3.5 wide surface asphalt in excess of the ainline trench and the horizontal drilling access pits in the roadway which will be paid under the appropriate tender ites 46. Excavation of Soft Spots for Roadway, Concrete Toe Wall, Ebankent Area and Sidewalk Special Provision No. RD408M01 Septeber 2012 When the Contractor encounters soft areas, the Contractor shall notify the Contract Adinistrator iediately. The Contract Adinistrator shall direct the Contractor as to the width, length and depth of the soft spot volue. In either case, the over excavated area shall be backfilled in the following anner: 1. If the existing ground is still unstable, the Contractor shall place geotextile fabric over the entire section. If the area has stabilized, the geotextile fabric shall not be required at this stage. 2. The over excavated area shall be backfilled with 50 crusher run liestone for roadways, Granular A for concrete toe wall and sidewalk or other suitable aterial deterined by the Contract Adinistrator to the subgrade elevation; 3. Geotextile fabric (Terrafix Geogrid BX 1500 for roadway and terrafix 360R for concrete toe wall, sidewalk and ebankent area) shall be placed across the section, between the existing subgrade aterial and the Granular Sub-base. The Contractor shall sooth the subgrade by rolling, if possible, or by other eans to provide a consistent grade for placeent of the fabric. The fabric shall be placed flat with no folds and shall overlap the trouble area by a iniu of 1.0 etre in all directions. If ore than one piece of fabric is required, the pieces shall overlap each other by a iniu of 500 in all Addendu No. 1, TCN of 13

6 directions. The Contractor shall pin the fabric in place with anufacturer approved staples, prior to the placeent of granular aterial. 4. The geotextile fabric shall be iediately covered with the full depth of granular road base as specified in the Contract Docuents, to provide the axiu stability possible. 5. If possible, the Contractor shall barricade off the area until such tie as the paveent is placed or filled. Payent at the Contract Price shall be full copensation for all labour, Equipent and Material to do the Work The supply and placeent of the geotextile fabric shall be paid under separate ite. 79. Flush, Clean and CCTV Inspection of Sewers Special Provision No. SW302M01 March 2015 Prior to the start of construction, the Contractor shall power-flush or hand and achine-clean or both the existing sewers, aintenance holes and catch basin leads. The Contractor shall deploy proper flushing and cleaning ethods required to reove all debris including scoops, buckets, shovels, vac-all equipent and sewer flushing equipent. The aterial shall be reoved fro the sewer syste and cannot be flushed further down the sewer. The aterial reoved shall be dried and ixed in equal parts with sand. The resulting aterial is then to be disposed of as a non-hazardous, non-registerable solid industrial waste by the Contractor. Iediately following the flushing and cleaning, the Contractor shall assure the Contract Adinistrator that the existing sewers, aintenance holes and catch basin leads are clean. The Contractor shall videotape the existing sewers, aintenance holes and catch basin leads according to TS 409 and supply the CCTV report along with a video on digital video disc (DVD) to the City for joint review between the Contract Adinistrator and the Contractor. The Contractor shall note that if excessive debris is encountered in the sewers additional highpressure flushing shall be copleted and included as part of the work. Payent for the initial flush, clean and CCTV work shall only apply for pre-construction activities. The payent will be ade only if properly supported by Inspector s report and the sub-contractor s invoice(s) for sewer flushing and videotaping. Post-construction flushing, cleaning and videotaping of existing sewers, aintenance holes and catch basin leads shall be considered part of the Contractors quality control process and the cost associated with this work shall be borne by the Contractor. Under no circustances shall additional payents be ade. This work ust also be undertaken iediately following surface Addendu No. 1, TCN of 13

7 course asphalt paving, topsoil and sodding to confir that no debris entered the sewer systes as a result of construction. The DVDs shall be subitted to the Contract Adinistrator for review within two Days of videotaping. Any daage caused to the sewers or structures by the Contractor because of their operations shall be repaired at no extra cost to the City. If aintenance holes are found to be filled with excess aterial following construction, the Contractor shall clean these out and dispose of the aterial at no extra cost to the City.
