2 REQUEST FOR PROPOSAL Flooring The Park Hill School District (DISTRICT) is seeking proposals from qualified providers (CONTRACTOR) for the materials and installation of flooring at various schools in the Park Hill School District. Sealed proposals will be received by the Operations Supervisor at the Support Services office located at 8500 NW River Park Drive Pillar 116, Parkville, MO on March 14, 2016 at 10:00 AM. Proposals will be opened at that time. Once the evaluation process is complete, the information will be available to all who responded. Said proposals must conform to the specifications and instructions. The DISTRICT reserves the right to reject any and all proposals and to waive informalities. Proposals must be returned on the form(s) provided, with Bid Proposal - Flooring clearly on the label. A proposal must consist of all pages of the proposal request including the signature page of the proposal, signed by an authorized representative of the firm. Non conformance with these instructions may be grounds for rejection of proposal. Faxed or e mailed proposals will not be accepted. Late proposals will be rejected, unopened and returned. 3 To: Whom it may concern Re: Invitation to bid flooring Dear Sir/Madam, Park Hill School District is seeking proposals from qualified companies to provide the materials and installation of flooring at various schools in the Park Hill School District. The Mandatory Pre- Bid walk through will be on February 29, 2016 at 9:00 a.m. We will begin at Plaza Middle School located at 6501 NW 72nd St, Kansas City MO, at 9:00 a.m. Companies interested in receiving a bid proposal packet can download the information for the district web site: Then select District Information at the top of the page then Requests for Proposals and Bids. 4 SECTION 1 DEMOLITION AND PROTECTIVE MEASURES 1. GENERAL 1.01 Scope - The work herein includes, but is not limited specifically to: furnishing all labor and equipment necessary to perform all removal of carpet, VCT and rubber bases as specified pursuant to the installation of carpet and base Methods Methods used in removing shall be in accordance with all ordinances and requirements of the local authorities. The Contractor shall be responsible for all required permits and fees. 1. Debris shall be removed from the site avoiding excessive accumulation, and more frequently when the presence of same constitutes a safety, health or fire hazard. 2. Any waste materials being moved through existing occupied areas, shall follow routes of egress coordinated with and approved by the Owner. Times of day shall be coordinated with and approved by the Owner on a daily basis. The carpet installer is responsible for the removal of carpet, VCT and base from the building and site Salvage and rubbish Salvage and rubbish shall become the property of the contractor and shall be promptly removed from the site. 2. EXECUTION 2.01 Completion of Work Existing floors, walls, ceilings, etc., defaced or damaged by construction operations shall be restored and refinished to their original conditions. 3. RECYCLED WASTE Owner specifies that all existing carpet and existing carpet waste be recycled in the best possible manner. A reclamation plan will be submitted that will provide directions for the reclamation of all carpet at the job site. Environmentally friendly carpet recycling methods should be performed subsequent to job completion. In this case, environmentally friendly methods consists of either: 3.1 Recycling is turning waste materials into new materials of the same value, such as vinyl backing into vinyl backing. 2. Upcycling involves turning waste materials into more valuable products. For example, using waste PET (plastic from soda bottles) to create Terratex panel fabric. 3. Downcycling creates less valuable products from waste materials. Turning nylon face fiber into car parts or carpet padding, including nylon face fiber in recycled backings, or using carpet for waste-to-energy are good examples of downcycling. 4. Repurposing allows a product to be reused by an organization or individual.

5 3.2 Processing: All possible recycling options must be clearly presented and/or submitted on paper subsequent to job start. The accepted reclamation option must be approved in writing by the District. 3.3 Certification: A certificate will be furnished upon request verifying the reclamation of the carpet SECTION 2 WALL BASE: SECTION 3 CARPETING Wall base materials should be top set four inch toe. Wall base material to be solid homogenous rubber only manufactured by Roppe. Vinyl or vinyl composites will not be acceptable. QUALITY ASSURANCE Installer qualifications: An experienced installer who is certified by the Floor Covering Installation Board or holds Certificate of Qualification from the carpet manufacturer. Installer to have minimum of five years experience of installation of commercial carpeting. THE REQUIREMENTS As set forth in the headings of general conditions, Supplementary General conditions and Division l. General Requirements shall apply to this branch of work. 3. GENERAL 1.01 Contractor shall install all carpeting, furnish all materials, equipment, transportation, seaming, adhesive, binding, sewing, edge strips, etc., necessary for a complete installation of carpeting specified here in, shown and scheduled on the drawings Quality Assurance - 1. General Standard: Carpet Specifier s Handbook by the Carpet and Rug Institute; for definitions of terminology not otherwise defined herein, and for general recommendations and information. 2. The Owner or Architect shall review the credentials of the installer, at least five years experience in the supervision of carpet installation and five installations of carpet similar to type specified herein Warranty 1. The contractor shall, at his expense and upon written notice, promptly and properly replace any and all improper work and material that may become apparent within two (2) years after substantial completion, to include repair of seams, puckering, and any other defects that might be directly pointed to defect in workmanship.

6 MATERIALS 2.01 Carpet shall be manufactured by Interface. Carpet will be direct-glue down on the outer edges. Tac-tiles will be used in open areas. Final Selection of colors and texture will be made by the owner. In most case we will match existing carpet where possible All exposed edges which about different floor finishes, shall be trimmed with vinyl carpet molding equal to Mercer Plastic Company s tile and carpet joiner Stock No. 15. Installation Adhesive Water-resistant, mildew resistant, non-staining, pressure-sensitive, type to suit products and sub-floor conditions indicated, that complies with flammability requirements for installed carpet tile and is recommended by carpet tile manufacturer for releasable installation.
