1 SECTION PART 1 GENERAL 1.01 THE REQUIREMENT A. Payment for various items of the Bid Schedule, as further specified herein, shall include all compensation to be received by the CONTRACTOR for furnishing all tools, equipment, supplies, and manufactured articles, taxes, commissions, transportation and handling, bonds, insurance, overhead and profit and for all labor operations and incidentals appurtenant to the items of work being described, as necessary to complete the various items of the WORK all in accordance with requirements of the Contract Documents, including all appurtenances thereto, and including all costs of permits and cost of compliance with the regulations of public agencies having jurisdiction, including Safety and Health Requirements of the Occupational Safety and Health Administration of the U.S. Department of Labor (OSHA). No separate payment will be made for any item that is not specifically set forth in the Schedule of Prices Bid. Hence, all costs therefore shall be included in the prices named in the Schedules of Prices Bid for the various appurtenance items of the WORK. B. Payment for the various items of the Schedule of Prices Bid shall constitute full compensation for all professional and construction services to be provided by the CONTRACTOR as noted in the specifications, conceptual drawings, and any other components of this Design Criteria Package. C. No separate payment for WORK OR MATERIALS will be made unless specifically stated on the plans or directed by the CITY. All bid items include, but are not limited to the following tasks: Mobilization Maintenance of Traffic Furnishing and Installation of all Proposed/Requisite Storm Water System Components, as applicable Furnishing and Installation of all Proposed/Requisite Water System Components Furnishing and Installation of all Proposed/Requisite Hardscape Improvements Furnishing and Installation of all Proposed/ Requisite Landscape and Irrigation Improvements. Furnishing and Installation of all Electrical Improvements. D. The CONTRACTOR s attention is called to the fact that the quotations provided for the various items on the Schedule of Prices Bid are intended to establish a GRAND TOTAL BASE BID AMOUNT for completing the WORK in its entirety. Should the CONTRACTOR feel that the cost for any item of WORK has not been established by the Schedule of Prices Bid in this Section, it shall include the cost for that Work in some other applicable bid item, so that its proposal for the project does reflect its total price for completing the WORK in its entirety SCHEDULE OF PRICES BID NEIGHBORHOOD NO. 8: LOWER NORTH

2 A. GENERAL 1. This is a Unit Price contract. Quantities of work under this contract are estimated and are not guaranteed as the quantities required to complete the project. There will be no change in unit prices as a result of an increase or decrease in the quantity of work performed under any contract item. 2. Units of measure shown on Bid Form shall be as follows, unless specified otherwise. All methods of measurement shall be approved by the CONSULTANT. PAYMENT GENERAL 1. This is a Unit Price contract. Quantities of work under this contract are estimated and are not guaranteed as the quantities required to complete the project. There will be no change in unit prices as a result of an increase or decrease in the quantity of work performed under any contract item. 2. Units of measure shown on Schedule of Prices Bid shall be as follows, unless specified otherwise. All methods of measurement shall be approved by the CONSULTANT. 3. Measurement and payment for each item in the Schedule of Prices Bid shall be as described below: NEIGHBORHOOD NO. 8: LOWER NORTH

GENERAL CONDITIONS ITEM 1.01) Bonds and Insurance 1.02) Consideration for indemnification 1.03) Allowance for Permit Fees DESCRIPTION Payments for Bonds and Insurance shall be at the lump sum price named in the Bid Schedule. This price shall be full compensation for the CONTRACTOR to provide all bonds and insurance as required in the Contract Documents. Payment will be made in full on the first monthly pay application providing the CONTRACTOR has provided adequate documentation demonstrating that the required items have been secured a Bonds and Insurance LS Payment for consideration for indemnification of the CITY will be based upon the lump sum price named for such work, in accordance with the requirements of the Contract Documents. Payment will be twenty-five dollars for consideration for indemnification named in the bid schedule and shall constitute full compensation for indemnifying the CITY as specified in the Contract Documents a Indemnification LS Payment for permit fees will be based upon the actual permit fees required by the CONTRACTOR from the various agencies having jurisdiction for construction of the project, in accordance with the Contract Documents. The allowance amount shown on the bid schedule is an estimate of permit fees required for the project and is a cost pass through item and no mark-ups will be added to this item. The CONTRACTOR shall submit documentation with pay request verifying actual cost. Only permit fees substantiated by the CONTRACTOR and approved by the CONSULTANT will be paid as part of this bid item. Any balance in this item at the end of the project shall be credited back to the OWNER a Permit Allowance AL NEIGHBORHOOD NO. 8: LOWER NORTH

4 ITEM 1.04) Mobilization / demobilization 1.05) Survey Layout and As-Builts 1.06) Maintenance of Traffic DESCRIPTION Payment for Mobilization /Demobilization shall be made at the lump sum price named in the Bid Schedule. Mob/Demob includes, but is not limited to: mobilization to, and removal of, all labor, equipment, and materials to the site, site cleanup and maintenance, sanitary facilities, labor associated with permit acquisition, contractors staging area, contractors field office, license for E-Builder software project signs and project coordination,. Partial payments for Mob/Demob will be made as follows: 25% at the beginning of the work 50% at 10% complete 75% at 25% complete 100% at 100% complete 1.04.a Mobilization / demobilization LS Payment for survey layout and as-builts will be made at the lump sum price named in the Bid Schedule. This shall provide full payment for the CONTRACTOR to provide field layout and surveyed documentation of the constructed work as required by the Contract Documents. Partial payments for survey layout and as-builts will be made as follows: 25% at the beginning of the work 50% at 10% complete 75% at 25% complete 100% upon submittal of approved as-builts 1.05.a Survey Layout and As-Builts LS Payment for maintenance of traffic will be made at the lump sum price named in the Bid Schedule. Payment for maintenance of traffic will be made in equal monthly amounts for the project duration stated in the Contract. The lump sum price for this item shall include full compensation for all maintenance of traffic, including but not limited to all labor, equipment, and material required to keep roadways and property accesses in service during construction NEIGHBORHOOD NO. 8: LOWER NORTH

5 ITEM 1.07) Engineer s Field Office 1.08) Public Information / Liaison Services DESCRIPTION activities as specified in the Contract Documents and as required by FDOT, County, City, or local authorities. The CONTRACTOR s lump sum price shall include full compensation for preparation of maintenance of traffic plans (MOTs), all personnel required to direct and maintain traffic (including local police as required), and all signs, cones, and temporary lights. Additionally, this item shall include all costs associated with reduced or changed hours due to construction in school zones and all costs associated with the coordination of all other work underway at the same time (city, county, state) within the project limits a Maintenance of Traffic LS Payment for Engineer s field office installation (complete), operation, maintenance and removal, including all associated items and equipment as described in the Contract Documents, will be at the lump sum price named in the Bid Schedule. Payment under this item will be made on a monthly basis in accordance with the approved cost loaded schedule based on the work completed or installed. No request for additional reimbursement for items relating to the field office requirements will be approved a Engineer s Field Office LS Payments for Public Information / Liaison services shall be at the lump sum price named in the Bid Schedule. This price shall be full compensation for the CONTRACTOR to provide all public information and liaison services as required in the Contract Documents. WATER AND SEWER ITEM 2.01) Furnish and Install Restrained Joint DIP Water Main 2.02) Furnish and Install Restrained Joint Ductile Iron (DI) Fittings DESCRIPTION Measurement for payment of furnishing and installing restrained joint ductile iron pipe (DIP) water main pipe will be based upon the number of linear feet of pipe actually constructed as determined by measurement along the centerline of the pipe in-place in accordance with the requirements of the Contract Documents. Pipe cost shall include all specified linings and coatings. Depth of installation is as indicated on the drawing details and in the specifications and all excavation is unclassified. Payment for furnishing and installing water main pipe will be made at the unit price, per linear foot of pipe, named in the Bid Schedule and includes but is not limited to transporting, storing, furnishing, and installing the pipe, excavation, dewatering, pipe bedding, backfilling, compaction, cleaning, testing, and all other specified work. Partial payment shall be made as follows: - 75% of the unit price for pipeline installed, backfilled, and compacted to grade; - 90% of the unit price following completion of leakage or pressure testing; - 100% of the unit price following installation of the temporary pavement a 12 restrained joint DIP water main LF 2.01.b 10 restrained joint DIP water main LF 2.01.c 2.01.d 2.01.e 8 restrained joint DIP water main LF 6 restrained joint DIP water main LF 4 restrained joint DIP water main LF Measurement for payment to furnish and install DI fittings shall be at the unit price bid per pound for such fittings furnished in accordance with the Contract Documents. Payment for furnishing and installing valves will be made at the unit price, for each, named in the Bid Schedule, and shall constitute full compensation for the complete installation of the valve, including valve box, extension, restraint, concrete collar (if required), transporting, storing, furnishing, and installing the valve, excavation, dewatering, backfilling, compaction, cleaning, testing, and all other specified work for a complete installation a 12 Gate Valve w/box 2.03.b 8 Gate Valve w/box 2.03.c 6 Gate Valve w/box 2.03.d 4 Gate Valve w/box Measurement for payment to furnish and install tapping sleeve and valve will be based upon actual quantity, each, of tapping sleeves and valves furnished, installed and tested, in accordance with the requirements of the Contract Documents. Payment for furnishing and installing tapping sleeves and valves will be made at the unit price, for each, named in the Bid Schedule, and shall constitute full compensation for the complete installation of the tapping sleeve, valve and valve box including valve box, extension, restraint and concrete collar, if required, transporting, storing, NEIGHBORHOOD NO. 8: LOWER NORTH

8 ITEM 2.05) Install Cut-In Tee on Existing Water Main 2.06) Furnish and Install Air Release Valve with Box 2.07) Furnish and Install Fire Hydrant Assembly with New Hydrant DESCRIPTION furnishing, and installing the valve, excavation, dewatering, backfilling, compaction, cleaning, testing, and all other specified work. Bid Items: 2.04.a 20 x 8 Tapping Sleeve & Valve 2.04.b 12 x 10 Tapping Sleeve & Valve 2.04.c 8 x 6 Tapping Sleeve & Valve Measurement for payment to cut-in tee on existing water main will be based upon actual quantity, each, of cut-in tees installed and tested, in accordance with the requirements of the Contract Documents. Payment for installing cut-in tees will be made at the unit price, for each, named in the Bid Schedule, and shall constitute full compensation for the complete installation of the cut-in tee including excavation, dewatering, restraint, backfilling, compaction, cleaning, testing, and all other specified work. Payment for the tee fitting will be paid for separately under item 2.02a a Cut-In Tee On Existing 4 thru 8 Water Main Measurement for payment to furnish and install air release valves with box will be based upon the actual number, of each air release valves installed in accordance with the Contract Documents. Payment for furnishing and installing air release valves will be made at the unit price for each named in the Bid Schedule, and shall constitute full compensation for the construction of air release valve, complete including valve, pipe, fittings, corp. stop, valve box, concrete collar, excavation, dewatering, backfilling, compaction, cleaning, testing, and all other specified work a Air Release Valve w/box Measurement for payment to furnish and install fire hydrant assemblies with new hydrant shall be at the unit price bid per each fire hydrant assembly furnished in accordance with the Contract Documents NEIGHBORHOOD NO. 8: LOWER NORTH

9 ITEM 2.08) Furnish and Install Fire Hydrant Assembly with Salvaged Hydrant 2.09) Furnish and Install Water Service DESCRIPTION Payment for furnishing and installing fire hydrant assemblies with new hydrant shall be at the unit price bid per each and shall include but is not limited to transporting, storing, furnishing, and installing. Fire hydrant assemblies shall be complete, including fire hydrant, fire hydrant extensions, lead piping, valves and valve boxes, concrete collars, fittings, testing, and all other specified work a Furnish and Install Fire Hydrant Assembly w/new Hydrant Measurement for payment to furnish and install fire hydrant assemblies with salvaged hydrant shall be at the unit price bid per each fire hydrant assembly furnished in accordance with the Contract Documents. Payment for furnishing and installing fire hydrant assemblies with salvaged hydrant shall be at the unit price bid per each and shall include but is not limited to transporting, storing, furnishing, and installing. This item shall including removing and reinstalling (salvaging) existing fire hydrants from the locations shown on the plans or as directed by the Resident Project Representative. The salvaged part of the existing fire hydrant assembly shall include the actual hydrant, all other portions of the assembly including fittings, piping, valves and restraints shall be new. The open end of the existing hydrant lead left after removing the existing hydrant shall be plugged or capped with an MJ cap or plug. Fire hydrant assemblies with salvaged hydrants shall be complete, including fire hydrant (salvaged), fire hydrant extensions, lead piping, valves and valve boxes, concrete collars, fittings, excavation, dewatering, backfilling, compaction, cleaning, testing, and all other specified work a Furnish and Install Fire Hydrant Assembly w/salvaged Hydrant Measurement for payment for furnishing and installing water service will be based upon the actual number of each water service installed, in accordance with the requirements of the Contract NEIGHBORHOOD NO. 8: LOWER NORTH

10 ITEM 2.10.a) Furnish and Install New Meter Box 2.11) Furnish and Install Sample Point Documents. DESCRIPTION Payment for furnishing and installing water services will be made at the unit price, for each item, named in the Bid Schedule, and shall constitute full compensation for the complete installation of the water service from the water main to the meter box at the property line. Water services shall include all fittings and polyethylene tubing from the main to the upstream side of the meter including connection to the meter, tracer wire, valves, corporation stop, saddle strap, meter stop, excavation, dewatering, backfilling, compaction, cleaning, testing, and all other fittings or items as required by the Contract Documents to provide a complete water service connection a 2 Water Service Measurement for payment for furnishing and installing meter boxes will be based upon the actual number of each meter box installed, in accordance with the requirements of the Contract Documents. Payment for furnishing and installing meter boxes will be made at the unit price, for each, named in the Bid Schedule, and shall constitute full compensation for the complete installation of the meter box, and removal and disposal of existing meter box when applicable. This bid item is for existing or new services requiring a new meter box a New Water Meter Box Measurement for payment for sample points will be based upon the actual number, for each sample point furnished and installed, in accordance with the requirements of the Contract Documents. Payment for sample points will be made at the unit price, for each, named in the Bid Schedule, and shall constitute full compensation for the complete installation of sample point and removal NEIGHBORHOOD NO. 8: LOWER NORTH

11 ITEM 2.12) Abandon and Cap Existing Water Main 2.13) Remove and Dispose of Existing Valve Box 2.14) Remove and Dispose of Existing Fire Hydrant DESCRIPTION and disposal after sampling is complete a Sample Point Installed on Hydrant 2.11.b Sample Point, Freestanding Measurement for payment of abandonment and capping of existing water mains will be based upon the unit price and number of each main capped and abandoned, in accordance with the requirements of the Contract Documents. Measurement for payment of removing and disposing of existing valve boxes will be based upon the unit price and number of each valve box removed in accordance with the requirements of the Contract Documents. Payment for removing and disposing of existing valve boxes will be made at the unit price, named in the Bid Schedule, and shall constitute full compensation for furnishing all labor, equipment and materials required to complete this item including but not limited to excavation, disposal, backfilling, compaction a Remove and Dispose of Existing Valve Box Measurement for payment of removing and disposing of existing fire hydrants will be based upon the unit price and number of each fire hydrant removed in accordance with the requirements of the Contract Documents. 12 ITEM 2.15) Sanitary Sewer Main Replacement DESCRIPTION fire hydrants will be made at the unit price, named in the Bid Schedule, and shall constitute full compensation for furnishing all labor, equipment and materials required to complete this item including but not limited to excavation, disposal, backfilling, compaction a Remove and Dispose of Existing Fire Hydrant Measurement for payment of sanitary sewer main replacement will be based upon the number of linear feet of pipe actually constructed as determined by measurement along the centerline of the pipe in-place in accordance with the requirements of the Contract Documents. Payment for sanitary sewer main replacement will be made at the unit price, named in the Bid Schedule, and shall constitute full compensation for furnishing all labor, equipment and materials required to complete this item including but not limited to excavation, disposal, backfilling, compaction, removing and disposing of existing pipe, epoxy lined DIP, and all fittings necessary for a complete installation a 8 DIP Sanitary Sewer Main Replacement LF NEIGHBORHOOD NO. 8: LOWER NORTH

Storm Water ITEM 3.01) Furnish and Install Storm Drainage Structure 3.02.a) Furnish and Install RCP Drainage Pipe DESCRIPTION Measurement for payment to furnish and install storm drainage structures will be based upon the actual quantity of each structure constructed in accordance with the requirements of the Contract Documents. Payment for furnishing and installing storm drainage structures shall be made at the unit price, for each named in the Bid Schedule, and shall constitute full compensation for the complete installation of the structure including but not limited to excavation, shoring, dewatering, specified base material, backfill, compaction, precast or cast in place structure, adjustments to grade, joint seals, frame with cover or grate, and for all other items necessary for a complete installation. Structures with inlet flumes shall also include construction of a concrete inlet flume as detailed in the contract drawings. Structures with concrete aprons shall also include construction of a concrete apron as detailed in the contract drawings a Type C Inlet w/slot and Inlet Flume 3.01.b Type D Inlet, 4 dia. w/storm Manhole Top 3.01.c Type D Inlet, 4 dia. w/valley Gutter Top 3.01.d Type E Inlet w/slot and Inlet Flume 3.01.e Type E Inlet w/concrete Apron 3.01.f Type D Inlet, w/valley Gutter Top Measurement for payment of furnishing and installing RCP drainage pipe will be based upon the number of linear feet of pipe actually constructed as measured along the centerline of the pipe in-place from outside face to outside face of the drainage structures at each end of the pipe. 14 ITEM 3.03) Connect New Drainage Pipe to Existing Structure 3.04) Furnish and Install Outfall Manhole DESCRIPTION Payment for furnishing and installing RCP drainage pipe will be made at the unit price, per linear foot of pipe, named in the Bid Schedule and includes but is not limited to transporting, storing, furnishing, and installing the pipe, excavation, dewatering, pipe bedding, backfilling, compaction, cleaning, testing, and all other labor, equipment and materials necessary to construct the specified work. Partial payment shall be made as follows: - 75% of the unit price for pipeline installed, backfilled, and compacted to grade; - 90% of the unit price following completion of leakage or pressure testing; - 100% of the unit price following installation of the temporary pavement a 18 dia. RCP Drainage Pipe 3.02.b 24 dia. RCP Drainage Pipe LF LF Measurement for payment to connect new drainage pipe to existing structure will be based upon actual quantity, each, of new connections made, in accordance with the requirements of the Contract Documents. Payment for connecting new drainage pipe to existing structures will be made at the unit price, for each, named in the Bid Schedule, and shall constitute full compensation for the complete connection of the new pipe to the existing structure including excavation, dewatering, core drilling the existing structure, providing a watertight seal using non-shrink grout between the existing structure and the new pipe, backfilling, compaction, and all other specified work a Connect New Drainage Pipe to Existing Drainage Structure Measurement for payment to furnish and install storm water outfall manholes will be based upon the actual quantity of each structure constructed in accordance with the requirements of the Contract Documents NEIGHBORHOOD NO. 8: LOWER NORTH

15 ITEM 3.05) Furnish and Install Manatee Grate 3.06) Furnish and Install Passive Relief Manhole DESCRIPTION Payment for furnishing and installing storm water outfall manholes shall be made at the unit price, for each named in the Bid Schedule, and shall constitute full compensation for the complete installation of the outfall manhole including but not limited to excavation, removing and disposing of existing storm pipe as required to install manhole structure, specified base material, dewatering, backfill, compaction, precast or cast in place structure, adjustments to grade, joint seals, frame with cover or grate, spool pieces, check valve and for all other items necessary for a complete installation a Outfall Manhole, 5 dia. w/tideflex Valve Measurement for payment for furnishing and installing manatee grates will be based upon the actual number of each grate installed, in accordance with the requirements of the Contract Documents. Payment for furnishing and installing manatee grates will be made at the unit price, for each, named in the Bid Schedule, and shall constitute full compensation for the complete installation of manatee grates as shown on the plans and detailed in the specifications a Manatee Grate, Aluminum, for 24 Outlet Measurement for payment for furnishing and installing passive relief manholes will be based upon the actual number of each manhole installed, in accordance with the requirements of the Contract Documents. Payment for furnishing and installing passive relief manholes shall be made at the unit price, for each named in the Bid Schedule, and shall constitute full compensation for the complete installation of the passive relief manhole including but not limited to excavation, specified base material, dewatering, backfill, compaction, precast structure, riser pipe, fittings, pipe supports, adjustments to grade, joint seals, frame with cover or grate, and for all other items necessary for a complete installation NEIGHBORHOOD NO. 8: LOWER NORTH

16 ITEM 3.07) Outfall Pipe Cleaning 3.08) Furnish and Install Storm Water Detention Box 3.09) Furnish and Install Storm Water Pumping Station DESCRIPTION 3.06.a Passive Relief Manhole, 4 dia. Measurement for payment of outfall pipe cleaning will be based upon the number of linear feet of pipe actually cleaned as measured along the centerline of the pipe in-place from outside face to outside face of the drainage structures at each end of the pipe. Payment for performing outfall pipe cleaning will be made at the unit price, per linear foot of pipe, named in the Bid Schedule and includes but is not limited to inspecting; installing, maintaining, and removing turbidity control; cleaning pipe; disposing of debris and all other items necessary to complete this item a Outfall Pipe Cleaning, 8 thru 24 dia. Measurement for payment for furnishing and installing storm water detention boxes will be based upon the actual number of each box installed, in accordance with the requirements of the Contract Documents. Payment for furnishing and installing storm water detention boxes shall be made at the unit price, for each named in the Bid Schedule, and shall constitute full compensation for the complete installation of the storm water detention box including but not limited to excavation, specified base material, dewatering, backfill, compaction, precast or cast-in-place structure, adjustments to grade, joint seals, frames with covers or grates, pipe sleeves, corrugated aluminum baffle, FRP grating, and for all other items necessary for a complete installation a Storm Water Detention Box Measurement for payment for furnishing and installing storm water pumping stations will be based upon the actual number of each station installed, in accordance with the requirements of the Contract Documents. Payment for furnishing and installing storm water pumping stations shall be made at the unit price, for each named in the Bid Schedule, and shall NEIGHBORHOOD NO. 8: LOWER NORTH
