1 1.0 Purpose: The purpose of the contract is to provide a Construction Project Engineer, Senior Inspector Roadway, and two certified G&B inspectors, as needed, to assist State staff on S.P during the 2018 construction seasons. S.P is a 14.2 mile bituminous mill and overlay on T.H. 210 from the West Junction of T.H. 65 at McGregor to East Junction of T.H. 169 at Hassman s Corner. (R.P ) 2.0 Construction Project Engineer (Source Type 1800) Construction Project Engineer tasks includes all services necessary to assure that proper coordination and management of the construction inspection, surveying, materials sampling and testing and contract administration activities are coordinated with all parties involved in accomplishing completion of a State construction project. Under the direction of the State, Contractor will: 2.1 Evaluate construction contractor s submittals of its Critical Path Method (CPM) schedules, baseline and monthly, for contract compliance and necessary requirements. 2.2 Ensure that construction contractor performs all work in accordance with the contract including scheduling and reviewing all required materials testing. 2.3 Prepare agendas and facilitate pre-construction meeting, utility meetings and weekly meetings as needed. 2.4 Monitor project progress to ensure on-time completion of construction project. Manage delay issues in the best interest of the State. 2.5 Provide construction project guidance to construction field staff as needed (interpretation of plans, specifications and contract provisions of a project and resolution of field problems. Contractor is expected to consult with State prior to implementing solutions to issues. 2.6 Visit construction project as needed for issue resolution, weekly meetings, etc. (minimum three times a week.) 2.7 Problem solve - address unforeseen issues that may arise that are not addressed in the plan (field problems such as plan omissions, out of tolerance work, out of specification materials, structural defects, accidental damage, underground obstructions, etc.); develop solutions, work with design authority to implement solution (i.e. revised plan sheets as needed) negotiate costs with the construction contractor, write appropriate change order, etc. Consult with State s District Resident Engineer for approval of solution, costs and acquire State s District Resident Engineer s signature on all Change Orders. 2.8 Using the American Association of State Highway and Transportation Officials (AASHTO) project software, AASHTOWare: Prepare and review daily entries; review vouchers for accuracy; notify State s Resident Engineer that voucher is ready for review/approval; negotiate, submit, process, enter Change Orders; enter stockpile items; generate reports. 2.9 Assist State in preparing for dispute resolution or litigation regarding the Project Prepare and submit project final documents. Deliverables All documents and form required by MnDOT Office of Construction to successfully administer and close out this type of project. Daily diaries, daily work reports, logs and records of all activities and events relating to the Project. Records of Work completed by the Contractor Report of review of construction contractor CPM schedule submittals 3.0 Construction Inspector (Source Type 1800) Construction Inspection tasks include all services necessary to ensure that the project is completed within reasonable conformity of the plans, specifications and contract provisions. 1

2 Contractor will perform technical duties in inspecting work to assure compliance with State s Contract Administration Manual, Specification Book, special provisions, construction plan, policies and procedures, standards, Occupational Safety and Health Administration (OSHA) regulations, and Federal Highway Administration (FHWA) guidelines, as well as any other applicable laws. The Contractor will provide on-site construction inspection on a full time basis during all necessary construction activities. The Contractor will provide sufficient quality and quantity of inspection services, as directed by the State, and the individual project Work Assignment, to ensure that all construction is in accordance with approved plans, specifications, special provisions, and that compliance with State construction standards and policies is maintained. Contractor will: 3.1 Construction Inspector Senior Attend a meeting with the State prior to the preconstruction conference. This meeting will provide a review of the construction contract and the requirements of the project along with information necessary to properly measure and pay for the construction contract work Attend and participate at the preconstruction conference for the project Maintain on a daily basis a complete and accurate record of all activities and events relating to the project and a record of all work completed by the construction contractor, including quantities of pay items in conformity with final estimates preparation procedures, and specifications Maintain a Contract Time and Project Diary for the entire Project Maintain a log of all materials entering into the work with proper indication of the basis of acceptance of each shipment of material. Ensure Certificates of Compliance associated with the materials, Buy America (where required) and ensure that specific items are on the State s Approved Product List. Note that Contractor will not allow any material to be incorporated into the project without meeting these requirements Maintain records of all sampling and testing accomplished and analyze such records required to ascertain acceptability of materials and completed work items. The field reports for records of work and testing results will be submitted within one week At least once each month, prepare a Monthly Pay Voucher using AASHTOWARE. Quantities will be based on daily records or calculations. Calculations will be retained Provide to the construction contractor interpretations of the plans, specifications and contract provisions. Contractor will consult with the State when an interpretation involves complex issues or may have an impact on the cost of performing the work Analyze changes to the plans, specifications or contract provisions and extra work which appear to be necessary to carry out the intent of the Contract. When it is determined that a change or extra work is necessary, the contractor will inform the State In the event that the construction contractor gives notice, either written or verbal, that it deems certain work to be performed is beyond the scope of the construction contract and it intends to claim additional compensation, the Contractor provided Senior Inspector will maintain accurate records of the costs involved in such work. These records will include manpower and equipment times and materials installed (temporary or permanent) in the portion of the work in dispute In the event that the construction contractor for the project submits a claim for additional compensation, the Senior Inspector will analyze the submittal and prepare a recommendation to the State covering validity and reasonableness of charges. Maintain complete and accurate records of work involved in claims The Contractor Senior Inspector will prepare and submit to the State a preliminary Grading and Base Report prior to Construction Contractor construction operations; a Final Grading and Base Report within 30 calendar days after construction project substantial completion or as requested by the Construction Project Manager; an up-to-date Materials Exception Summary; all original Field Record Documentation forms within 30 calendar days after 2

3 construction project substantial completion; all records, material certifications, and other items used in acceptance of the project construction and materials, submitted within 30 calendar days after construction project substantial completion Monitor the construction project to the extent necessary to determine whether construction activities violate the requirements of any permits. Notify the construction contractor of any violations or potential violations and require its immediate resolution of the problem. Violations must be reported to the State immediately As required by the State, the Contractor Senior Inspector will provide coordination between the construction contractor and utility companies to assure that conflicting utilities are removed, adjusted or protected in-place in a timely manner to minimize delays to construction operations. Documentation will be maintained in accordance with State s policies As required by the State, provide inspection of utility work including reimbursable utilities that are shown in the contract. This will also include all required documentation To prevent delays in construction contractor s operations, promptly produce reports, verify quantity calculations, field measure for payment purposes and/or write communications Videotape the pre-construction conditions throughout the project limits. Provide a digital photo log of project activities, with heavy emphasis on potential claim items/issues and areas of potential public controversy Videotape all roadways designated by the State, as Temporary Trunk Highways for haul roads Other activities as deemed necessary by the State. 3.2 Construction Inspector Grading and Base (Anticipate 2 required) Complete a diary on a daily basis Report discrepancies to the State as soon as they are realized Inspect various components of the project construction operations as assigned by the State, including drainage facilities installation; excavation and embankment; base material placement; concrete pavement placement; concrete curb and gutter placement; concrete sidewalk placement; bituminous paving; traffic control; signing; traffic markings; erosion control; turf establishment Run Dynamic Cone Penetrometer (DCP) for compaction testing Measure items for payment Other activities as deemed necessary by the State. 3.3 Deliverables and Deliverable Due Dates Contractor will provide the following to the State in accordance with schedule indicated: Deliverables will consist of scanned reports and scanned lab worksheets ed to both the project-assigned Senior Inspector and project-assigned Engineer/Supervisor on a daily basis. Original paper copies of all lab worksheets to be delivered to the State on a weekly basis. All paperwork, reports, worksheets, and diaries shall be legible. Entries determined to be in error and corrective action is needed, must not be obliterated by erasing, application of correction fluid, taped over, or in the case of computer-generated documents, deleted. A line is to be cleanly drawn through the mistaken entry and corrections entered directly above with the initials of the person making the change. The Contractor will be responsible for the Subcontractor s legibility on their paperwork All services will be performed in accordance with the established standard procedures and practices of the State. Prior to furnishing any services, the Contractor will be familiar with State standard procedures and practices which will be provided and with informal procedures and practices for construction and engineering used by the State. 3

4 3.3.4 During the time that the Senior and other Inspectors are under the direction of State, the Contractor will comply with State work policies which will be provided. These include policies on appropriate use of technology, no-smoking policy and safety issues. 3.4 State Furnished Items: The State will furnish: Access to all testing and inspection forms necessary to carry out assigned duties prior to project start up Access to a non-secured internet connection in the field office via an Ethernet connection when available One set of Project Plans and Proposal in either paper or electronic format One copy of all Work Orders, Change Orders and Supplemental Agreements from the project that will affect the work being performed by the Contractor Access to the State s field office/lab, which includes all field lab equipment and tools necessary to perform tests in a lab. 3.5 Contractor Furnished Items: The Contractor will provide: A clearly identifiable vehicle for the field personnel assigned to a project. The vehicles will have the necessary warning lights to be within the work zone and be able to safely carry concrete cylinders, concrete beams, bituminous samples and cores, aggregate samples or any other equipment or materials Personal protective equipment: Vest meeting American National Standards Institute (ANSI); Hard hat; High visibility soft hat/cap; Steel toed boots; Safety glasses with side shields A cellular phone for the personnel assigned to a project. This phone number will be available to the State s Project Inspection Team, the State, the Construction Contractor and other project related contacts. Must be capable of taking and storing photos with date stamping capabilities Necessary writing and marking items such as paper, pens, pencils, markers and marking sticks A digital camera, at least 6 mega pixel capable of date stamping the photos. A cellular phone that has a camera of at least 6 mega pixel s would be acceptable A laptop computer with a Microsoft Excel 2007 or newer, and ability to send and receive when connected to the internet. The Contractor s field personnel will have a working account All street testing equipment and tools necessary to perform field testing, other than that provided for in 6.5 of the and as provided for in the Construction Contract for the specific project field laboratory. 4.0 Construction on Surveying (Source Code 1800) 4.1 Be responsible to review, balance, adjust, correct, and investigate State provided data and to perform work on survey data and control points that may be necessary to use the survey points and data. 4.2 Start and end all level runs, traverses, or GPS control surveys, from known control. Complete all control surveys at no worse than the standards specified for supplemental control in Chapter 2, MnDOT Surveying and Mapping Manual. 4.3 Unless otherwise agreed to, set all stakes and marks in accordance with the Staking Information Sheets included in the Plan. 4.4 Furnish and install traffic control devices in accordance with the Field Manual for Temporary Traffic Control Zone Layouts, Part VI, (MN MUTCD), when crew members are exposed to traffic. 4

5 4.5 Perform all Construction Surveying for all Project construction as shown in State Specification 1508, and place reference points as needed for the use of any public utility crews that are staking or accomplishing utility relocation or construction associated with this project: From Horizontal and Vertical Control Points established by the State According to the Plan, Proposal and Standard Specifications for S.P According to the MnDOT Surveying and Mapping manual According to actual existing field conditions. 4.6 Document surveying during construction in a form acceptable to the State s Project Manager and allow State Personnel access to surveying notes and calculations. The survey documentation will include: Control station monumentation with reference ties Field notes that were used to set construction stakes, control the Project, and document monument locations. The Contractor will use bound, hard cover field books for recording survey data and field notes or store field notes on an electronic medium or use both methods. If an electronic medium is used, the raw field data files must be available. When using an electronic medium, the Contractor will make all files and data available in the Standard formats used by the Department. 4.7 Provide the State s Project Manager with the record Survey Data. The record Survey Data must include following: Changes from the Plan Alignment Profile Utilities Identify any alignment, Right-of-Way, property, or control monumentation destroyed during the Project Any alignment, Right-of-Way, property, or control monumentation that was placed during the Project and that still exists at Project completion The information will include the x, y and, if applicable, the z coordinates in the Project datum. If the original item had no coordinate reference, then show the revised centerline station and offset. The information must be provided in both electronic (Microstation and GeoPak) and hard copy format. In the case of new monumentation, there should also be a report describing how the monumentation was placed. This will include copies of any fieldwork (traverse or leveling) as well as any adjustments used. It will also include tie sheets, to include a description of the physical object placed as the monument. 4.8 Furnish survey documentation and record Survey Data to the State s Project Manager within the time limits indicated. 4.9 Attend a kick off meeting with the State and attend the Pre-Construction Conference in Duluth. The crew chief must attend weekly on site construction meetings during the course of the work The crew chief must be the same for the entire construction season. Use of multiple crews is acceptable only if under the direction of a primary crew chief. 5

6 4.11 Contractor Deliverables set of construction limit stakes as needed set of key hubs and slope stakes at ramp areas and as necessary set of guardrail stakes Attendance at weekly construction meetings by crew chief As-built survey information State Furnished Items A hard copy of the plan and special provisions At the beginning of a project, MnDOT will provide information on the Geodetic control stations that were established and used for the preliminary survey work in the project area. It will be the surveyor s responsibility to establish project control within the project limits. This control will consist of stations established by MnDOT s Geodetic Survey Unit (1 st and 2 nd order). Information on Geodetic control stations can be obtained from the Geodetic database via the web at It is the surveyor s responsibility to assure that the control datum is in agreement with the plan datum, both vertical and horizontal At the discretion of the State s Project Manager, spot checks may be performed upon the Contractor s surveying calculations, records, field procedures, and actual staking. If the Project Manager determines that the work is not being performed in a manner that will assure proper controls and accuracy, the State will order the Contractor to redo such work, to the standards specified in the contract, at no additional cost to MnDOT. 5.0 Personnel: 5.1 General Requirements: Before the project begins, Contractor staff will have a working knowledge of the current State Specifications book and must possess all the knowledge, skills and abilities required as necessary for performing the duties of the position they will perform. Contractor will ensure that the State s current practices, policies and procedures are met throughout the course of the project. Contractor s staff must be proficient with AASHTOWARE. State will provide AASHTOWARE training Training and training costs of personnel is the responsibility of the Contractor. No compensation for training will be made by State. 5.2 Personnel Minimum Experience Requirements: Construction Project Supervisor Minimum four years of responsible and related experience (including project management, CPM project scheduling and construction of major road projects.) The qualified personnel must have training, experience, knowledge and expertise in the appropriate areas necessary to do the projects in accordance with AASHTO, FHWA, State and all other applicable construction policies, procedures, practices and standards. Must have a proficient working knowledge with Word, Excel and AASHTOWARE. Must have experience working with MnDOT Standard Specifications for Construction and State Special Provisions. Must have previous experience working with State s various bidding processes, procedures and project requirements Senior Inspector Roadway High school graduate plus five years of experience in construction inspection with two years in bituminous roadway paving. Responsible for performing highly complex technical assignments in construction, making and checking engineering computations, inspecting construction work and conducting/coordinating materials sampling and field tests and is responsible for coordinating and managing the 6

7 inspectors. Work is performed under general supervision of the State s Project Manager. Can apply para-professional engineering skill and knowledge to difficult transportation issues. Must be able to make sound and rational decisions after considering alternatives. Has expertise in human relation skills and oral communication skills. Has expert technical knowledge to handle most complex problems. Documents or directs documentation of work performed. Minimum Required Certifications: Aggregate Production, Concrete Field II, Bituminous Street II, Grading and Base II, Site Management (Erosion Control) Grading and Base Inspectors (2 required) High school graduate plus three years of experience in roadway construction inspection with a minimum one year of bituminous paving experience. Responsible for performing assignments and assisting Senior Inspector in the performance of his duties. Receives general supervision from Senior Inspector who reviews work while in progress. To be certified in # s 1, 5, 9, and 13. Performs moderately difficult duties in a wide range of field, laboratory and office assignments. Has ability to interpret and analyze written material. Has above average oral and written communication skills. Can perform advanced engineering calculations. Has the ability to organize work and perform project planning. Minimum Required Certifications: Aggregate Production, Grading and Base I, Concrete Field I, Bituminous Street I. 5.3 Staffing: Responsible personnel, thoroughly familiar with all aspects of construction and final measurements of the various pay items, will be available to resolve disputed final pay quantities until the contract has been paid off. No personnel will be assigned to this project by the Contractor until the qualifications of each person proposed have been reviewed and approved by the State. Contractor s personnel approval request will be submitted at least two weeks prior to the date an individual is to report to work An individual previously approved by the State, whose performance is later determined by State to be unsatisfactory will be replaced by Contractor The Contractor will utilize only competent personnel who are qualified by experience and education. Contractor will submit in writing to the State the names of all personnel to be considered for assignment to the construction project, together with a detailed resume with respect to salary, education and experience qualification of each individual. Minimum qualifications for the Contractor personnel are set forth in this Contract. State may require that these minimum qualifications be altered on individual Work Assignments in instances when the unique nature of the work requires specific specialized skills Personnel identified in Contractor s technical proposal will be assigned to the construction project as proposed by the Contractor and are considered by State to be committed to performing services under this Contract. Any changes will require written approval from the State When the construction contractor s operations on the project diminish, Contractor will reduce the number of its personnel assigned to the project as directed by the State. Any adjustment of Contractor forces as recommended by the State, will be accomplished within one week after notification. 5.4 Licensing for Equipment Operation: The Contractor will be responsible for obtaining proper licenses for equipment and personnel operating equipment when licenses are required. 5.5 Subcontractor Professional Services: Upon written approval by the State and, prior to performance of work, Contractor may subcontract for engineering or specialized professional services. 7
