If you have questions, please contact the appropriate staff member who conducted the review. The project will retain a "HOLD" status until all comments are satisfied. The following comments need to be addressed as noted: Rejected Review: Engineering Stormwater Review Reviewed By: Brett Rosenblum brettrosenblum@colliergov.net Phone #: (239) Correction Comment 1: Please clarify if a SFWMD Permit modification is required. Correction Comment 2: Provide Right-of-Way permit number. Engineering Services Policy and Procedures. Correction Comment 3: Please verify whether handicap parking spaces are required. If so, please provide handicap striping detail. Correction Comment 4: Sheet 8 The minimum 25 year elevation (road crown) is 14.70, however, there appear to be proposed roadway elevations below the minimum elevation required. Please review and revise roadway grading as applicable. Correction Comment 5: Please clarify if additional lake excavation is proposed that would require an excavation permit per Code of Ordinances Chapter 22, Article IV, Section 22.

2 Correction Comment 6: Per Code of Ordinances (1a), control elevation shall be 50 feet from the right-of-way line of any existing or proposed private or public street. The control elevation of the lake is less than 50 feet from Vanderbilt Way. Revise plans or request exception to this setback requirement per this code section. Correction Comment 7: Per Code of Ordinances (1b), control elevation shall be 50 feet from abutting property lines. The control elevation of the lake is less than 50 feet from the north property line. Revise plans or request exception to this setback requirement per this code section. Correction Comment 8: Provide handicap ramp curb cut detail. FDOT Index 304 Correction Comment 9: Relocate the drainage from DS #114 to DS #116 into the road right-of-way. Correction Comment 10: Label crosswalk striping. Correction Comment 11: Please provide an engineer s certification that the receiving system has been inspected. Correction Comment 12: Depict the existing Vanderbilt Way drainage in profile to ensure there are no conflicts with proposed drainage and utilities. Correction Comment 13: Please clarify the basin #1 drainage. It appears that during the 25 year storm event the hydraulic grade line will overtop the existing inlets. Correction Comment 14: Removal / demolition of existing utilities and drainage facilities should be included in the OPC. Please provide a revised signed and sealed cost estimate and additional review fees as applicable. Correction Comment 2: Sheet 14, please provide a Vanderbilt Way cross section. Correction Comment 3: Sheet 8. On the Drainage Data Table, please check the slopes. Six of the last seven slopes appear to need correction.

3 Correction Comment 4: Please label and dimension that portion of the sidewalk / pathway on the North side of the street which is not 5' in width, and reflect in cross section. Correction Comment 5: Removal / demolition of existing utilities and drainage facilities should be included in the OPC. REV 1: Verify the statuses of the previous FDEP permits. If expired, submit water and wastewater forms for new permits. Correction Comment 2: $1, Utility Modeling and Analysis Fee (for projects zoned PUD or DRI). REV 1: Submit the required fee. (See related correction comment #3.) Correction Comment 3: CCPU Policy - Sewer hydraulics to first master pump station downstream. Please contact CCPU, Planning and Project Management Department, at REV 1: The peak design flow has increased from 43.9 GPM to 74.7 GPM. Submit a satisfactory wastewater system capacity report for the additional flow. Correction Comment 4: Additional Items that need to be addressed for Engineering Utility Review: REV 1: Address the following issues in regard to the Force main Information section of the pump station design calculations in Appendix B : a) The force main length identified below is incorrect. b) What is identified as wet well top is not the top elevation, and it is being added to the tie-in pressure head to obtain a value of 52.20, which is identified as an elevation ( NAVD ). These numbers may be intended as the elevation head, which is actually around and not 6.00, and the static head, which is reported as 54.7 in the system head curve. Please correct this information and adjust the calculations accordingly. c) Please submit verification of the tie-in pressure from Public Utilities Planning and Project Management. Contact Nathan Beals at (239) d) Given the short length of pipe and the relatively high frequency of fittings and valves, consider incorporating minor losses into the friction head calculation. Correction Comment 5: DC 2.3 Pump station wet wells shall be designed to withstand flotation forces when empty and water elevation is at the top of the structure. REV 1: The wet well design calculations indicate a slab diameter of feet with an 8 I.D.

4 and 8 wall thickness. This indicates a 13 lip rather than the standard 8 lip. Revise detail WW-7 accordingly, or revise the wet well design calculations. Correction Comment 6: DC 2.3 Pump station structures and equipment shall be protected from physical damage by a 100-year flood and shall remain fully operational and accessible during a 25-year flood. REV 1: The 100-year, 3-day storm stage is reported as FT-NAVD but the wet well top elevation is only Revise the wet well design and flotation calculations in the Engineering Report, the pump station data label on sheet 9 of the plans, and the dimension table and flotation calculations on sheet 19 of the plans. Correction Comment 7: UO h - Pump station information in Engineer s Report and on detail sheet. REV 1: The operating point identified in the Engineering Report is inconsistent with the operating point identified on the pump curve provided on sheet 19 of the construction plans. Correction Comment 8: USM Section 3 - Use the latest details and only those that apply to the project. REV 1: This pump station is a community pump station, as defined in DC 2.3. Provide the corresponding details (WW-8A/B, WW-9B, and WW-9D/E). Correction Comment 9: UO i - Note as to who owns and maintains the potable water, non-potable irrigation water, and wastewater systems. REV 1: The water main, sanitary sewer pump station, and force main will be public. Will the gravity sewer be public or private? Revise General Note 20 on sheet 2. Correction Comment 10: Additional Items that need to be addressed for Engineering Utility Review: REV 1: Sheet 2 Water & Sewer Inspection Notes 3.D. # s 4-17 are construction related notes, not general inspections. Correction Comment 11: DC 2.3 Wastewater pump stations shall be designed in accordance with FDEP regulatory requirements, Section 2 Technical Specifications, and Section 3 Utilities Detail Drawings. REV 1: Obtain Utility Deviation approval to utilize a wall in lieu of a chain link fence. Correction Comment 12: UO All County Utility Easements (CUE) shall be for the exclusive sub-surface use of the Board or other County or District approved uses. REV 1: Obtain Utility Deviation approval allowing the existing irrigation main to be located within the pump station CUE.

5 Correction Comment 13: UO c - Pump station easements shall be at least twice the depth by twice the depth of the pump station inside-of-wet well bottom or thirty (30) feet by thirty (30) feet, whichever is greater. REV 1: The limits of the pump station CUE are not clear. It appears that the CUE and/or wall may extend beyond the access easement for Pristine Drive and may encroach into the proposed handicapped curb ramp at the corner. Is the CUE proposed inside or outside the wall? Correction Comment 14: UO f - Plan and profile sheets shall indicate the horizontal and vertical locations for all potable water, non-potable irrigation water and/or wastewater additions, extensions and/or improvements, including all appurtenances, as well as other proposed and/or existing system(s) or portions(s) thereof, together with all conflicts in the same general location. REV 1: Sheet 8 In the drainage data table, correct the inverts shown for the storm pipes from DS #101 to DS #102 and from DS #102 to DS #103 per the profiles on sheets 11 and 12. Correction Comment 15: REV 1: Sheet 11 The label for exist. san. manhole #104 misidentifies the SW invert as NW. Correction Comment 16: REV 1: Sheet 11 Show the existing force main in the profile. Correction Comment 17: REV 1: Sheet 11 Show the storm line between existing drainage structures DS-5 and DS-6 in the profile. Correction Comment 18: REV 1: Sheets The proposed storm lines from DS #101 to DS #103 and DS #109 to DS #112 coincide with the existing 2 irrigation main line. Indicate that this line is to be removed and replaced in these locations or move the proposed storm lines to avoid the conflicts. Correction Comment 19: REV 1: Sheet 12 Show the storm line between existing drainage structure DS-7 and relocated drainage structure DS-8 in the profile. Correction Comment 20:

6 REV 1: Sheets Show the storm lines between existing drainage structures DS-7 and DS-2 at crossing locations in the profile. Correction Comment 21: REV 1: Sheets Show the existing 2 irrigation main line on the plan and show any crossings in the profile. Correction Comment 22: UO All County Utility Easements (CUE) shall be for the exclusive sub-surface use of the Board or other County or District approved uses. REV 1: Move the light pole next to the pump station outside of the required CUE for the gravity sewer. (See related comment.) Correction Comment 23: UO b - Minimum width of gravity wastewater easements shall be twice the depth of the bottom of the line or fifteen (15) feet, whichever is greater. REV 1: Provide a CUE at a minimum 14.5 offset for the gravity sewer entering the pump station. Correction Comment 24: DC and Water and force main setbacks (minimum 7.5 ft. setback from centerline of the pipe to residential roadways, curbs and gutters, permanent structures, or plantings not specifically allowed by the ordinance). REV 1: The water main does not maintain the minimum setback from back of the valley gutter. Obtain Utility Deviation approval of the reduced setback. Correction Comment 25: DC Satisfy the min. 18 vertical and 5 horizontal separation criteria for non-potable irrigation water pipelines 2 and larger relative to potable water mains, wastewater lines, and stormwater lines. REV 1: The existing irrigation main line is close to the existing force main just southwest of the existing pump station and runs with the existing water main from sta to sta and from sta to the dead end near sta The irrigation plans approved for SDP-2006-AR show a 2 main line. Therefore, a Utility Deviation must be approved to allow reduced separation distances. Correction Comment 26: DC Satisfy the min. 18 vertical and 5 horizontal separation criteria for wastewater pipelines relative to stormwater pipelines. REV 1: Sheet 12 Based on the inverts identified on sheet 8, the existing 24 RCP between CS-2 and the outfall structure is at inv. elev over the existing sewer. This will provide less than 18 vertical clearance. Show the 24 RCP at the correct invert elevation and obtain Utility Deviation approval to reduce the minimum vertical separation distance.

7 Correction Comment 27: DC Valves shall be provided at all intersections and branches in sufficient numbers as to allow for zone isolation of distribution areas in order to limit impacts of line breaks and service disruptions to customers. In-line gate valves shall be provided at 1000 max. intervals. REV 1: An isolation valve is required near the midpoint of the water main. Correction Comment 28: DC 2.3 A water main is accessible in the adjacent ROW or CUE for water service to pump station. REV 1: Provide a 4 casing across the road. Correction Comment 29: USM Section 3 - Use the latest details and only those that apply to the project. REV 1: Obtain approval of a Utility Deviation from detail WW-16 for the sewer lateral extension to Lot #7. Correction Comment 30: LDC B.4 and DC 2.3 Public utility ancillary system landscaping requirements. Surrounding fences or walls must have, at a minimum, ten-gallon shrubs, 5 feet tall at the time of planting, placed 4 feet on center along the exterior perimeter of the surrounding fence or wall. Landscaping shall NOT be located in the CUE. REV 1: Specify the container size and height for the Simpsons stopper around the wall surrounding the pump station and show the shrubs on all sides of the pump station, leaving a gap for the gate. Rejected Review: Transportation Planning Review Reviewed By: Stephen Baluch StephenBaluch@colliergov.net Phone #: (239) Correction Comment 1: Right-of-Way (Ordinance ) 7/02/15 Comment: On construction plan sheets, change the label for Pristine Dr. to private as it is not maintained by Collier County, and change Buckstone Dr. to public since it is maintained, at least in part, by Collier County. The limits of Collier County maintenance need to be confirmed with the Maintenance Division. Correction Comment 2: Right-of-Way (Ordinance ) 7/02/15 Comment: On construction plan sheets 7 and 8, add a note that a County Permit to Perform Work and/or Maintenance in Public Right-of-Way is required for work within public road ROW (Buckstone Dr. and Vanderbilt Beach Rd.) Correction Comment 3: Interconnection to adjacent parcels is required (LDC J.4) 7/02/15 Comment: On the construction plan sheets, since each proposed lot does not have an

8 access off of Vanderbilt Way, will inter parcel access easements be provided as part of the replat? Correction Comment 4: Traffic Control (LDC ) 7/02/15 Comment: Prior to approval of any Site Development Plans on the lots, it will be necessary to modify the median opening at Buckstone Dr. and Vanderbilt Beach Rd. for left-in only, since additional left-out traffic cannot be accommodated at the existing full median opening. It is recommended that the modification be included with this plan set, so as not to delay review and approval of future development applications for the various lots. Correction Comment 5: Miscellaneous Corrections 7/02/15 Comment: On construction plan sheet 8, the drainage facilities from DS #114 to DS #116 need to be contained in a drainage easement, or relocated onto the roadway ROW.
