1 Addendum #1 Contract MAINE TURNPIKE AUTHORITY ADDENDUM NO. 1 CONTRACT BRIDGE REPLACEMENT MAXWELL ROAD UNDERPASS MILE 90.0 The bid opening date is Thursday, December 8, 2016 at 11am. The following changes are made to the Proposal, Specifications and the Plans. GENERAL PROPOSAL Intentionally left blank. Proposal Sheet P-9 is deleted and replaced with P-9 (Revised 12/05/2016) attached hereto. The revision to the proposal sheet adds $ to the Unit Price of Item Truck Mounted Attenuator. SPECIFICATIONS Special Provision Section 403 Hot Mix Asphalt Pavement, Page SP-17, a new Complimentary Note K has been added to reflect the revised application rate, Tack coat shall be applied between all layers of pavement at a rate of G/SY and at 0.04 G/SY on milled surfaces. Complimentary Notes K thru N have been re-lettered. Special Provision Section 409 Bituminous Tack Coat Page SP-19, Subsection Bituminous Material - delete the second sentence and replace with, Bituminous material shall conform to the Specifications for Emulsified Asphalt RS-1, of the AASHTO Designation M-140. Special Provision Section 603 Pipe Culverts and Storm Drains Page SP-50, Subsection HDPE Pipe, the first sentence is revised so the SDR rating is 32.5 rather than PLANS Intentionally left blank. 2 Addendum #1 Contract QUESTIONS The following are questions asked and comments made at the pre-bid meeting held on November 29, 2016 or submitted to the Maine Turnpike Authority in writing. Answers to the questions are noted. Bidders shall utilize this information in preparing their bid. Comment 1: An additional item was noted at the Pre-Bid Meeting: Special Provision Utility Coordination. The contractor should review this carefully as it requires Utility Coordination including clearing and embankment construction prior to a new utility pole being set. Comment 2: An additional emphasis was made that the contractors should be aware that the new MTA Supplemental Specifications have been updated since last construction season. The new Supplementals contain the addition of Sections 401, 502, 518, 652, and 656. Question 1: Please explain again the begin and end dates, along with the maximum durations noted in the Prosecution of Work, Pre-Bid Agenda #13. Answer: There are two windows noted: Maxwell Road Closure a seven month maximum duration within a window of February 15, 2017 and November 17, 2017; and Median pier construction with long term mainline lane closure three month maximum duration within an available window of March 1, 2017 and July 1, Question 2: Special Provision 403 appears to be missing a Complimentary Note for hot rubber used for surface vertical joints. Answer: Refer to Supplemental Specifications Section Joints, the sixth paragraph describes the requirements of applying joint sealant to pavement joints. Question 3: Special Provision 403 Note E indicates that MaineDOT will verify the Job Mix. Is this true? Recent MTA projects with a no RAP requirement have caused delays in finding approved mixes. Answer: This contract allows the use of RAP and therefore will rely on MaineDOT job mix verification. Question 4: Special Provision 403 Note J calls for Tack Coat application rate of 0.04 g/sy. Can this be reduced to or even changing to read up to an application rate of 0.04 g/sy. An application rate this high on a freshly paved surface can cause poor layer adhesion. Answer: A new Complimentary Note K has been added to reflect the revised application rate of G/SY and 0.04 G/SY on milled surfaces. Complimentary Notes K thru N have been re-lettered. Question 5: Special Provision 409 specified and Emulsified Asphalt meeting an RS-1h designation. MaineDOT allows material meeting an RS-1 designation. Will the MTA allow the RS-1 material? The project requires an amount of emulsified asphalt that is less than the minimum amount typically available. Answer: RS-1 is acceptable. Page 2 of 12

3 Addendum #1 Contract Question 6: The Specifications and the Pre-Bid Agenda call for use of only Class III safety vests. Is this strictly enforced? It appears a few contractors are still wearing Class II vests. From an overall safety perspective, all Contractors should be held to one standard. Answer: Per the Specifications, including Responsibility of the Contractor, The contractor shall ensure all jobsite personnel shall wear a safety vest labeled as ANSI standard performance for Class 3 risk exposures at all times. Question 7: On sheet S-13, 41 of 54, Bearing Details, Note No. 7 states Vulcanizing elastomer to steel plates shall be done during the primary mold process. Are the steel plates the 1/8 internal reinforcement plates, sole plates, or both? Answer: Both. Question 8: Has anyone investigated the condition or obstructions in the existing 18 pipe to be slip lined? Our pipe supplier states that the DR 13.5 specified is heavy and suggests using DR 32. Please confirm that DR 13.5 is correct. Answer: MTA has inspected the condition of the existing 18 culverts to be slip lined with a high powered camera and light. The pipes have soil and roots in them. Answer: The specified dimension ratio may be reduced from 13.5 to 32.5 as long as all other Specification requirements are met. Question 9: Please supply a detail for Item Temporary Berms. Answer: Page 802(08) in the MaineDOT Standard Details show temporary berms in conjunction with temporary slope drains. Also Page 21 in the MaineDEP s Maine Erosion and Sediment Control Practices Field Guide for Contractors has a detail. Question 10: Can you please confirm that the Contractor needs to give a separate GPS rover unit to the Turnpike for the project, or is it possible for the turnpike to use ours? Answer: The Contractor shall furnish the Authority it s own rover unit as described in Supplemental Specification Contractor Provided Equipment to the Authority. Question 11: If we use the precast slab option to construct the bridge deck, we feel that the reinforcing steel item should be paid on a lump sum basis rather than a unit price. Answer: Per Subsection of the Special Provisions: If precast concrete deck panels are utilized in the construction, no deducts will be applied to Structural Concrete Roadway and End Posts, Reinforcing Steel, and Protective Shielding for the material not used due to the use of the precast panels. Tack coat shall be applied between all layers of pavement at a rate of 0.04 G/SY. K. Tack coat shall be applied between all layers of pavement at a rate of G/SY and at 0.04 G/SY on milled surfaces. L. PGAB shall conform to the provisions of Polymer Modified PGAB for HMA M. The contractor shall furnish a quality control technician equipped with an approved densometer to ensure density requirements are met. N. Hydrated Lime shall be incorporated into the mixture. O. No vehicular loads shall be permitted on newly completed pavement until adequate stability has been attained and the material has cooled sufficiently to prevent distortion or Page 6 of 12

7 loss of fines. The newly paved area may be opened to traffic after the internal temperature of the pavement has cooled to 120 F. The Resident will test the internal temperature of the pavement and shall be the sole judge as to the opening to traffic. The period of time before opening to traffic may be extended at the discretion of the Resident. The lane closure may not be removed until the internal temperature has cooled to 120 F. Page 7 of 12

8 MAINE TURNPIKE AUTHORITY Pre-Bid Conference CONTRACT Bridge Replacement Maxwell Road Underpass Mile 90.0 November 29, 2016, 10:00 AM 1) Location: The general limits of work are as shown in the contract plans. 2) General Description: The Project consists of highway and bridge work required to replace the existing one lane Maxwell Road bridge over the Maine Turnpike in the Town of Sabattus, Maine. The work includes clearing, grading, drainage, base, pavement, guardrail, structural concrete, concrete deck, fabrication and erection of steel beams, steel bridge rails, maintenance of traffic and all other work incidental thereto in accordance with the Plans and Specifications. 3) Bid Questions: a) All bid and contractual questions shall be directed to Nate Carll, Purchasing Manager at Phone No. (207) b) All questions on plans and specifications shall be in writing and shall faxed or ed to Nate Carll of the Maine Turnpike Authority. Fax No. (207) , 4) Notification: a) Contractor shall notify and obtain approval from the Authority prior to visiting the Project site for field inspection. The contact person is Mr. Steve Tartre at (207) , ext ) Contract Specifications a) The Specifications are divided into three parts: Part I, Supplemental Specifications, Part II, Special Provisions and Part III, Appendices. Contractor to review updated Supplemental Specifications. b) The Maine Turnpike Supplemental Specifications are additions and alternations to the 2014 Maine Department of Transportation Standard Specifications. 6) Maine Department of Labor Fair Hourly Wages (Special Provision ) 7) Responsibility for Property of Others (Special Provision ): a) Contractor responsible for responding to all damage claims within 15 days. 8) Hauling of Materials and Equipment General Requirements (Supplemental Specification ) Page 8 of 12

9 a) U-Turns at toll plazas and median openings not allowed. 9) Lead Paint: a) Contractor to acknowledge the potential presence of lead paint and associated precautions thereto. 10) Construction Surveying (Section 105.6) a) The MTA has developed a construction surveying protocol based on the project size. b) The Contractor is to provide MTA with same survey/layout (Rover) equipment being utilized by contractor for the duration of the project. 11) Permit Requirements (Special Provision ) a) The Project is being constructed under MaineDEP NRPA Permit by Rule Section 11 and USACOE Programmatic General Permit, Category 2. b) The Project is subject to the requirements of the Maine Pollutant Discharge Elimination System (MPDES) General Permit for Stormwater Discharge from Construction Activity. c) A Notice of Intent (NOI) was submitted by the Authority to the DEP for coverage under the Maine Construction General Permit (MCGP). Compliance with the erosion and sedimentation control requirements outlined in this Contract is required by the Contractor. d) Limit of Disturbance Plan shall be submitted prior to any disturbance. e) Compliance with the erosion and sedimentation control requirements outlined in this Contract is required by the Contractor. f) The Project will be performed in accordance with the MaineDOT Best Management Practices (BMP) latest issue. 12) Traffic Control (Special Provision Section 652): a) All jobsite personnel shall wear a safety vest labeled as ANSI standard performance for Class 3 risk exposures. This requirement also applies to truck drivers and equipment operators when out of an enclosed cab. b) All vehicles used on the Project shall be equipped with amber flashing beacons in accordance with the Special Provision The requirements for flashing beacons has been revised and should be reviewed. c) Bridge will be closed to through traffic and a detour shall be established and maintained at all times in accordance with the details shown on the Plans. d) Erection or removal of structural steel is allowed nightly from Sunday night to Friday morning between the hours of 10:00 p.m. and 5:00 a.m. with traffic stoppages. e) Fee of $2,500 per five-minute period, or portions thereof, for each roadway (northbound and southbound) if the complete stoppage of traffic exceeds 25 minutes. f) Lane closures shall be removed if work requiring the lane closure is not ongoing unless included in the Contract as a long term traffic control requirement or approved by the Resident. 10 g) The Contractor is responsible for supplying, inspecting and maintaining traffic control devices in accordance with the project specifications. Contractor is subject to Penalty Damages for violation(s) per Supplemental Specification 652 and Special Provision 652. h) All signs, which do not apply to current construction activity, shall be 100% covered or removed in accordance with the plans. This includes speed limit signs when the work zone speed is in effect. i) All traffic control devices shall be NCHRP 350 compliant. Contractor access to and from the mainline shall not negatively impact mainline traffic flow. The Contractor may be required to establish lane closures to provide for safe access. Project Specific Items Bid Opening Opening: December 8, 2016 at 11:00 A.M. at MTA Headquarters 2360 Congress Street, Portland 13) Construction Schedule/Prosecution of Work: a) December 15, 2016 MTA Board to consider Contract Award b) February 15, 2017 Maxwell Road cannot be closed until on or after this date. May remain closed for no longer than seven months c) July 1, 2017 pier construction shall be complete and the long term lane closure and shoulder closure shall be removed d) November 17, 2017 (or 7 months after Maxwell Road closure) Maxwell Road shall be substantially complete e) March 1, 2017 Long term lane and shoulder closure with concrete barrier for pier demolition and construction may not be in place until on or after this date. The maximum duration for this lane closure is three months. f) June 15, 2018 Contract Completion Date 14) Cooperation With Other Contractors (Special Provision ): a) Within the vicinity of Maxwell Road, the following contracts are considered adjacent: i) MTA Contract Paving MM 80.8 to 88.6; Ferry Road Bridge Repairs MM 80.8; and Toll System Upgrades at Exit 86. ii) MTA Contract Northern Bridge Repairs: Auburn Interchange MM 75.3, River Road MM 79.4 and Goddard Road, MM ) Substantial Completion (Special Provision ) a) Maxwell Road shall be fully opened to traffic on or before November 17, i) Supplemental Liquidated Damages will be assessed per the table in the Supplemental Specification for every day substantial completion is not meet. 11 16) Specific Contract Items a) Section 603 Pipe Culverts and Storm Drains i) Existing 18 CMP under the existing Maxwell Road embankments needs to be cleaned prior to pulling a mandrel through and the new 12 inch HDPE pipe installed. b) Section 652 Maintenance of Traffic Truck Mounted Attenuator i) Utilized in lane closures or other construction operations where workers are exposed to traffic and not protected by positive means. ii) Addendum #1 will include an allowance of 100 calendar days at $150 each for Truck Mounted Attenuators. c) Contractor shall erect Portable Changeable Message Signs at least 10 days prior to Maxwell Road closing. 17) Questions Page 11 of 12
