1 March 23, 2018 Request for Proposal BID #: Lutsen # PROJECT NAME: Lutsen Apartments, Lutsen, MN ANNOUNCEMENT DATE: March 23, 2018 DUE DATE: April 6, 2018 at Noon PUBLIC BID OPENING: April 6, 2018 at 1:00 pm PROJECT OVERVIEW: One Roof Community Housing ( One Roof ) and Common Ground Construction as general contractor, in partnership with Grand Marais/Cook County EDA is requesting proposals for the installation (sub-contract) of four 4 plex modular units in Lutsen, MN. This project is slated to begin in May 2018 and completed by November 30 th, Bids will be accepted for the following work: excavation/site/road work, septic & water systems, plumbing, electrical, modular setting, modular finishing, foundation, and carpentry for porches and decks. Potential bidders may bid on a single or multiple components of the project. This project must be paid according to current MN State Commercial prevailing wages for Cook County, MN. If at all possible, American made steel must be used where applicable. Information associated to this request for proposal includes: 1. Northland Consulting Engineers Site Specifications signed and dated March 23, Homark Homes Installation Specifications for Electrical, Utility, and Foundation 3. The applicable Cook County Commercial Prevailing 2018 Wage rate information 4. Preliminary Certificate of Survey dated May 25, Specifications for the Steffes heating units 6. 2 Your proposal must be received electronically or via mail no later than Friday, April 6 th at noon. Please submit to: Brad Gilbertson, Project Manager Common Ground Construction brad@commongrnd.org Questions regarding this Request for Proposal should be directed to: Brad Gilbertson, Project Manager at or brad@commongrnd.org PROPOSALS: Subcontractors wishing to be considered for this project must submit proposals which conform to the following requirements: A. Cover sheet proposals must include a cover sheet that states: a. Bid # and Project Name (Lutsen #1-2018; Lutsen Apartments, Lutsen, MN) b. Bidder s company name, address, and point of contact c. If the Bidder is a partially owned or fully-owned subsidiary of another organization, identify the parent organization and describe the nature and extent of the parent organization s approval rights, if any, over the activities of the Bidder d. Bidder will comply with the current and applicable MN State Commercial prevailing wage for Cook County and submit weekly certified payroll information on form WH-347 e. The bid is a firm fixed-price f. Bidder has the capacity, availability, and experience to meet the timelines g. Bidder agrees that payment for work shall be on a percentage complete, plus onsite stored materials minus retainage. Final payment shall be made after: a Certificate of Occupancy or Completion has been executed; all lien releases have been received for completed work; and final inspection has been conducted B. Pricing provide a highly detailed bid C. Additional Documentation: a. Copy of Subcontractor s license b. Proof of general liability, risk, and workers compensation insurance Page 2 of 7

3 IMPORTANT: Late or incomplete proposals will not be accepted. All accepted proposals will be opened at our office in a public setting on Friday, April 6, 2018 at 1:00pm where Subcontractors are welcome to be present. Proposing bidders may be contacted for additional information. All bidders may request the results. Common Ground Construction reserves the right to engage in discussions or negotiations with none, any, or all bidders as part of the selection process. Common Ground Construction reserves the right to select or reject any or all proposals, and if deemed necessary, to engage in subsequent proposal rounds. After selection, Common Ground Construction and awarded bidder will finalize all outstanding matters and execute a contract. SPECIFIC REQUIREMENTS: 1.1 All requirements and standards must be included in a FIRM FIXED PRICE submitted by Sub-contractor and shall become part of a NO-LIEN CONSTRUCTION AGREEMENT between Common Ground Construction and Sub-contractor. Sub-Contractor may not assign construction agreement to another party. 1.2 All Subcontractors who procure Subcontractors must ensure that State procurement and prevailing wage regulations are complied with. 1.3 Final payment will only occur after the Certificate of Occupancy or Completion has been issued. 1.4 Sub-contractors are responsible for exact measurements, materials, and labor needed for bidding purposes. Bids must include all labor and materials to complete the job in a workmanship-like manner. 1.5 All costs to complete the Work described in these specifications and included on any drawings or plans should be included in the bid. Any scope of work Subcontractor does not believe is covered, but is needed for a workman-like completion of the Work, should be discussed with Common Ground Construction before submitting a proposal. 1.6 All materials used are to be new, of first quality and without defects, except in the case of harvest, salvage, or recycled materials used which have been approved by Common Ground Construction. 1.7 Change orders are for unforeseen circumstances or substantial changes to the Scope of Work only. Common Ground Construction must approve all change orders before additional Work is started. 1.8 Subcontractor is Common Ground Construction s partner in the field and Subcontractor will work closely with Common Ground Construction to resolve any defects in materials or workmanship. Subcontractor agrees to remedy any defect due to faulty materials or workmanship and pay for all damage to other work resulting there from, in accordance with the timeframe indicated in the current State statute. 1.9 Subcontractor must be licensed and bonded, if applicable. 4 1.10 Subcontractor must obtain and pay for all applicable permits and fees. Common Ground Construction shall ensure proper zoning of building lot to allow permitting to proceed. Subcontractor must comply with all rules of the permitting process as established by Cook County All work will comply with any and all applicable Federal, State and local Codes and Ordinances Sub-contractor understands that grant funds used to underwrite this project require oversight and approvals. SCOPE OF WORK Excavation/Site Work Clearing and grubbing Prep for foundations Execute stormwater management plan according to engineered specifications Road construction with bituminous surface as an alternate Remove and replace existing culvert at entrance of Hwy 61 Saw cut approximately 75 feet along Hwy 61 entrance Demo and remove existing building (list as an option on bid) Remove or abandon existing septic tank Design and construction of septic systems Excavate and install water distribution lines between wells and buildings; per engineered specifications Supply and install drain tile inside and outside; drain tile to be day lighted Supply crawl spaces with pea gravel and waterproofing in Buildings #1, #2, and #3 Supply pea gravel and water proofing to crawl space section of Building #4 and prep 8 foot section of Building #4 for concrete slab (slab poured by others) Prep for sidewalk at Building #1 from parking lot to front entrance of home, approximately 40 feet Final grading to include mulch, seed, blanketing, and boulder sloping per plan Excavator must be State certified to design and construct septic systems and submit design for approval to Cook County Land Use Department and obtain permit If awarded Subcontractor will obtain and provide a copy of the National Pollutant Discharge Elimination System (NPDES) MN permit and a copy of the Subcontractor s Erosion and Stormwater Certification to Common Ground Construction Any another specs as indicated on the engineered drawings Page 4 of 7

5 Foundation/Mason Supply and install three (3) crawl space foundations to the specifications provided and meet State code requirements Supply and install one (1) poured foundation which includes partial crawl space and partial 8 (foot) foundation per plan with concrete floor and 36 service door (not noted on drawing; location TBD); follow specifications provided and meet State code requirements Soil on interior of foundations to be covered with polyurethane to meet code requirements for perimeter foundations Water proofing of foundation walls Provide and install marriage wall supports per specifications Stucco applied to foundation walls above grade Provide price for one (1) 12 x 12 concrete pad for dumpster (list as an option on bid) Pour approximately 40 feet of sidewalk for Building #1 Provide option price for ICF foundation Electrical All work must be performed according to federal, state, and local building codes and completed by a licensed and insured electrician Electrician is responsible for permit application, permit approval, and fees Supply and install all materials for meter pedestals, including meters, at each building to accommodate four electric meters and four off-peak meters with disconnects Trench and install wire from pedestals to home; approximately 50 feet away from home; power company will connect to transformers Install Steffes off-peak electric heating units as shown on floor plans; units are supplied by Common Ground Construction; rough in electrical for these units will be factory installed; only on-site connections needed 200 Amp panel factory installed in all buildings/units Install 4 porcelain lights with switch in full basement Verify that all electrical connections are working in the home Wire off-peak Rheem electrical water heaters that are supplied by Common Ground Construction Supply and install electric heater in basement portion of Building #4; based on square footage of basement Page 5 of 7

6 Plumbing All work must be performed according to federal, state, and local building codes and completed by a licenses and insured plumbing Plumber is responsible for permit application, permit approval, and fees Water and sewer will be stubbed into the house by the excavator Supply and install water line from water supply line provided by excavator to the home and its plumbing system per specifications provided by modular home manufacturer (Homark Homes) Connect sewer and waste system from modular home to main sewer connections in crawl space per specifications provided by modular home manufacturer (Homark Homes) Supply and install a passive radon mitigation system in the crawlspace of the foundation; connect the passive radon mitigation system to the rest of the system provided by and installed in the modular home by Homark homes Test all plumbing systems (drains, waste, vent, and supply) to verify that they are in working condition Install and plumb Rheem Marathon non-metallic electrical water heaters that are supplied by Common Ground Construction House Transportation Delivery of 4 Buildings (32 sections) from Red Lake Falls, MN to Lutsen, MN including all associated permits Crane Work Transfer of 4-4 piece modular homes from delivery truck onto crawl space and all permits associated Setting Guiding the transfer of the modular units onto their foundations, including proper bolting and anchoring Installation of hinge roof system Supply and install support posts in foundation per manufacturer specifications Complete temporary weatherproofing to prohibit any water damage to buildings before button up is completed Page 6 of 7

7 Finish Carpentry Work Install LP SmartSide to gable ends; materials supplied by Homark Soffit, sub-fascia, and fascia to be completed on gable ends; materials supplied by Homark Complete roofing and shingle over cap on peak of home (approximately 12 feet across) Tape and texture on the marriage line and any other damage to drywall which may have occurred in transport Install doors, casing, and base on marriage walls Clean up and confirm everything works as it should; compile a list of service items Decks/Porches Supply and install all material to complete construction of decks, patio, and stairways Construction to be completed out of green treated bullnose decking and treated material Build according to supplied drawings Well Drill a non-com water well with casing to a depth of approximately 500 ; actual depth unknown (include MOB, casing, grouting, rock drilling, permitting for well, 1 water sample, and well cap) Include option for hydrofracking if needed Install water system (pump, drop pipe, wire down the well, pitless adaptor tank and controls) Include option for converting the existing well to a non-com public water well Page 7 of 7
