The following changes and/or substitutions to the plans and specifications are hereby made a part of same and are incorporated in full force as part of the contract. B. Bidders shall acknowledge receipt of this Addendum in writing on his Proposal Form. C. See the attached Supplemental Geotechnical Report for Drilled Pier Foundations and Structural Floor Slabs herein. Also - Add the following to the report: The angle of the bell to the shaft should be 30 degrees (slope of bell = 1.75V:1H or steeper). D. See the attached Proposal Form (Revised ) herein revising Unit Price #2 and Alternate #4, and the Completion Times. A7.2 SPECIFICATIONS A. Refer to Section , Para. 4.4, Typical Drains, and provide as indicated below. a. Floor Sink (FS): No with 3/4 Nikaloy top, aluminum dome strainer, clamp and acid-resistant porcelain enamel interior finish. Provide deep seal cast iron p-trap. b. Clarification: Note the FS requirement for the whirlpool in Training Room 105. Coordinate requirement with Owner s purchased equipment and provide as required. B. 3 PROPOSAL FORM (REVISED ) A NEW GYMNASIUM AND RENOVATED FINE ARTS FACILITY FOR THE MONTGOMERY CATHOLIC PREPARATORY SCHOOL MONTGOMERY, ALABAMA PROJECT NO Pursuant to and in compliance with the Advertisement To Bid and the proposed Contract Documents prepared by McKee and Associates, for the above referenced project, including Addenda, the undersigned, having become thoroughly familiar with the terms and conditions of the proposed Contract Documents and with local conditions affecting the performance and costs of the work at the place where the work is to be completed and having fully inspected the site in all particulars, hereby proposes and agrees to fully perform the work within the time stated in strict accordance with the proposed Contract Documents, including furnishing any and all labor and materials and do all work required to construct and complete said work in accordance with the Contract Documents, for the following sum of money. UNIT PRICES: The below Unit Price establish Unit Prices that the Owner can delete/add quantities from the Contracts required. Unit Price #1: The Contractor shall include in his Base Bid proposal the stripping of the existing topsoil layer and undercutting of the existing soil material to the depth as called for in the Geotechnical Report. This Unit Price shall be for the furnishing and installation of an additional 500 CYMIP of undercutting soft material and replacement with suitable compacted and tested material: 500 per CYMIP = $ Included in Base Bid Unit Price #2: This Unit Price shall be for the furnishing and installation of the following for each concrete pier size per linear foot: (The Contractor shall include in his Base Bid Proposal the length of 24 feet for each pier as described or shown on the drawings or issued by Addendum, this Unit Price shall be for any addition or subtraction per linear foot for each diameter described below): 30 $ per LF (Earth Unit) 66 $ per LF (Earth Unit) BASE BID: Amount to complete all work indicated in Contract Documents including the Unit Prices listed above, for the sum of: Dollars $ (Revised )

4 ALTERNATES: If alternates as set forth in the Bid Documents are accepted, the following adjustments are to be made to the Base Bid (Owner can choose any alternate in any order at his discretion): Alternate No. One: This alternate shall consist of all work and materials as described, shown and specified for the Polymer Flooring System in lieu of other flooring at the following Rooms: A108, A115, A116, A121, A136, A138, A139, A140. See the attached Supplemental Drawing Supp#1a and Supp#1b indicating the limits of this Alternate. (add) (deduct) $ Alternate No. Two: This alternate shall consist of all work and materials as described, shown and specified for the furnishing and installation of LVT Flooring in lieu of Porcelain Tile Flooring or other flooring at the following Rooms: A100, A101, A105, A106, A122, A128, A132, A144, A147, B126 and B140. (add) (deduct) $ Alternate No. Three: This alternate shall consist of all work and materials as described, shown and specified for the furnishing and installation of the Interior Synthetic Turf (Section 09657) in lieu of the Resilient Athletic Flooring (Section 09656). See the attached Supplemental Drawing Supp#2 indicating the limits of this Alternate at Weight Room A123. (add) (deduct) $ Alternate No. Four: This alternate shall consist of all work and materials as described, shown and specified for all work at the Student Center Room B122 except for the new exits (Doors B121, B122) and the new Mechanical Room and Equipment at Room B138. (add) (deduct) $ I agree to complete all the work to the stage Substantially Complete as follows: All work at the New Gymnasium shall be completed in 365 calendar days from the Notice To Proceed. All work at the Existing Gymnasium shall commence after the completion of the New Gymnasium Building. Therefore, this work shall be completed in 120 calendar days from the Notice Of Occupancy from the City Of Montgomery on the New Gymnasium. I understand that substantially complete will be complete to the stage of approval for occupancy as determined by the Architect (Revised )

5 I understand that the Owner reserves the right to reject this bid, but that this bid shall not be withdrawn for a period of thirty days from the date prescribed for its opening. If written notice of the acceptance of this bid is mailed or delivered to the undersigned within thirty days after the date set for the opening of this bid or at any other time thereafter before it is withdrawn, the undersigned will execute and deliver the Contract Documents to the Owner in accordance with this bid as accepted, and will also furnish and deliver to the Owner the Performance Bond, labor and Material Payment Bond and proof of insurance coverage, all within ten days after personal delivery or after deposit in the mail of the notification of acceptance of this bid. Notice of acceptance, or request for additional information may be addressed to the undersigned at the address set forth below. NOTE: If the bidder is a corporation, set forth the legal name of the corporation together with the signature of the officer or officers authorized to sign contracts on behalf of the corporation; if bidder is a partnership, set forth the name of the firm together with the signature of the partner or partners authorized to sign contracts on behalf of the partnership.
