THIS ADDENDUM IS ISSUED TO MODIFY, CLARIFY, EXPLAIN, OR CORRECT THE ORIGINAL SPECIFICATIONS. ANY CHANGES CALLED OUT IN THIS ADDENDUM SUPERSEDE THE ORIGINAL SPECIFICATIONS. 2. BIDDERS SHALL SIGN AND ATTACH THIS ADDENDUM TO THE INSIDE FRONT COVER OF THE SPECIFICATIONS AND ALSO ACKNOWLEDGE THE RECEIPT OF THIS ADDENDUM BY STATING THAT ACKNOWLEDGMENT ON THE OUTSIDE LOWER RIGHT HAND CORNER OF THE SEALED ENVELOPE CONTAINING THE BID. 3. 2 PRE BID CONFERENCE MINUTES PROJECT: Hawthorne and Thad W & WW Improvements ITB #: 124D 16F Date/Time: Friday, May 6, 2016 at 3:00 p.m. Location: 2 ND Floor Conference Room 2A Contacts: Darlene Humphries, Purchasing Supervisor, DHumphries@CityOfIrving.org Deborah Mcvean, M/WBE Program Administrator, DMcvean@CityOfIrving.org Matthew Estridge, Senior Civil Engineer, MEstridge@CityOfIrving.org Minutes from the Pre Bid Conference and the attendance lists will be posted on the Purchasing web site for this solicitation. Attendance of the Pre Bid Conference is not mandatory. PURCHASING DEPARTMENT INFORMATION 1. All questions/comments regarding the Project Documents must be directed to the Purchasing Agent or Engineer in writing. No addendum/clarification to the project documents will be given verbally; only a Bid Addendum may alter the Project Documents. The deadline for question/comments is Monday, May 9, 2016 at 3:00 p.m. 2. A bid addendum will be issued no later than Tuesday, May 10, 2016 at 3:00 p.m. It is recommended that Bidders verify receipt of all Addenda prior to submitting a bid since Bidders must acknowledge all Addenda including marking the lower right hand corner of the sealed envelope containing the bid. Check the purchasing solicitation website 3. The bids/proposals will be publicly opened immediately after the Closing Time on Friday, May 13, 2016 at 3:00 p.m. Any bids received after the Closing Date/Time will be returned unopened. No bid may be revised after it is opened. 4. Submitted spreadsheets will NOT be accepted in lieu of filling out the Bid Pricing Pages in the Solicitation Book. Bidders are highly encouraged to verify all Bid Pricing Pages have been properly filled out (i.e. Unit Prices written in words match numerical representation of the same and Total Price for each bid item has been correctly calculated). 5. City is requesting Bidders to submit THREE (3) complete sets (1 original for Purchasing & 2 copies for Capital Improvements) of all documents being submitted in response to this ITB. Each set should include all items identified in the top two boxes of the Checklist For Bidders Attachment B Experience Record: Bidders are required to submit a minimum of three references for work completed within the past 24 months that was similar in scope and size. 60% or more of work is to be done by Contractor s crews. Provide a list of all proposed Sub Contractors and their three references for similar work of same size and scope. 7. M/WBE Goal 30%: Bidder acknowledges they will attempt to reach the M/WBE goal with submittal of their proposal. Recommended bidder will be required to submit additional forms prior to award showing how they are or will be attempting to meet the M/WBE goal with subs.

3 GENERAL PROJECT INFORMATION 8. Project Completion: Contract Time for this project is 180 Calendar days with no allowance for weather or holidays, etc. Contract time will start as identified in the written Notice To Proceed (NTP). Contract time will stop when ALL work is complete, including punch list items and stabilization of disturbed ground. This project includes a liquidated damages clause that calls for 1/500 of the contract amount per day with a maximum of $1, per day (Ref: General Provisions and Requirements, Section G.7.7, Failure to Complete on Time. ) Anticipated Council Approval date is Thursday, June 9, Anticipated Pre Construction Meeting date is Monday, July 11, Anticipated NTP & Contract Start date is Monday, July 11, Anticipated Contract End date is Saturday, January 7, Public Awareness: City will mail letters to residents/businesses notifying them of the impending construction activities and applicable contact information for the Project. 10. Contractor Submittals: Awarded Bidder is encouraged to prepare/submit to the City ALL product/materials based or other related shop drawings at or before the Pre Construction Conference. Product/materials based submittals should include the latest applicable technical information from the manufacturer that constitutes a complete installed item. The City desires these types of submittals to be made via in PDF format. The SWPPP Book is required to be submitted two (2) days prior to the Pre Construction Conference. 11. Construction Materials Testing: City will provide independent laboratory testing services for initial construction material tests only that are required for the Project. Subsequent re testing of substandard materials shall be paid for by the Contractor. 12. Public & Franchise Utilities: Conflicts with public and franchise utilities are not anticipated at this time. All existing public and franchise utilities (water, wastewater, storm sewer, gas, electric, communication, CATV, etc.), whether shown on plans or not, must be supported/protected in place (Non Pay Item). Any damages to the same resulting from construction activities for this Project shall be repaired or replaced to the satisfaction of and no additional expense to the City and/or Franchise Utility Owner. 13. Traffic Control: Traffic Control Plans have been prepared for this Project and are included in the plans. City and TxDOT reserve the right to make adjustments to these TCPs as needed. A minimum seven (7) day advanced notice of lane closures is required. Temporary riding surfaces for utility cuts, if required, are considered subsidiary to Section Trenching, Embedment and Backfilling per Section Construction Sequence/Schedule: A detailed construction schedule will be required for this Project and shall be submitted two (2) days prior to the Pre Construction Conference. 15. Pavement Restoration: Estimated limits for pavement restoration are identified on the plans. Final pavement restoration limits will be coordinated with and approved by City s Engineering Inspector and/or Streets Department. 16. Specification Clarifications & Additional Requirements: Clarification Wastewater Manholes (02016): All concrete wastewater manholes will include ConShield both precast and cast in place. 4 Clarification Temporary Riding Surface for Utility Cuts (02621): The temporary riding surface throughout project shall be asphalt or concrete. The use of flexbase as the riding surface must be approved by Engineer. No measurement or payment shall be made for this work. Maximum difference in adjacent riding surfaces is two (2 ) inches. Additional Requirement Cleaning and TV Inspection Of Exist Storm or WW (02036): The depth of standing water allowable for mains 6 inch through 24 inch shall be no greater than 0.50 inches for grades less than 0.70%. No standing water is allowable for grades 0.70% and greater. Any storm sewer mains that are removed and replaced as part of this project shall be TV inspected in accordance with Section (non pay item). 17. Water Shutdowns: Water shutdowns will be coordinated with the City s Engineering Inspector and affected residents/businesses to minimize impacts accordingly. 18. Permits: TxDOT Utility Installation Review permits will be required. The Engineer s estimate for ADA related Bid Items (35 27 & 35 28) totals $15,300. If the awarded Bidder s total value of the same is equal to or greater than $50,000, TDLR Project Registration and inspection by a Registered Accessibility Specialist will be required. 19. The Engineer s opinion of probable construction cost for the project is $1.18M. QUESTIONS AND ANSWERS Are Items thru for filling all the pipe? The City requires all existing water and wastewater main pipes that are abandoned in place to be cut, capped and filled with grout and these items, in addition to 35 26, are compensation for same by the linear foot for the various diameters of pipe to be abandoned. See Sections and for more information. Cellular grout mix designs may be submitted for consideration as an alternate. Is surveying included in price? Yes. Construction related surveying is the responsibility of the Contractor and costs are considered subsidiary to the project (i.e. non pay item). See Section for more information. How long does it take to get water turned off after requesting a date? City s Engineering Inspector requires a minimum 72 hour notification from the Contractor for a planned water shut down. Who notifies residents of a water shut down? City s Engineering Inspector is responsible for providing residents and businesses with a minimum 48 hour advanced notice via door tag hanger for planned water shut downs. Is a certified plumber for sewer line required? City requires a licensed plumber for all work performed on private property only. ITB # 124D 16F Pre Bid Conference Minutes Page 3 of 4

5 Is depth of pipe defined? Per Section 15060, a minimum of four feet of cover is required along improved streets and a minimum of five feet of cover is required along unimproved streets. Identified pay items for water main pipe are measured by the horizontal linear foot installed regardless of depth of cover. Pipe installed at shallower depths will require approval by the City Engineer. What is maximum length for open trench? Six hundred feet. Can trenches be covered with steel plates?
