1 ELECTRICAL SAFETY INSPECTION REPORT LIBAS TEXTILES LTD., Mouchak, Kaliakoir, Gazipur, Bangladesh Factory List: 1. Libas Textiles Ltd. Inspected on April 6, 2014

2 SUMMARY The Libas Textiles Ltd., factory established in three building along with a shed. The main factory building, reportedly, built in 2007, for industrial purpose, started production in the same year.the building was approved for industrial purpose. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design / installation constraints, shall be submitted to the Accord for approval. Libas Textiles ltd Page 2 of 7

3 FINDINGS AND RECOMMENDATIONS Finding #: E- 1 Power cables installed close to steam lines. Power cables installed near boiler steam lines must be protected from external heat and moisture (may keeping sufficient clearance between steam pipes and cable/installing adequate thermal-insulation on the steam pipe). Cable tray and stem line in production floor Within 1 month Finding #: E- 2 MCB mounted on wall without enclosures. 4 Finding #: E- 3 SERVICE LINE Service cable from pole not supported. Service cable must be supported with suitable means (may be installing supporting-pole and the cable clamped on pole). Service line entering building Finding #: E- 4 Cables are not protected and randomly arranged in cable tray. Cables on floor may be supported on covered tray/ladder. Cables must be arranged & tied inside the tray/ladder, securely fixed on wall/floor. Power cables in electrical substation room. 5 Finding #: E- 6 WIRINGS The opening of the wall after passing cable not sealed. The openings remaining after passage of the wiring system through permanent wall should be sealed according to the degree of fire resistance of building construction before penetration. Cable passing through wall Finding #: E- 7 Cable entering panel protected by flexible PVC not supported. Cables entering panel must be protected in rigid conduit and clamed with saddle properly. Flexible conduit must not be used for long point wiring (except for special wirings). Cables drawn from changeover switch supported by rope from ceiling Finding #: E- 8 Cable entering panel not supported. Recommendation. Cables entering panel must be protected in covered cable-tray/ladder up to the base-plate of the panel to prevent any physical damage. Power transformer installed in electrical room. 6 Finding #: E- 9 Excessive dust deposit inside panel. Disconnect the power source of the panel and clean dust and debris of all interior components. Establish a periodic cleaning program and maintain records of the activities. Provide cover made of noncombustible material on unused opening for preventing ingress of dust and debris in future. Cable arrangement inside distribution floor Finding #: E- 10 The opening of the floor after passing cable not sealed Remaining gap after passing cable through the floor should be sealed according to the degree of fire resistance of building construction before penetration. Remaining gap of the floor Finding #: E- 11 WIRINGS Wires and cables randomly supported on same cable tray Cables must be arranged and tied using cable-tie inside the cable tray. 7 Finding #: E-12 Power cables directly laid on concrete floor. Install cable duct to protect the noted cables and provide covers made of non-combustible material preferably metal to protect the cables insulation from any physical damage as well as prevent the ingress of debris, dust and lint. Power cables on floor Finding #: E- 13 Multiple cables terminating to MCBs in panel.. Multiple cables connecting at a MCCB terminal must be removed.
