1 I INVITATION TO BID FLOORING MATERIALS The is accepting sealed bids for the purchase and installation of Flooring Materials at the until 2:30 p.m. (our clock) on November 28, 2016 at City Hall located at 110 Main Street, Fort Morgan, Colorado at which time and place all bids will be publicly opened at 2:45 p.m. If hand delivered, they are to be sent to 110 Main Street, Fort Morgan, CO If mailed, the address is PO Box 100, Fort Morgan, CO Bids must be received at City Hall prior to 2:30 p.m. (our clock) on November 28, A mandatory pre-bid meeting will be held on Thursday, November 17, 2016 at 2:00 p.m. at the in the conference room, 414 Main Street, Fort Morgan. All prospective bidders must attend this pre-bid meeting in order for the City to accept their bid. All questions regarding this bid should go to Chandra McCoy, Director of Library/Museum Services. Questions must be submitted in writing via to chandra.mccoy@cityoffortmorgan.com. 2 I INSTRUCTIONS TO BIDDERS I. GENERAL INSTRUCTIONS All bids must be properly signed by an authorized representative of the company with the legal capacity to bind the company to the contractual agreement. In addition, the bidder s authorized signature of the bid assures the company s understanding of the City s Procurement Policy. A copy of the policy is available upon request at the City Clerk s Office. Bid(s) may be withdrawn up to the date and time set for closing upon written, faxed or ed notice to the City. Bid(s) may not be withdrawn for a period of sixty (60) days after the date of opening. Negligence upon part of the bidder in preparing their bid(s) shall not constitute a right to withdraw a bid(s) subsequent to the bid opening. A conditional or unqualified bid may be cause for rejection. Only Bids properly received will be accepted. Each Bid and applicable documentation as requested must be submitted in a sealed envelope, addressed to: and bear the name of the bidder and their address. Separate pricing will not be considered unless otherwise noted. The intent of this bid is to purchase fully functional equipment for the purpose it is designed to meet. I. Scope of Work A. Provide all materials, equipment and labor to remove and dispose of existing flooring and to install new flooring in the entryway, main library floor, and the Howard Rollin Memorial Community Room floor at the, 414 Main Street, Fort Morgan, Colorado. II. Specifications ALL MATERIALS MUST BE MINIMUM FULL GRADE COMMERCIAL OR BETTER. THE CITY OF FORT MORGAN RESERVES THE SOLE RIGHT TO DETERMINE IF THE BID MEETS SPECIFICATIONS. A. Installation: 1. Bidder shall provide all materials, equipment and labor to install flooring materials in accordance with all local, state, and national codes, laws, regulations and ordinances. Page 1 of 2

Bidder shall prepare any surface for the installation, including the moving of furniture, or repair of carpet and floor coverings, and install carpet, resilient sheet goods, resilient tile, wood floors and flooring (including the finishing and repairing thereof), and any other materials established as flooring and floor covering material. 3. Install new carpet and any associated required padding, adhesive, tack strip, transition strips, and seam work. B. Minimum Specifications: 1. Flooring materials will consist of commercial grade carpet, carpet squares, floating vinyl plank, laminate, cork flooring or linoleum. C. Warranty: Manufacturer's warranty shall be not less than one (1) year on all flooring materials. Bidders submitting literature stating warranties which do not fully comply with warranty requirements of these specifications must submit a letter from the manufacturer certifying warranty compliance as an integral part of their proposal. D. Options: Any option that could improve the safety, serviceability, reduced ownership/operating costs, durability or operator comfort of unit is of interest. Please list all available options and cost for these options that are not covered in the Minimum Specifications separately on the Bid Form under Options. E. Delivery: 1. Delivery and installation shall be completed at the following address: 414 Main Street Fort Morgan CO III. Submittals All bids submitted pursuant to this invitation shall contain: 1. Completed and signed Bid Form; 2. Original manufacturer's product data sheet (including dimensions, rated capacities, shipping weights, and accessories). 3. THE UNDERSIGNED CERTIFIES THAT HE/SHE HAS EXAMINED THE SPECIFICATIONS AND INSTRUCTIONS TO BIDDERS AND HAS SUBMITTED A BID IN FULL COMPLIANCE AND WITHOUT COLLUSION WITH ANY OTHER PERSON, INDIVIDUAL OR CORPORATION. The undersigned, having become familiar with the local conditions affecting the cost of the work, hereby bid and propose to furnish all management, operators, tools, supplies, equipment, material, transportation and labor service necessary to perform and complete in a workmanlike manner all of the work required in connection with the installation of the items listed and for the unit prices set forth in the Bid Schedule. All Bids must be made on the required Bid Form. All blanks spaces for Bid prices must be filled in, printed in ink or typewritten, and the Bid Form must be fully completed and executed when submitted. Only one copy of the Bid Form is required.
