2 Church Wardens Fabric Report Completion of the Form Use a ballpoint pen when completing the form to ensure it shows through to the copies beneath The sheets have a perforation at the spine to help with removal Once the form has been completed in full, please detach two of the forms (coloured white and pink) and send them to your diocesan office. The remaining copy should be retained within the book for parish reference. Collection of Data The Churchwardens Fabric Report fulfils sections 4 and 5 of the Church Fabric Regulations so is the means by which Churchwardens can communicate the key building fabric issues to their PCC and to the wider church. The information will also be analysed to help the Representative Body understand the key issues for church buildings and plan resources accordingly. Guidance Notes Guidance notes to assist you in the completion of your forms are enclosed in this booklet. Deadline Date Please could you ensure that the Church Warden s Fabric Report is sent back to your diocesan office as soon as possible after your Annual Vestry Meeting, but no later than 1 May

3 Guidance Notes Building Condition Section: This section is designed to give an at a glance indication of the general state of the building and the priorities in the year ahead. Please refer to the Quinquennial Inspection report but try to complete this section from your own observation and knowledge. Please ensure a valid faculty is in place before starting any work; contact your Diocesan Advisory Committee Secretary. A checklist is also provided, in this booklet, to help you with the inspection of your building. Regulatory Requirements Section: This section is designed to act as a prompt to make sure you are complying with key regulations where needed. It will also help the Church in Wales identify where additional support is needed to help PCC s meet their legal responsibilities. Detailed guidance is available in the Parochial Administration Handbook but in outline: Health and Safety Policy: Excellent guidance and proformas are available from Ecclesiastical at aspx Fire Risk Assessment: Detailed guidance is contained in the Ecclesiastical Guidance for Health and Safety and also at Asbestos Survey and Management Plan: The Quinquennial inspection should advise on this but guidance is available from Lightning Conductors: Specialist advice should be sought as to whether these are required and how they should be inspected and maintained. Gas Inspections: Your gas installation, if you have one, should be checked annually by a Gas Safe engineer. Memorial Safety Check: Memorials in your graveyard should be checked for stability and safety annually.
