Contract Description: General Contract Dear Mrs. Dorrell, PMg respectfully requests the consideration and approval by the Board of County Commissioners of Change Order Number Three (3) to the contract with Helterbrand Builders' for construction work at the Jury Assembly Room. Approval of this request would add $4, to the contract amount. PMg is familiar with the requested item(s) and the justification for each change follows the item description below: 1. Add for excavation and examination of tunnel roof for water infiltration $3, issues along north exterior basement wall of the jury assembly room. Justification: Required for installation of new waterproofing at that location. 2. Add additional vinyl lettering "Jurors Only" at the three entrance doors $ per AS #2 dated August 14, Justification: Owner Requested 3. Add additional emergency exit lighting in the north half of the jury $1, assembly building and one wall box for the television. Justification: Unforeseen condition. Required by the city building inspector. Funding for this change order is available in the contingency line item of the "construction budget". Boulder Ave. Suite 1200 Tulsa, OK re: Tulsa County Courthouse Jury Assembly Room Remodel Change Order #3 Recommendation Mr. Crowe: We recommend approval of Change Order #3 to add $4,935 to Helterbrand Builders' contract for construction. This Change Order adds to the contract work that was required or requested as the project was nearing completion. The first item was for a verbal directive provided by PMG to Helterbrand to uncover the top and sides of the tunnel leading from the north end of the jury assembly room to the parking garage to allow for the installation of new waterproofing at that location. Helterbrand's subcontractor, N-D-N Waterproofing and Restoration, performed this work. Their invoice includes $2,450 for seven days of equipment rental. The waterproofing work was performed by the County. This work is categorized as Owner requested work. The second item was to add additional lettering on the entrance doors. This work is cateqorized as Owner requested work. The final item adds additional emergency egress lighting at the existing ceiling at the north half of the room, which was not originally included in the scope of work.
