The project included specific intersection signal upgrades and replacement of all existing curb ramps with ADA compliant curb access ramps. Bayou Texar Bridge Replacement Escambia County, FL Senior Project Engineer for this contract. Complete replacement of the Bayou Texar Bridge between East Pensacola Heights and East Hill in of Pensacola Florid The Bayou Texar Bridge was designed with emphasis on architectural treatments including pedestrian parapet walls, pedestrian handrails, and bridge lighting. The project included specific intersection signal upgrades and replacement of all existing curb ramps with ADA compliant curb access ramps. Intersection signal improvements required the construction of drilled shafts to support the proposed mast arm signal poles. Due to numerous utility conflicts, direct organization with the District Geotechnical Engineer and the Engineer of Record was required to direct the efforts of the Contractor for required changes to the proposed mast arm foundations. 8 (Present as many projects as requested by the agency, or 10 projects, if not specifie Bayou Texar Bridge Replacement Pensacola, Escambia County, FL PROJECT OWNER POINT OF CONTACT POINT OF CONTACT TELEPHONE NUMBER Florida Department of Transportation Eric Benson, P.E POINT OF CONTACT ADDRESS POINT OF CONTACT FAX NUMBER The 511-foot long Bayou Texar Bridge spans Bayou Texar between East Pensacola Heights and East Hill within the historical district of Pensacola Florid The construction of the new Bayou Texar Bridge necessitated the phased removal of the existing bridge structure (circa 1950's) and utilization of a 690' ACROW Bridge to carry vehicle traffic over Bayou Texar during the duration of construction. The Bayou Texar Bridge was designed with specific emphasis on architectural treatments. The exposed exterior faces of the 57' Type III Girders included parabolic recessed striations to depict the look of a sawn girder. The Bridge also included architectural treatment of the pedestrian parapet walls, pedestrian handrails, and bridge lighting. The CEI inspection staff for the steel girder erection, high strength bolting operations, utilized the turn-of-the-nut method, and proper documentation of the project Torque wrenches utilizing a Skidmore-Wilhelm apparatus. 10 (Present as many projects as requested by the agency, or 10 projects, if not specifie Carpenter Creek Bridge Replacement Escambia County, FL PROJECT OWNER POINT OF CONTACT POINT OF CONTACT TELEPHONE NUMBER Florida Department of Transportation Eric Benson, P.E POINT OF CONTACT ADDRESS POINT OF CONTACT FAX NUMBER The $2.4 million 98-foot long Carpenter Creek Bridge Replacement Project spans Carpenter Creek on SR 291 (Davis Highway). The existing crossing of Carpenter Creek was over a 4 barrel box culvert. Construction of the Carpenter Creek Bridge required the phased removal of the existing box culvert and the utilization of a 120' ACROW Bridge to carry vehicle traffic over Carpenter Creek. The substructure consisted of 24" pre-stressed concrete piles driven in the abutment locations. The superstructure consisted of a single span of 97' Type III Girders. 11 (Present as many projects as requested by the agency, or 10 projects, if not specifie Davis Highway/Olive Road Intersection Escambia County, FL PROJECT OWNER POINT OF CONTACT POINT OF CONTACT TELEPHONE NUMBER Florida Department of Transportation Eric Benson, P.E POINT OF CONTACT ADDRESS POINT OF CONTACT FAX NUMBER The Davis Highway/Olive Road Intersection is one of the most heavily traveled intersections in Escambia County. Prior to construction of this project, Davis Highway was a 4-lane urban roadway and Olive road was a 2-Lane roadway. This project required the 6-Laning (grassed medians) of Davis Highway and the 4-Laning of Olive Roa Storm water drainage was routed from the project improvements to a controlled release storm water retention pon The Storm water pond was constructed with a sand filter under drain to facilitate a controlled drawdown of the storm water pond volum The pipe system for this project required the installation of 1.2 miles of mainline pipe ranging in size from 18" to 66" installed to depth of 22 feet deep. Overhead truss signs were included as part of the work for this project for routing of traffic to the adjacent Interstate 10 access ramps. The overhead truss signs were installed on 5' Diameter shafts installed to a depth of 23'. The span width of the overhead trusses ranged from 144' to 156'. The overhead trusses were set during a night time road closure operation in order to allow for minimal disruption to traffic on the Davis Highway roadway. 8 STANDARD FORM 330 (05/2008) PAGE 11

12 (Present as many projects as requested by the agency, or 10 projects, if not specifie SR 87 Reconstruction Nichols Lake Road to US 90 Santa Rosa County, FL PROJECT OWNER POINT OF CONTACT POINT OF CONTACT TELEPHONE NUMBER Florida Department of Transportation Billy Robinson (850) This 3.4 mile capacity improvement project and reconstruction project required the complete removal of the existing two lane SR 87 roadway and the construction of both a divided 4-lane urban roadway and a divided 4-Lane rural roadway. 13 (Present as many projects as requested by the agency, or 10 projects, if not specifie SR 30 Shoulder Revetment on Okaloosa Island Okaloosa County, FL PROJECT OWNER POINT OF CONTACT POINT OF CONTACT TELEPHONE NUMBER Florida Department of Transportation Billy Robinson (850) This $14.4 million project was constructed across the barrier island in south Okaloosa County which supports US 98 as the primary traffic route for commuters between the coastal towns of Ft. Walton Beach and Destin. This roadway section is vulnerable to storm surge overtopping associated with Hurricane landfall events. This Project included the construction and installation of 9 miles of stainless steel gabion mats for shoulder revetment of the critical US 98 corridor. This project also required the construction and installation of 6 miles of composite sheet pile walls with a reinforced concrete cap. The sheet wall was constructed in highly vulnerable sections of the roadway that have failed and been destroyed during prior Hurricane landfall events. Bob Worley is a highly experienced Construction Engineer specializing in intricate Roadway and Bridge Heavy Construction Projects. Mr. Worley has worked in the CEI Field since Mr. Worley began his career in the CEI Field as an Inspector and has served as Office Engineer, Project Engineer and a Senior Project Engineer for Florida Department of Transportation projects during his 24 year CEI career. Mr. Worley has supervised a staff of up to 26 engineers and technicians overseeing work in multiple counties within the FDOT District 3 are Mr. Worley currently serves as the President and Senior Project Engineer for. Mr. Worley is highly knowledgeable in construction methods and has specific in-depth knowledge related to: pile foundation construction, test pile static and compression loading programs, bridge substructure construction, bridge superstructure construction, false work construction, plate girder (structural steel) assembly and erection, high strength bolted connections, concrete foundation construction in wet conditions, sheet pile installation, and cofferdam construction. Mr. Worley has also served as a Design Engineer for multi-lane roadway projects. Specific design experience includes: Geometric Design, Super Elevation Alignment and Design, Maintenance of Traffic Plans, Erosion Control Plans, and quantity cost estimates and calculations. I.
