1 Invitation to Bid Blacktop Sealcoating and Striping Dunlap High School Dunlap Middle School Dunlap Grade School Sealed Bids will be received by the District Office, 3020 Willow Knolls Road, Peoria, IL 61614, on April 18 th, Sealed bids are due no later than 2:00 PM. All Contracts for the Construction of Public Works are subject to the Illinois Prevailing Wage Act (820 ILCS 130/1-12). The bid documents naming the specific Project, Scope, Bidding Instructions and Submission Details can be found on the District website (click on District, open Request for Bids) or by contacting Tom Grimm at This project will take place the Summer of 2017.

3 Dunlap CUSD #323 Blacktop Sealcoating/Striping Dunlap High School, Dunlap Middle School, Dunlap Grade School Bidder Name Sealcoat and Striping Contact Name Phone# or Fax# Safety and Insurance Requirements Job Site Specific Safety Plan to be prepared and submitted to Dunlap CUSD #323 prior to mobilization. Insurance requirements necessary to complete installations are included in base bid. A Certificate of Insurance must be issued to Dunlap CUSD #323 prior to commencement of work at the site. Identify additional insured as required. Prior to starting any work provide MSDS sheets for all material used on the job. To promote safety awareness on our jobsites, Subcontractor may be asked to participate in a site-wide Safety Walk-Through. General Items All sales and use taxes are to be excluded. Successful contractor will be provided a tax exempt certificate. Union and/or prevailing wage installations for all work performed on the site Full time supervision is included for this scope of work If clean-up becomes an issue, the offending Contractor will be requested by Dunlap CUSD #323 to remove identified trash. Failure to promptly remove trash will result in Dunlap CUSD #323 removing it at Contractor's expense, a minimum of $250. The process will be repeated as needed. If numerous Contractors repeatedly use common trash piles, all Contractors on site at the time of the clean -up will share the cost of the clean-up. Identify all second tier subcontractors. Lien waivers to be provided for each payment request. Licenses, business or bond fees as required by the City or Village. All plan review and inspection fees for your work will be included in your bid. Mobilization as required will be included in your bid. All material FOB to the job site. Your company representative will attend all coordination meetings and prepare fully coordinated shop drawings. The cost for engineering, CAD drafting, and plan reproduction will be included with your bid. Provide the name of the foreman and PM you have designated for this job. You are responsible to complete all the necessary drawing and permit applications forms as required by the City or Village. Drawings & As-built on the latest version of AutoCAD. Did you attend the pre-bid walk through or visit the site prior to submitting your bid? Supply O&M manuals and owner training session. Any testing required by local authorities or testing agencies is included in your bid Subcontractor pricing is to include all work that is reasonably inferable from the drawings, specifications and this scope of work sheet.

4 Each contractor has the responsibility to immediately examine the contract documents for errors, inconsistencies, or omissions. Any such errors, inconsistencies, or omissions shall immediately be brought to the attention of Dunlap School District. Schedule Requirements Premium costs necessary to complete the schedule have been considered and are included in your base bid. You will guarantee the man power to complete the job per Dunlap CUSD #323 schedule, or work overtime if you fall behind the schedule at your cost. Base Bid Tax Exempt $ Scope of Work Description Seal Coating Clean all surfaces where sealcoat is to be applied. Apply Seal Master Coal Tar Pavement Sealer, or equivalent, at square feet per gallon. Sealer to be modified with 2 pounds of silica sand per gallon. Sealer to have Tarmax additive, or equivalent, for faster curing, stronger more durable finish. 8 Notice to contractor: You are receiving this notice because you may or will be performing public works for the Dunlap School District #323 as that term is defined in Section 2 of the Illinois Prevailing Wage Act (820 ILCS 130). This notice applies to the public works described as: Dunlap Community Unit School District #323. The Prevailing Wage Act requires contractors and subcontractors to pay laborers, workers, and mechanics performing services on public works projects no less than the prevailing rate of wages (hourly cash wages plus fringe benefits) in the county where the work is performed. For information regarding current prevailing wage rates, please see the Illinois Department of Labor s website at All contractors and subcontractors rendering services for the public works must comply with all requirements of the Prevailing Wage Act, including but not limited to, all wage, notice, and record-keeping duties.
