3 INTRODUCTION AND BACKGROUND PURPOSE OF THE REQUEST FOR PROPOSAL Mid Michigan Community Action (MMCAA) is requesting and accepting proposals to construct a natural playground at our Farwell Early Childhood Services Building located at 2050 West Cadillac Drive Farwell, Mi (Property is located on the corner of M-115 and Surry Road) Services shall be performed as specified in the scope of services. Start RFP Distribution to Contractors 05/14/ Questions from Contractors about scope or approach due 05/21/ Responses to Contractors about scope or approach due -05/22/ Proposal Due Date 05/29/ Anticipated decision and selection of Contractor(s) 05/31/ Anticipated commencement date of work 06/11/ Anticipated end of work date 07/11/2018 GUIDELINES FOR PROPOSAL PREPARATION PROPOSAL SUBMISSION Award of the contract(s) resulting from this RFP will be based upon the most responsive bidder(s) whose offer(s) will be the most advantageous to MMCAA in terms of cost and quality. MMCAA reserves the right to: Reject any or all offers and discontinue this RFP process without obligation or liability to any potential Contractor Accept other than the lowest priced offer Award a contract on the basis of initial bids received, without discussions or requests for best and final offers Award more than one contract Bidders shall submit bid packages containing several parts as set forth below. Bidders will confine submission to those matters sufficient to respond to this RFP and to provide adequate basis for MMCAA to evaluate a bidder s qualifications. 4 Successful bidder s submission in response to this RFP will be incorporated into the final agreement between MMCAA and the selected bidder(s). Complete submissions must contain the following: Title Page: Show the submittal subject, name of firm, local address, telephone number, contact person and date. Company Profile: Provide an overview of the company including location, number of staff, and describe the services, materials and products the firm provides. Project Team: Identify the project team members (management, support staff, field workers, etc.) that will be assigned to the project. Pricing: Indicate the firm s pricing, caps and other requirements for each requested service, material and/or product that the bidder is responding to. The detailed requirements for each of the above-mentioned sections are outlined in this RFP. Bid packages should consist of single copies of each required document (no duplicates necessary). Bids submitted that lack any of the required information or documentation may be deemed incomplete and may not be considered for selection, at MMCAA s discretion. Submissions must be received no later than 4:00 p.m. on 05/29/2018 at the offices of MMCAA. All bids may be ed, faxed of mailed. Washington Road PO Box 768 Farwell, MI tellenwood@mmcaa.org Fax: Attn: Terry Ellenwood DETAILED RESPONSE REQUIREMENTS SCOPE FOR CONTRACTORS Outlined below is the scope of work that MMCAA requires from the Contractor: Contractor will supply all tools, equipment, materials, and supplies to complete the project. Contractor will provide a copy of liability insurance and workers compensation policy, including amounts. MMCAA will provide a detailed review or walk through of the project with contractor upon request before May 22, There may be $25.00 monetary fee subtracted from the final price for each day the project extends past the project deadline. All components of the project must have a one year warranty from completion date. All wood must be pealed debarked Cedar Species. Unless otherwise specified. All treated lumber must be non-arsenic (CCA) treated wood. A Material Safety Data Sheet for all treated lumber used in this project is required before using it in the project. 5 1. Install a maximum of 160 linear feet of four inch cedar post fencing, along the south side. Posts and Rails are to be pealed of the bark. This will be set up in front of the present chain link fence. See photos: 2. Plant four clusters of native Michigan flowers and grasses consisting of 15 to 20 plants in each cluster. (Black-eyed Susan s, Butterfly bushes, daisies, lavender, maiden grass, blue stem grass, etc..) Plants must be suitable for location. 3. Remove approximately 20 cubic yards of Rubber chips on the outside of bike path. Will need to install two 2 x 8 X 10 treated boards to terminate mulch bed. Install approximately 20 cubic yards of top soil (top of soil must be level once compacted with existing concrete sidewalk / bicycle path), and Hydroseed with premium grass seed. 4. Move one 10 2 W X 14 6 L shed about 100 feet. Shed is to be installed with the front doors and front trim piece inside fence. About 11 of fence will have to be removed and two new 2 terminal posts installed and cemented in place. Stretching and reattaching the present fence to the terminal posts. The new location for this shed has very uneven ground the ground will have to be filled with base material and leveled. RFP Mid Michigan Community Action Agency Page 5

6 5. Build one 8 X 8 stage that is level to the ground with slab wood as a roof. Wood shall be natural with no finish, peeled of bark. Install a supplied Xylophone in the floor under the roof. All wood in contact with the ground shall be either a cedar species or Non Arsenic (CCA) treated lumber. Location to be determined. See photo: 6. Build ½ log seats. Each log section needs to be ½ of a log that is cut in half length wise. Each section needs to be 8 to 10 inches wide by 8 feet or longer. Three sections with seating for twenty children three to four per log. Must be a Cedar Species either Red or White Cedar. Pealed of all bark. Each section needs to be anchored in the ground to prevent tipping. Location to be determined. See Photo: RFP Mid Michigan Community Action Agency Page 6

7 7. Construct two cedar post trellis like the one in the photo on the inside of the 5 gate. See Photo: 8. Construct two Log Balance Beams. Each beam should be crooked and about 4 to 6 feet long and 8 to 10 inches wide. These need to be buried so that the top is no more than three inches above ground level. Logs must be either Red or White Cedar, with the bark removed. See photo: 9. Construct a stepping log course. Highest stepping stump shall be no more than 18 high or less. All stumps must be anchored in the ground a minimum of thirty (30) inches deep. Logs must be either Red or White Cedar, with the bark removed. 8 10. Supply a small sand hill of washed sand. Hill is to be approximately four feet high by five feet round. This hill should be hemmed in with small logs on each side. Logs must be either Red or White Cedar, with the bark removed. 11. Install rain gutter on one side of shed. Purchase and install one rain barrel that meets or exceeds this rain barrel: 50-Gallon Darkened Ribs Rain Wizard Barrel, Color: Terra Cotta, by Good Ideas, Model #: RW50-DR-TC. Barrel must include a Rain Barrel Diverter to fit the barrel. 12. Construct two individual play houses. The plans will be available for this project See Photo: RFP Mid Michigan Community Action Agency Page 8

9 13. Construct one raised bed planter. Using Non Arsenic (CCA) treated lumber landscape timbers. This will be a corner bed located in an undetermined corner of the playground. 14. Construct one water course lined with 3 to 4 rocks. Wood should be Non-Arsenic (CCA) treated wood or of a Cedar Species, Red or White Cedar. Supply this water pump with cistern or equivalent. 10 15. Construct one small seating area, with seating for ten children. All wood should be either red or white cedar. Need one large stump in front for an adult to sit on between 16 and 18 inches high. Stump may not exceed 18 inch in height. Stump must be either Red or White Cedar, with the bark removed. Project Team Provide names of key staff and management personnel Name Title Contact Information Pricing Provide an accurate price list to complete the project. AWARD CRITERIA The following elements will be the primary factors in evaluating all proposals and in the selection of a Contractor or Contractors: Overall cost of Contractor s proposal. Capacity to provide service MMCAA may, at their discretion and without explanation to the prospective Contractors, at any time choose to discontinue this RFP without obligation to such prospective Contractors.
