1 Addendum No. TWO Date: Project: PUBLIC HOUSING COMMUNITY RE-ROOFING PROJECT FOR THE ALBERTVILLE HOUSING AUTHORITY, ALBERTVILLE, ALABAMA MCKEE PROJECT NO The following modifications/ changes and/or substitutions to the plans and specifications are hereby made a part of same and are incorporated in full force as part of the contract documents. Bidders shall acknowledge receipt of this Addendum in writing on their Proposal Form. A1.1 GENERAL MODIFICATIONS: A. The Attached Mandatory Pre-Bid Meeting, Sign In Sheet is hereby made part of the contract documents. A1.2 SPECIFICATION MODIFICATIONS: A. Provide a pre-manufactured Fiber Glass Cupola, as shown on Main Office Roof, equal to the following, see attached manufactures cut sheet: Cupola Model No. 400 By Piedmont Fiberglass Taylorsville, NC Submit Equals per specification process B. See attached Revised specification section SHINGLES in its entirety i. Note the Change to a 130 mph wind warranty ii. Note the following change to Pre-Approved Shingle Manufacturers: Manufacturers: The following manufacturers' products have been used to establish minimum standards for materials, workmanship and function. 5 EXISTING CONDITIONS & DEMO EXAMINATION OF THE SITE AND ALL EXISTING CONDITIONS: DUE TO THE NATURE OF EXISTING BUILDINGS, THE GC MUST COMPLETELY REVIEW ALL EXISTING BUILDINGS/ ROOFS PRIOR TO BIDDING TO VERIFY ALL QUANTITIES AND DIMENSIONS. DRAWINGS ARE INTENDED TO AID THE GC AND DEFINE THE INTENTIONS OF THE OWNER. TYPICAL SCENARIOS ARE SHOWN ON DRAWINGS AND ARE INTENDED TO BE ACCURATE; HOWEVER NO GUARANTEE IS MADE THAT QUANTITIES SHOWN ARE ALL INCLUSIVE OR THAT DIMENSIONS SHOWN ARE ACCURATE. ADDITIONALLY, BUILDING TYPES MAY HAVE VARIATIONS FROM BUILDING TO BUILDING. GCs SHALL REVIEW ALL PROPERTIES AND DETERMINE THEIR OWN QUANTITIES AND DIMENSIONS PRIOR TO BIDDING. ALL BID PROPOSALS SHALL TAKE INTO CONSIDERATION ALL SUCH EXISTING CONDITIONS THAT MAY AFFECT THE WORK UNDER THIS CONTRACT. DEMOLITIONS SCOPE OF WORK & ADDITIONAL REQUIREMENTS: 1.) REMOVE ALL EXISTING ROOFING CONSTRUCTION MATERIALS (METAL PANELS WITH PREVIOUS ASPHALT SHINGLE LAYERS BELOW). REMOVE ALL EXISTING UNDERLAYMENT LAYERS (FELT, ETC.) TO EXPOSE WOOD ROOF DECKING. EXISTING WOOD DECKING IS TO BE EXAMINED BY OWNER WHILE EXPOSED. GC TO COORDINATE SCHEDULE WITH THE OWNER'S ON SITE REPRESENTATIVE. DAMAGED ROOF DECKING IS TO BE REPLACED AT THE DIRECTION OF OWNER PER THE ESTABLISHED UNIT PRICES (REFER TO PROPOSAL FORM IN PROJECT MANUAL). RECYCLING OF METALIC MATERIALS. GC WILL HAVE TOTAL RESPONSIBILITY FOR REMOVAL AND RECYLING OF METALIC MATERIALS. NOTE THAT THIS WORK WILL NO LONGER BE HANDLE BY A THIRD PARTY WORKING FOR THE INSURANCE AGENCY BUT WILL BE THE DIRECT RESPONSIBILITY OF THE GENERAL CONTRACTOR. THE GC SHALL PROVIDE DOCUMENTAITON STATING THE AMOUNTS (WEIGHT) THAT IS RECYLCLED AND THE DOLLAR AMOUNT THAT IS RECOUPED BY THE GC. 3.) THE GENERAL CONTRACTOR IS RESPONSIBLE FOR ALL OTHER TRASH AND DEBRIS REMOVAL. CLEAN UP ALL ROOF CONSTRUCTION RELATED DEBRIS AND PLACE IN DUMPSTERS ON A DAILY BASIS SO THAT NOTHING IS LEFT ON THE GROUND. ANY TRASH OR DEBRIS MATERIAL TEMPORARILY REMAINING ON SITE, MUST BE STORED IN A GC SUPPLIED DUMPSTER. NO TRASH OR DEBRIS MATERIALS CAN BE PLACED ON THE GROUND. ALL CONSTRUCTION RELATED ITEMS SHALL BE REASONABLY SECURED SO THAT CHILDREN OR OTHER RESIDENTS CAN NOT ACCESS ANY ITEMS POSING HARM. ON A DAILY BASIS, THE CONSTRUCTION TEAM MUST USE A MAGNETIC DEVICE TO REMOVE ANY ROOFING NAILS WHICH MIGHT HAVE FALLEN TO THE GROUND. 4.) REMOVE AND RE-ATTACHED ALL EQUIPMENT (CABLE TV & SATELLITE EQUIP, ETCETERA) AND ALL OTHER FASCIA AND ROOF ATTACHMENTS AS NEEDED TO PROPERLY INSTALL NEW FASCIA TRIM AND OTHER ROOFING COMPONENTS. RE-ATTACH ALL ITEMS IN THE ORIGINAL LOCATION. SUPPLY NEW FASTENERS IF NEEDED. 5.) THE GENERAL CONTRACTOR IS RESPONSIBLE FOR SECURING AND PROTECTING ALL EXISTING TO REMAIN PORTIONS OF THE BUILDING. ANY ITEMS DAMAGE DURING THE CONSTRUCTION PROCESS SHALL BE REPLACED BY GC AT NO ADDITIONAL COST TO THE OWNER. NOTIFY THE ARCHITECT & OWNER PRIOR TO BEGINNING WORK IF ANY ITEMS ARE ALREADY DAMAGED. GC IS RESPONSIBLE FOR VIDEOING OR PHOTOGRAPHING ALL EXISTING CONDITIONS TO VERIFY ANY PRE-EXISTING DAMAGED PROPERTY. PRIOR TO BEGINNING WORK WITHIN AN AREA, PROVIDE A WRITTEN REPORT TO THE ARCHITECT UPON DISCOVERY OF DAMAGED PROPERTY, WITHIN THAT AREA. COORDINATE THIS ACTIVITY AND OWNER RESPONSE TIME WITH YOUR OVERALL SCHEDULE OR WORK. 6.) CONTRACTOR SHALL ORGANIZE WORK TO PROTECT BUILDINGS FROM WEATHER. WATER SHALL BE PREVENTED FROM ENTERING THE BUILDING AT ANY TIME. THIS SHALL INVOLVE KEEPING PENETRATIONS SEALED, AND SEQUENCING THE WORK SO THAT NO AREA IS LEFT EXPOSED DURING A RAIN EVENT. TARPS WILL BE ALLOWED TO TEMPORARILY PROTECT FROM RAIN. ANY DAMAGE TO TENANT PROPERTY OR AUTHORITY'S PROPERTY DUE TO WATER REACHING THE INTERIOR OF THE BUILDING, IS TO BE REPLACED BY THE GC AT NO ADDITIONAL COST TO HOUSING AUTHORITY. 7.) ANY DAMAGE TO EXISTING LANDSCAPE SHALL BE REPLACED AT NO ADDITIONAL COST TO THE OWNER. IF ANY GC EQUIPMENT, OR OTHER ITEMS, IS LEFT ON THE GROUND LONG ENOUGH FOR THE GRASS TO DIE, THEN THE AREA SHALL BE RE-SODED AT NO ADDITIONAL COST TO THE OWNER. THIS INCLUDES THE PROPOSED MATERIAL LAY DOWN AREAS. NEW SOD SHALL MATCH EXISTING. 8.) RIDGE VENTS AND AIR CIRCULATION: EXTENTS OF NEW RIDGE VENTS ARE INDICATED ON EACH UNIT TYPE ROOF PLAN. CONTRACTOR SHALL INCREASE THE OPENING IN THE ROOF DECKING FOR THE RIDGE VENT AIR CIRCULATION, AS DIRECTED IN THE TYPICAL RIDGE VENT DETAIL AND PER MANUFACTURER'S RECOMMENDATION. REFER TO DETAILS AND PROJECT SPECIFICATIONS FOR ADDITIONAL REQUIREMENTS. 2

HOWEVER, WORK MUS BE COMPLETED IN A GIVEN AREA PRIOR TO INITIATING NEW WORK IN ANOTHER AREA, SO THATNO MORE THAN 25% OF THE TOTAL PROJECT SHALL BE UNDER CONSTRUCTION AT ONE GIVEN POINT IN TIME.

7 SECTION SHINGLES (30 YEAR ARCHITECTURAL, 15 YEAR NON-PRORATED) PART 1 - GENERAL RELATED DOCUMENTS: Drawings and general provisions of the contract including General and Supplementary Conditions and Division 1 Specification Sections apply to work of this section. DESCRIPTION OF WORK: Extent of shingles is shown on drawings and is hereby defined to include units employed as weather protection for walls as well as for roofs. Types of shingle applications specified in this section include the following: 1. Fiberglass shingle roofing QUALITY ASSURANCE: UL Listing: Provide labeled materials which have been tested and listed by UL for Class and Rating indicated for each shingle type required. SUBMITTALS: Product Data: Submit technical product data, installation instructions and recommendations from shingle manufacturer, including data that materials comply with requirements. Shingles shall be installed in accordance with manufacturer's instructions. Samples: Submit full range of samples for color and texture selection. After selection, submit 2 full-size shingles for verification of each color/style/texture selected. Product Handling: Deliver materials in manufacturer's unopened, labeled containers. Store materials to avoid water damage, and store rolled goods on end. Warranty: Supply an unexecuted copy of the Manufacturer s Product Warranty. JOB CONDITIONS: Substrate: Proceed with shingle work only after substrate construction and penetrating work have completed SHINGLES (ARCHITECTURAL)

8 Weather Conditions: Proceed with shingle work only when weather conditions are in compliance with manufacturer's recommendations and when substrate is completely dry. Provide copy of manufactures weather conditions limits or factors to the Architect at the Pre-Roofing Conference. GENERAL CONTRACTORS ROOFING WARRANTY OR GUARANTEE: The General Contractor must execute and honor the Roofing Guarantee that follows this spec section. SPECIFIED PRODUCT WARRANTY OF MANUFACTURER: The roofing manufacture shall be required to provide documentation certifying that the roof design provided complies with the performance requirements as set forth in IBC Chapter 15, Section The documentation shall be attached to the roof warranty at the close out of the project. Manufacturer s Warranty: Provide shingle manufacturer's warranty on installed work, agreeing to pay for repair or replacement of defective shingles as necessary to eliminate leaks. Warranty shall cover the cost of full labor and materials. The requirements of warranty shall include: 130 mph wind warranty 30 years Warranty from the date of substantial completion of Project Must be non-prorated labor coverage for a minimum period of 15 years Must have an algae protection plan for the first 10 years Must have a wind protection plan for the first 15 years Standard manufacturer s roofing guarantees which contain language regarding the governing of the guarantee by any state other than the State of Alabama, must be amended to exclude such language, and substituting the requirement that the Laws of the State of Alabama shall govern all such guarantees. All roof warranty s shall be provided to the Owner, by the Contractor at the Final Inspection to obtain the Substantial Completion. Supply an unexecuted copy of the Manufacturer s product warranty along with all other submittal requirements. PART 2 - PRODUCTS SHINGLES 30 Year Architectural Series Shingles, UL Class "A": Mineral-surfaced, self-sealing, fiberglass based asphalt strip shingles complying with ASTM D 3018, Type I, bearing SHINGLES (ARCHITECTURAL)

9 UL Class "A" external fire exposure label and UL "Wind Resistant" label. Color to be selected by the Owner from manufactures standards. Manufacturers: The following manufacturers' products have been used to establish minimum standards for materials, workmanship and function. Color as selected from manufactures standards: Equal to Tamko Building Products, Heritage 30, 130 mph wind warranty. Owens-Corning GAF Atlas Certain Teed Equal products of other manufacturers may be used in the work provided such products have been approved by the Architect, not less than five (5) days prior to scheduled bid opening. Impact Resistance: Roof coverings installed on low-slope roofs (roof slope <2:12) shall resist impact damage based on the results of tests conducted in accordance with ASTM D 3746, ASTM D 4272, CGSB 37-GP-52M or the Resistance to Foot Traffic Test FM All valleys shall be California Cut. Felt Underlayment: One layer of a Class A synthetic flame retardant underlayment with following requirements: 1. Tensile Strength ASTM D lbs 2. Tear Strength ASTM D lbs 3. Puncture Strength ASTM D psi 4. UV Rating ASTM G90 90% 5. Permeability ASTM 96-A 0.10 Perms Products shall be equal to Palisade SDP Advanced Polymer Products, or equal products as manufactured by GAF, Tamko, ELK or CertainTeed. Install Felt with roofing nails no staples will be allowed. Ice & Water Shield Underlayment: Supply 40 mil self adhering ice and water shield membrane equal to Carlisle: Dri-Start A or Palisade SA-HT. Install as directed below and Refer to Section Details in Drawings for additional placement requirements, as well as the layering of adjacent materials. Valley Install: Install 18 in each direction from middle of all valleys SHINGLES (ARCHITECTURAL)

10 Eave Install: Install upward from the edge of all eaves a total distance of 36. Ridges & Hips: Install on all ridges and hips a distance of 30 on each side of all ridge and hip lines. Rake Install: Install 30 Minimum from edge of all Rakes. Over hang rake edge as indicated in details. Roof Penetrations Install: Install around all roof penetrations as indicated in section details. Wall intersection Install: Install 18 minimum up the face of the wall. Remove existing vinyl siding and reinstall per manufacturer s requirements. Ice & Water shield shall be applied in single un-cut strips as much as possible by using the widest product available to cover the above areas as indicated. Roofer shall take great care to minimize the number of joints. If adjoining or overlapping strips are necessary, the strips must overlap a minimum of 6. Fixed Ridge Vents: Ridge vent equal to V-600 by Cor-A-Vent, 11 wide, 20 square inches NFVA per lin. ft., or approved equal. PART 3 - EXECUTION Installer of shingles must examine substrate and conditions under which shingling work is to be performed and must notify Contractor in writing of unsatisfactory conditions. Do not proceed with shingling work until unsatisfactory conditions have been corrected in manner acceptable to Installer. Pre-Roofing Conference: A pre-roofing conference is required before any roofing materials are installed. For projects that consist solely of Re-Roofing work, this conference may be one and the same as the required Pre-Construction Meeting. This conference shall be conducted by a representative of the Architect and attended by representatives of the Owner, General Contractor, Roofing Contractor, Sheet Metal Contractor, Roof Deck Manufacturer (if applicable), and the Roofing Materials Manufacturer (if warranty is required of this manufacturer). If equipment of substantial size is to be placed on the roof, the Mechanical Contractor must also attend this meeting. Coordinate the scheduling of the pre-roofing conference with the Architect, provide a minimum of ten (10) days notice prior to desired date of conference SHINGLES (ARCHITECTURAL)

11 The pre-roofing conference is intended to clarify demolition and application requirements for work to be completed before roofing operations can begin. This would include a detailed review of the specifications, roof plans, roof deck information, flashing details, and approved shop drawings, submittal data, and samples. If conflict exists between the specifications and the Manufacturer s requirements, this shall be resolved. If this pre-roofing conference cannot be satisfactorily concluded without further inspection and investigation by any of the parties present, it shall be reconvened at the earliest possible time to avoid delay of the work. In no case should the work proceed without inspection of all roof deck areas and substantial agreement on all points. The following are to be accomplished during the conference: 1. To review all Factory Mutual and Underwriters Laboratories requirements listed in the specifications and resolve any questions or conflicts that may arise. 2. To establish trade-related job schedules, including the installation of roofmounted mechanical equipment. 3. To establish roofing schedule and work methods that will prevent roof damage. 4. Require that all roof penetrations and walls be in place prior to installing the roof. 5. To establish those areas on the job site that will be designated as work and storage areas for roofing operations. 6. To establish weather and working temperature conditions to which all parties must agree. 7. To establish acceptable methods of protecting the finished roof if any trades must travel across or work on or above any areas of the finished roof. Installer will at the Pre-Roofing Conference provide a minimum of 4 copies of the Manufacturer s Installation Guide, that will be initialed and dated by the Installer and General Contractor, indicating a full understanding of the Manufacturer s Installation Guide. Architect will provide a written report indicating actions taken and decisions made at this pre-roofing conference. This report shall be made a part of the project record and copies furnished to the General Contractor and the Owner. PREPARATION OF SUBSTRATE: Clean substrate of any projections and substances detrimental to shingling work. Coordinate installation of shingles with flashing and other adjoining work to ensure proper sequencing SHINGLES (ARCHITECTURAL)

12 INSTALLATION: General: Comply with instructions and recommendations of shingle manufacturer in relationship to roof application, except to extent more stringent requirements are indicated within drawings and specs. 1. Installer Qualifications: Installer must be licensed or otherwise authorized by all federal, state and local authorities for installation of all roofing products to be installed under this section. 2. Installer Qualifications: Prior to beginning work, provide written documentation that Roofing Company is Approved or Certified by the same Shingle manufacturer as the approved submitted shingle product. SHINGLES: Underlayment: Apply one layer felt horizontally over entire surface, lapping succeeding courses 2" minimum and fastening with sufficient nails (no staples) to hold in place until shingle application. Install saturated felt starter courses as per low slope application requirements lapped and cemented as indicated by the manufacturer. Selection and application of asphalt roofing and siding products published by the Asphalt Roofing Manufacturers' Association Copyright 1966 Revised Shingles: Install manufactures starter strip of shingles with tabs removed at eave edge, ridge and hip conditions. Fasten asphalt shingles to roof sheathing with nails. The application of the shingles will be by hand and the application pattern will be diagonally across the slope of the roof. Racking of the shingles will not be permitted. Pneumatic nail guns will not be permitted for installation of shingles. Staples will not be permitted. SHINGLE ATTIC STOCK: At Completion of the job, provide 3 Squares of roofing shingles to Owner. END OF SECTION SHINGLES (ARCHITECTURAL)
