2 Addendum #2 The following information is issued to change, clarify, supplement or further explain the above referenced RFP. This information shall supersede and take precedence over the information contained in the RFP in the event of a conflict. Q1. What is the deck height on 1 st & 2 nd floor? 1 st floor level to 2 nd floor level is nd floor level to 3 rd floor level is 13. Q2. Will this project have an accelerated schedule or normal schedule? Please refer to bid documents, UTHSCSA, General Conditions and Owners Special Conditions. Ultimately, the contractor must follow UTHSCSA guidelines & establish a schedule as part of their means and methods. A proposed schedule is shown in the RFP; however, contractors should present what they deem to be a realistic schedule in the proposal response. Q3. Will this project be during regular business hours or after hours? Please refer to bid documents, UTHSCSA, general conditions and owners special conditions. Ultimately, the contractor must follow UTHSCSA guidelines & establish a schedule as part of their means and methods. Most work can be performed during normal hours; however, work that produces vibrations (jackhammering, hammer drilling, etc) or exceptionally loud work must be performed after hours. Q4. What is the estimated start date and end date? Please see Addendum #1, page 9, section 2.4 at the below link: Q5. Do you know of any general contractors that are bidding this project? Please refer to the pre-submittal conference sign in sheet at the below link: Q6. Please indicate UTHSCSA s budget for this project. From a planning perspective this project falls into our category of work deemed to be a major renovation. Each proposer should provide their best price as part of the RFP response. Q7. Does UTHSCSA s have the ability to award to a contractor with a price proposal above the cited budget if UTHSCSA decides that contractor would bring best value to the completion of this project? 3 Q8. Will UTHSCSA award this project if all offerors submit price proposals that are above UTHSCSA s budget for this project? We did not provide any budgeting ranges. The contractor shall submit their best price for the proposal along with their realistic schedule. The RFP document gives them a place for both price as well as duration. We ve posted only suggested durations, for planning purposes. Q9. The UTHSCSA, General Conditions Section 00, Para. 0.3, states that the work hours for this project are from 7:45am to 4:15pm, Monday through Friday. However, if the contractor s project schedule determines that the project cannot be completed in the time frame provided by UTHSCSA using standard hours but rather needs to be worked 7 days a week, 12 hours per day, at a minimum, is this going to be acceptable to UTHSCSA? The standard work hours for UTHSCSA are as shown; however there are many activities that will need to be done during after 6PM: jack hammering, heavy demolition, hammer drilling, etc. The schedule is up the contractor s own means and methods, and FM can provide access to the job site with prior coordination. We have staff available between 7:45am and 2:30 am M-F and other access can be arranged with advance notice. We also plan to provide at least one exterior entrance that is available for access via a construction core. Q10. Will this project require any city permits to be pulled? There are no city permits that are for work on State property. Q11. Please clarify the definition of a printed page as it relates to the 50-page limitation for proposals. Does one page equal one side of a sheet of paper (for a limit of 50 printed sides), or does one page equal one sheet of paper (for a limit of 100 printed sides)? 1-sheet printed on both sides equals 1 page. Q12. May offerors use paper larger than 8.5 x 11-inch for charts and similar elements (e.g., the required Gantt Chart for the project plan and work schedule) that are more clearly presented in a larger format for better readability / clarity of presentation? Yes. Q13. Will UTHSCSA consider excluding the Gantt Chart from the 50-page limitation for proposals? Yes. Q14. If the Gantt Chart is not excluded from the page limitation, and if paper larger than 8.5 x 11-inch for the Gantt Chart is acceptable, please confirm that the larger page(s) will each count as a single page against page limitation. We will exclude this from the count. Q15. Will UTHSCSA consider excluding the detailed Safety Plan from the 50-page limitation for proposals? This could account for a significant percentage of the allotted pages. Yes. A full safety plan can be submitted after a contractor has been selected. ADDENDUM [#2] PAGE 3 OF 7 06/16/2016 ISSUED

4 Q16. RFP p. 10, item , states, One (1) original and six (6) copies of the Proposal shall be submitted to include 1) Qualifications and the 2) Pricing and Delivery Proposal with Execution of Offer in a sealed envelope. a) Should the Pricing and Delivery Proposal with Execution of Offer be submitted is a sealed envelope separate from the other elements of the proposal? No. b) If yes, please confirm that the separate envelope containing the Pricing and Delivery Proposal with Execution of Offer does not need to be bound (e.g., spiral bound) with the other elements of the proposal. Only the HUB Subcontracting Plan must be submitted in a separate envelope. Q17. Please confirm that (in addition to the cover, table of contents, divider sheets, HUB Subcontracting Plan, Pricing and Delivery Proposal, and Execution of Offer, as noted on RFP p. 10) the following elements are also excluded from the 50-page limitation set for responses: Cover letter Bid security bond Vendor Payee Form Samples reports (associated with item of Exhibit 4, Respondent s General Questionnaire) Yes. Q18. Please clarify the tab requirements. Should offerors set tabs in alignment with Section 3 (RFP p. 10) or in alignment with the sections of Exhibit 4, Respondent s General Questionnaire? Please follow section 3 of RFP, page 10. Q19. Please confirm that offerors do not need to include an actual copy of Exhibit 4, Respondent s General Questionnaire (i.e., the PDF pages in the RFP) in their submissions as long as they respond to all the questions in a way that clearly aligns with the questionnaire. I think this is fine. Q20. Please clarify whether offerors need to repeat each question when responding to the items of Exhibit 4, Respondent s General Questionnaire. The instructions of the questionnaire itself note that offerors should repeat the questions; however, the instructions on RFP p. 11 (para ) indicate that repeating questions is not necessary. Regarding Exhibit 4, item 1.2.3: Offerors are instructed to provide a work plan (i.e., Gantt chart): a) It is our understanding that offerors will satisfy the requirements of this item (including sub-items ) by submitting a Gantt chart alone. Please confirm or clarify. Confirmed. b) Please confirm that the requirements of Section 3, item 3.5, align with the requirements of Exhibit 4, item Confirmed. Q22. Please confirm that the requirements of Section 3, item 3.6, align with the requirements of Exhibit 4, item Confirmed. Refer to response to Q15. Q23. Does UTHSCSA have a structural repair protocol in place that would cover repairing rebar materials cut during core drilling operations? If yes, please provide this information. Cores should avoid rebar. If core location is critical and rebar is encountered, submit RFI during construction for review prior to cutting rebar. Q24. Does UTHSCSA know how thick the existing 1st and 2nd floor concrete is in the restroom areas? If yes, please provide this information. Existing building documents show the concrete slab thickness to be 4 1/2. Q25. Does UTHSCSA know the rebar sizing and layout that might be encountered during core drilling? If yes, please provide this information. Existing building documents show heavy gauge WWM in these areas along with rebar extensions from concrete beams. Q26. Will the replaced generator become the property of the General Contractor or is to be turned over to the client? If it is to be turned over, what is the protocol for returning this material? No, leave the existing 150kw generator in place, direct connected to the existing fire pump. Q27. Does UTHSCSA have a facility report identifying any ACM that exists in this building? If so, please provide. We have had materials tested in the work site and no ACM was been identified, based on the preliminary survey. If materials are uncovered during the project that are suspected ACM, the contractor should report those immediately to the owner, where we will then follow the General Conditions. 6 Q28. Can the General Contractor use the existing elevators to move materials up to the second floor if needed? Elevators may be used after hours, provided that they are protected and cleaned prior to the following morning. It is our preference that the exterior windows identified during the project walkthrough be used to transport material during the business day. Q29. Does UTHSCSA have a protocol for maintaining fire suppression coverage during renovation projects, or can the General Contractor elect to have a full-time fire watch in place if they need to remove or drain the existing system down for more than 24 hours? Interim protection will be discussed in detail with the awarded contractor. All NFPA rules apply; however, we can usually agree upon interim measures consisting of partial fire alarm Coverage and partial fire sprinkler coverage using the following guidance: Minimum, daily, Construction Site coverage is required. Monitoring Devices can be left hanging in the area. During active construction periods they must be programmatically disabled, bagged, or covered with thin plastic to minimize dust contamination of internal detection element. With respect to fire sprinklers, any over 8 hours needs to be provided with a fire watch if the project site, unless the project Additionally, only the floors under construction can be drained, all other building floors must remain active. Q30. Is it assumed that the General Contractor will not be held responsible if the existing air handlers fail to achieve the required cfm flows as reflected in the mechanical plans? Replacement of the existing AHU s are not part of this project. Q31. On plan page AD 1.2, note D9 states to remove the terrazzo base, but it does not mention removing the main terrazzo flooring. Is this an error, or is the intent to install the new flooring over the existing terrazzo flooring? It is the intent to keep the terrazzo floor but to remove the base as noted. New Cementous flooring is to applied on top of the prepped terrazzo flooring finish. Q32. On plan page AD 1.1, room 135, 138.1, 139 and 143 have dashed lines on the door but do not have note D2 labeled on them. Is the intent to demo these? Yes. These doors are to be replaced with new and shall be equal to new doors as shown on Door schedule. Q33. In hard ceiling areas that are not being demoed but require existing rework of the circuitry, is it acceptable if we need to install a paint grade access panel in these areas? Paint grade flush panel 24 x24 access panels will be acceptable. Reference spec section in the UTHSCSA Technical Specifications. Coordinate with ODR for acceptable locations.. Q34. Please confirm that, as stated at the pre-submittal conference, the installation of the generator will be a distinct scope element to be completed at a later date (i.e., after the completion dates indicated on the revised Pricing and Delivery Proposal form provided in Amendment 1). This is correct. The pricing and delivery schedule allows for the proposer to enter separate dates for the completion of the base bid as well as for the alternate. 7 Q35. It was discussed during the 2nd walk-through meeting that Specifications have multiple division sections that are not relative to this project. Will there be an addendum to clear the specifications and omit the sections that will cause confusion during the bid process? No addendum will be issued. Q36. Due to the short anticipated schedule of the project, there will be several trades that will have to work after hours. Can we get an exact time frame of when our trades (i.e., demolition crew) will be able to enter the building after normal working hours to prevent disruption to the tenants on the above floors? Refer to the response to Q9. Generally, extensively loud or vibration producing work can occur between 6:00 PM and 1:30AM; however, access can be provided at any time with advanced notice. Q37. Are the existing concrete floors post tension? If so we will need as-builts showing the location prior any core drilling. No PT is known to exist in the building. Q38. Sheet A3, Note 4 under miscellaneous schedule, calls to apply 45 minutes quickset skim, 2 coats over existing wall coverings. There were multiple areas where the wall covering was not fully adhered to the wall, this is of concern. The preferred method to deal with the existing wallcovering would be to: 1) Demo the existing wallcovering 2) Seal the remaining glue with a primer 3) Skim coat with mud This method would be preferred in lieu of apply 2 coats of quick set mud on the existing wallcovering. Please advise. We see this as the contractor s means and methods. Q39. The specifications include Liquidated Damages of $1,000 per day. Due to the schedule of this project, we request that the Liquidated Damages be reduced. No adjustment will be made. Refer to the response to Q2, the contractor should propose a realistic and obtainable schedule with their proposal. The dates provided by the contractor in the RFP response will form the basis for the required contractual completion date. Q40. Will there be a restriction on when our MEP trades can work in the space above the ceilings below the third floor due to the 3rd floor occupants? If so, this would require after hours work in that space. Refer to the response to Q3. Drilling into concrete would need to occur outside of regular business hours for this floor.
