1 ELECTRICAL SAFETY INSPECTION REPORT. Plot # B-32,BSCIC I/E,Tongi,Gazipur Factory List: 1. Kento Asia Limited. Inspected on May 15, 2014

2 SUMMARY Kento Asia Ltd. factory is established in a rented premises consisted of Six (6) sheds built for industrial purpose in 2008, but the officials we met were not sure of the year when the sheds were built. Factory rented the premises and started operation in 2008, and the factory, at the time of survey, reported to have about 664 workers working on regular basis. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design / installation constraints, shall be submitted to the Accord for approval. Page 2 of 9

3 FINDINGS AND RECOMMENDATIONS E- 1 TRANSFORMER ROOM In adequate space around the transformer. Panels installed near transformer may be rearranged and guards around the transformer may be expanded. Barrier wall between the transformer and panels may be constructed. 6 Months Panel installed near transformer and narrow space between the tranformer and guard. E- 2 TRANSFORMER ROOM Silica gel in transformer breather, discoloured. Replace silica gel in the transformer breather and refill the oil cup to trap dusts entering transformer through breather. Transformer breather (at far end). Silica gel color turned white. E- 3 SERVICE LINE Sharp bent in HT cable near termination, at the HT breaker panel. Cable must be supported at the panel base plate and terminated without stressing at the termination point. HT cables near termination are sharp bent. Page 3 of 9

4 E- 4 SERVICE LINE HT Cables laid directly on concrete floor. 11kV HT cable must be supported in cable trays or laid in trenches. The cable must be protected against physical injury. HT Cables laid directly on concrete floor near HT panel. E- 5 SERVICE LINE HT service cable dropping from pole is not protected near the base of the pole, above ground level. HT cable dropping from 11kV pole must be protected in steel pipe of required size at least 2m from the ground level to protect from physical injury by moving objects. HT service cable droping from pole. E- 6 SWITCH BOARD & PANELS Panel base plates removed to allow cable entry. Panel base plates must be installed, at all time, and cable(s) entering panel must be firmly fixed with cable gland. Cables entring panel. Page 4 of 9

5 E- 7 SWITCH BOARD & PANELS Hot spots at terminations inside panel. Loose connection or overloading are the causes of over heating. Regularly check terminations for loose connections and tighten it. Overloading may be the cause. Thermal imager measuring hotspots inside panel. E- 8 SWITCH BOARD & PANELS Cables and wires inside panel are not securely fixed. Cables and wires inside panel must be arranged and securely fixed to the panel safely. Layout of MCCBs inside panel may be rearranged. Cables and wires terminating at MCCBs inside panel E- 9 Cables terminating at MCCB (ACB) not protected and extention plates not securely placed in safe position. Cables terminating at MCCB terminals with extenders must be securely fixed in safe possitions and phase barriers between terminals must be installed. Cables may be fixed securely to connect to the MCCBs. SWITCH BOARD & PANELS Cable termination at MCCB are extended using additional copper bars. Page 5 of 9

6 E- 10 SWITCH BOARD & PANELS Multiple wires terminating to MCCB terminals. Multiple cable connecting at a MCCB terminal must be disconnected. Existing multiple circuits may be distributed through bus bars. Multiple cables are terminating at single MCCB inside panel (typical). E- 11 SWITCH BOARD & PANELS Multiple cables connected at a terminal of the busbar. Cables and wires terminating to bus bars must be independently connected installing thimble/lugs. Multiple cable terminating at bus bars inside panel E- 12 CABLE & CABLE SUPPORTS Wooden wiring ducts used for wiring support. Existing wooden ducts supporting wiring may be replaced with non-combustible ducts supported at regular intervals. Wooden material is used as cable duct for wiring. 6 Months Page 6 of 9

7 E- 13 GENERATOR ROOM Generator frame not connected to earth. Generator frame must be connected to earth with proper size earth conductor. Solid copper conductor connecting to earth must be connected securely with loops. Generator frame is not connected to earth. E- 14 WIRINGS Wirings in flexible PVC conduit partially supported to roof trusses. Wirings under ceiling or roof must be protected from external heat and supported in cable tray/ ducts. Wiring for ceiling fan and lights supported at roof trusses. E- 15 CABLE & CABLE SUPPORTS Steam line crossing electrical wirings. Steam line (pipes) crossing wiring (protected or not protected) must be separated and installed at safe distances. Steam line (damaged glass wool insulation) crossing wirings above SDB panel. 8 E- 16 EQUIPMENT & MACHINE Compressor machine mounted on wheel. Compressor machine mounted on wheel must be anchored or the wheels must be locked to prevent from trolling. Compressor machine installed with wheels. E- 17 BOILER & COMPRESSOR ROOM Motor in the boiler room, not firmly fixed on the foundation/frame. Motor in boiler must be firmly grouted on the concrete floor or fixed on the foundation structures. Foot mounted motor without proper foundation and laying on bricks. E- 18 BOILER & COMPRESSOR ROOM Wiring and cables installed in boiler room are not protected. All electrical installations, including wiring and cable works must be protected against heat from the boiler by supporting and separating at safe distance. Wirings and cables installed in the boiler room are not properly protected from the heat. Page 8 of 9

9 E- 19 BOILER & COMPRESSOR ROOM MCBs and contactors are not fixed firmly on the panel board, inside boiler control panel. MCB, contactor and other devises used in panel must be firmly fixed and protected from touching unintentionally to other live parts. MCBs and contactors inside the boiler control panel. E- 20 GENERATOR ROOM Wet floors in generator room. Floors in generator room must be free from water and oil spillage. Water spillage inside generator room. E- 21 WIRINGS Cables terminating to distribution panels (switchboards) are not protected. Flexible PVC conduit used for cable protection, but slit open from side may be removed and supported on rigid cable supports. Cable tray, riser or conduits may be used. PVC hose conduit used is not proper and open gaps are still found(typical). Page 9 of 9
