1 Job Order Sewer Repair Services Bid Addendum No. 1 October 10, 2018 All Contractual Requirements as defined within the Contract Documents shall remain in full force to the extent that they are not modified or in conflict with the attached addendum. A. CLARIFICATIONS 1. REPLACE the BID ITEMS and Bid Item description from Sheets 30 to 35 of Bid Documents with the attached. B. QUESTIONS AND ANSWERS Q1. Will the tasks limited to residential/local streets? What about busier streets? A1. The expected tasks will be limited to residential/local streets. Bid Item No. 4 covers local streets using typical traffic control using San Diego Regional Standard TCP-08 and TCP-09. Any other activities requiring more intense traffic control will use the traffic control allowance. Q2. Will message boards be needed? A2. The City does not foresee the need for message boards on local/residential streets. Any other activities requiring more intense traffic control than TCP-08 and TCP-09 will use the traffic control allowance. Q3. Will tasks be located in the County of San Diego and City of San Marcos jurisdictions? A3. Locations shall include County of San Diego and NOT the City of San Marcos. Contractor shall consider trench detail repairs and traffic control permits in the County of San Diego. Bid Item descriptions have been updated to include County of San Diego. Q4. What type of backfill material is required? A4. Contractor shall use SE 30 in the trench zone per Bid Item descriptions. Q5. Who should provide compaction testing? A5. Contractor shall provide compaction testing in the various bid items.

Inspection will be performed by the City s Public Works Department. Q8. Will the example task orders be the first task orders? A8. Yes. Q9. What is the timing of starting the task orders? A9. The following Description of Bid Items, Work Listed in the Schedule of Work Items and Work Not Listed in the Schedule of Work Items are part of the Bid Schedule and shall be submitted with the Bid. The items marked with an asterisk (*) are contingency items and can be used only if approved by the Agency. 31

5 A. Description of Bid Items: Bid Item 1, Mobilization, Demobilization and Clean-Up: Bid item shall include full compensation for all bonds, insurance, sureties, licenses, City of Vista Encroachment Permit (no fee), facilities, labor, equipment and material necessary to complete all work required in advance of construction operations, and upon the initiation of construction operations, to mobilize all required equipment, materials and personnel on-site, and complete all associated and appurtenant work in accordance with the Standard Specifications, the Special Provisions, and as directed by the Agency. The Bid Item shall also include all labor, equipment and material required to clean-up and restore the site, demobilize and remove all equipment, materials, and staging, and complete all work required at the end of construction operations in accordance with the Standard Specifications, Standard Drawings, the Special Provisions, and as directed by the Agency. Bid Item 2, Stormwater Management: Bid item shall include full compensation for all labor, equipment and material required to complete all work necessary to obtain and comply with all provisions of the City of Vista s Stormwater Management Permit. This work shall include, but may not necessarily be limited to the implementation, maintenance and removal of all required Best Management Practices (BMPs); the development, performance and submittal of all required storm water documentation, site inspections, employee training, wet weather response and associated activities in accordance with the Standard Specifications, Standard Drawings,the Special Provisions, and as directed by the Agency. Bid Item 3, Sheeting, Shoring and Bracing: Bid item shall include full compensation for all labor, equipment and material required to complete all work required for trench excavation plans, sheeting, shoring, and bracing necessary for the protection of life and limb, in conformance with the State of California Labor Code Section 6705 and 6707 and all other applicable safety orders. Bid Item 4, Traffic Control and Construction Staging - Local Street: Bid item shall include full compensation for all labor, equipment and materials required to complete all work required to prepare, submit and obtain approved Traffic Control Plans from the City of Vista Traffic Engineering Department or County of San Diego; handle traffic (including flagging costs and person(s) to safely control pedestrian and vehicular traffic), furnish tools, equipment and incidentals, and for doing all work involved in staging, constructing, furnishing, placing, removing, storing, maintaining, moving to new locations, replacing and disposing of the components of the traffic control system (including traffic control devices, temporary steel plate bridging, temporary road resurfacing); installation and removal of all required temporary striping; associated and appurtenant work as specified in the Standard Specifications, Standard Drawings,the Special Provisions, and as directed by the Agency. Bid Item 5, Spill Prevention and Emergency Response Plan: Bid item shall include full compensation for all labor, equipment, maintenance and material required to complete all work to prepare and implement a project-specific Spill Prevention and Emergency Response Plan for the duration of the project. The Contractor shall submit the plan to the Agency no later than the seven calendar days before start of task order. 32

6 Bid Item 6, Pre and Post CCTV of Sewer Mains: Bid item shall include full compensation for all labor, equipment, maintenance and material required to pre and post CCTV the sewer main in accordance with National Association of Sanitary Sewer Companies (NASSCO) PACP Guidelines, Standard Specifications, Standard Drawings,the Special Provisions, Plans, and as directed by the Agency. Contractor shall provide the Post CCTV inspection in NASSCO Exchange format. Bid Item 7, Waste Management: Bid item shall include full compensation for all labor, equipment and material required to complete all work necessary to obtain and comply with all provisions of the City of Vista s Waste Management Permit in accordance with the Standard Specifications, Standard Drawings, the Special General Provisions, Plans, and as directed by the Agency Bid Item 8, Point Repair (Depth less than 10-feet): Bid item shall include full compensation for all labor, equipment, maintenance and material required to complete all work to saw cutting asphalt concrete, excavate, remove and dispose of existing pipelines, subgrade preparation, installation of pipe bedding, pipeline installation, wyes, tees, bends, fittings, manhole connections, pipe (bedding) zone backfill, remove and dispose of excavated material and import of SE 30 backfill for the backfill zone and compaction to subgrade, remove and dispose of bituminous asphalt concrete pavement, base and sub-grade to the depth and limits identified in the field, pipeline installation, and perform a point repair to a 6-inch to 8-inch diameter gravity sewer pipeline at a trench depth not to exceed 10-feet deep including bedding and a trench length not to exceed 10-feet, and asphalt and subgrade resurfacing, in accordance with the Standard Specifications, Standard Drawings, the Special Provisions, Plans, and as directed by the Agency. Bid item shall include notification of Dig Alert and potholing, locating, protection of all Private and Public utilities which may be affected by the Work. Bid Item 9, * Open-Cut Replacement (Depth less than 10-feet): Bid item shall include full compensation for all labor, equipment, maintenance and material required to complete all work to install new 6-inch to 8-inch diameter PVC SDR-35 sewer pipeline including, but not necessarily limited to the location and protection of existing sewer laterals, saw cutting asphalt concrete, excavation, removal and disposal of existing sewer pipeline, subgrade preparation, installation of pipe bedding, pipeline installation, wyes, tees, bends, fittings, manhole connections, pipe (bedding) zone backfill, remove and dispose of excavated material and import of SE 30 backfill for the backfill zone and compaction to subgrade, remove and dispose of bituminous asphalt concrete pavement, base and sub-grade to the depth and limits identified in the field, pipeline installation, and asphalt and subgrade resurfacing, and other associated and appurtenant work activities, testing and inspection in accordance with the Standard Specifications, Standard Drawings, the Special Provisions, Plans, and directed by the Agency. Bid Item 10, Locate and Reconnect Sewer Lateral to Sewer Main (Depth less than 10-feet): Bid items shall include full compensation for all labor, equipment, maintenance and material required to complete all work to locate and reconnect sewer laterals to sewer main in accordance with the Standard Specifications, the Special Provisions, Plans, and as directed by the Agency. Bid item shall include notification of Dig Alert and potholing, locating, protection of all Private and Public utilities which may be affected by the Work. 33

7 Bid Item 11, * Remove and Replace Existing Manholes with New Manhole (4 x 3 ) per SWR-3, SWR-7, and SWR-30B. (Depth less than 10-feet): Bid item shall include full compensation for all labor, equipment and material required to demolish and remove the existing sewer manhole and construct a new 4 x 3 diameter sewer manhole in accordance with City of Vista Standard Drawings. Manhole cover and collar shall be constructed in accordance with City of Vista Standard Drawing SWR-30B. Work shall include, but may not be necessarily limited to removal of existing sewer manholes & associated and appurtenant facilities, excavation, saw cutting asphalt concrete, pipeline connections, cast-in-place manhole base, installation of precast manhole components, frame and cover, testing, backfill, raise to finish grade, concrete collar, restore pre-existing pavement removed and/or damaged by excavation, manhole reconstruction and/or other associated and appurtenant work activities in accordance with the Standard Specifications, Standard Drawings, the Special Provisions, Plans and as directed by the Agency. Bid item shall include notification of Dig Alert and potholing, locating, protection of all Private and Public utilities which may be affected by the Work. Bid Item 12, * Sewer Pipeline Clean-out (Depth less than 10-feet) Bid item shall include full compensation for all labor, equipment and material required to construct a new sewer dead end clean out in accordance with the Standard Specifications, Standard Drawings, the Special Provisions, Plans and as directed by the Agency. Bid Item 13, Point Repair (Depth more than 10-feet and less than 20-feet): Bid item shall include full compensation for all labor, equipment, maintenance and material required to complete all work to saw cutting asphalt concrete, excavate, remove and dispose of existing pipelines, subgrade preparation, installation of pipe bedding, pipeline installation, wyes, tees, bends, fittings, manhole connections, pipe (bedding) zone backfill, remove and dispose of excavated material and import of SE 30 backfill for the backfill zone and compaction to subgrade, remove and dispose of bituminous asphalt concrete pavement, base and sub-grade to the depth and limits identified in the field, pipeline installation, and perform a point repair to a 6-inch to 8-inch diameter gravity pipeline at a trench depth more than 10-feet and not to exceed 20-feet deep including bedding and a trench length not to exceed 10-feet, and asphalt and subgrade resurfacing, in accordance with the Standard Specifications, Standard Drawings, the Special Provisions, Plans, and as directed by the Agency. Bid item shall include notification of Dig Alert and potholing, locating, protection of all Private and Public utilities which may be affected by the Work. Bid Item 14, * Open-Cut Replacement (Depth more than 10-feet and less than 20-feet): Bid item shall include full compensation for all labor, equipment, maintenance and material required to complete all work to install new 6-inch to 8-inch diameter PVC SDR-18 sewer pipeline including, but not necessarily limited to the location and protection of existing sewer laterals, excavation, saw cutting asphalt concrete, removal and disposal of existing sewer pipeline, subgrade preparation, installation of pipe bedding, pipeline installation, wyes, tees, bends, fittings, manhole connections, pipe (bedding) zone backfill, remove and dispose of excavated material and import of SE 30 backfill for the backfill zone and compaction to subgrade, remove and dispose of bituminous asphalt concrete pavement, base and sub-grade to the depth and limits identified in the field, pipeline installation, and sewer lateral construction and reconnection and/or other associated and appurtenant work activities, testing and inspection, and asphalt and subgrade resurfacing in accordance with the Standard Specifications, Standard Drawings, the Special Provisions, Plans, and directed by the Agency. Bid item shall include notification of Dig 34

8 Alert and potholing, locating, protection of all Private and Public utilities which may be affected by the Work. Bid Item 15, Locate and Reconnect Sewer Lateral to Sewer Main (Depth more than 10-feet and less than 20-feet): Bid items shall include full compensation for all labor, equipment, maintenance and material required to complete all work to locate and reconnect sewer laterals to sewer main in accordance with the Standard Specifications, Standard Drawings, the Special Provisions, Plans, and as directed by the Agency. Bid Item 16, * Remove and Replace Existing Manholes with New Manhole (4 x 3 ) per SWR-3, SWR-7, and SWR-30B. Depth more than 10-feet and less than 20-feet): Bid item shall include full compensation for all labor, equipment and material required to demolish and remove the existing sewer manhole and construct a new 4 x 3 diameter sewer manhole in accordance with City of Vista Standard Drawings. Manhole cover and collar shall be constructed in accordance with City of Vista Standard Drawing SWR-30B. Work shall include, but may not be necessarily limited to removal of existing sewer manholes & associated and appurtenant facilities, excavation, saw cutting asphalt concrete, pipeline connections, cast-in-place manhole base, installation of precast manhole components, frame and cover, testing, backfill, raise to finish grade, concrete collar, restore pre-existing pavement removed and/or damaged by excavation, manhole reconstruction and/or other associated and appurtenant work activities in accordance with the Standard Specifications, Standard Drawings, the Special Provisions, Plans and as directed by the Agency. Bid item shall include notification of Dig Alert and potholing, locating, protection of all Private and Public utilities which may be affected by the Work. This is a contingency Bid Item 17, * Sewage Bypass and Pumping Plan Allowance: This is a contingency item and will only be used when authorized by the Agency when sewer bypass is needed due high flows. An allowance of $5,000 has been set up for this item. Bid Item 18, * Traffic Control Allowance: This is a contingency item and will only be used when authorized by the Agency when construction activities are not on local streets. An allowance of $5,000 has been set up for this item. B. Work Listed in the Schedule of Work Items: 1. Work under this contract will be paid on a unit price or lump-sum basis as outlined on the Bid Form for the quantity of work installed. 2. The unit prices and lump-sum prices include full compensation for furnishing the labor, materials, tools, and equipment and doing all the work involved to complete the work included in the contract documents. 3. The application for payment will be for a specific item based on the percentage completed or quantity installed. The percentage complete will be based on the value of the partially completed work relative to the value of the item when entirely completed and ready for service. 4. Extra work or changes in the Work shall be accomplished as provided in the General 35

9 Provisions. C. Work Not Listed in the Schedule of Work Items: 1. The General Provisions and items in the Special Provisions, general requirements, and specifications which are not listed in the schedule a of work items of the Bid Schedule are, in general, applicable to more than one listed work item, and no separate work item is provided therefore. Include the cost of work not listed but necessary to complete the project designated in the contract documents in the various listed work items of the Bid Schedule. 2. The bids for the work are intended to establish a total cost for the work in its entirety. Should the Contractor feel that the cost for the work has not been established by specific items in the Bid Form, the Contractor shall include the cost for that work in some related bid item so that the Proposal for the Project reflects the total cost for completing the work in its entirety. --- END OF BID SCHEDULE
