1 Westerly Housing Authority, 5 Chestnut Street, Westerly, RI, Client INVITATION TO BID Name: Carlos Lopez, Executive Director Town of Westely Housng Authority Address: 5 Chestnut Street, Westerly, RI Site Location: 9 Dixon Street, Westerly, RI Project Type: Design/Construct Bid for a Retaining Wall The Project This project description focuses upon the design/construct requirements for a new retaining wall along the southerly perimeter of the Housing Authority property, Park View Apartments, 9 Dixon Street and the residential property at 7 Dixon Street. The existing, but damaged retaining wall must be removed and all materials removed down to ground level, prepared to receive the new retaining wall. The length of the wall is approximately 120 feet and ranges in elevation from 6 feet down to 1 foot. The wall is to be constructed from the rear of the adjacent garage, wrapping around the rear of the existing garage for approximately 3 feet so as to prevent any further erosion of the slope. Please consider any damage to the adjacent driveway. Materials may be a decorative concrete block, stone or another alternative material acceptable to the Westerly Housing Authority. Any and all requirements of the Town of Westerly Building Official must be included. The retaining wall must include an acceptable tie-back system as needed. The 7 Dixon Street neighbors are willing to allow a construction easement, but the neighbors must be held harmless against any liability. Please provide a written description of the design/construct method of work to be utilized. The scope includes all landscaping required to restore any damaged landscaping due to the wall construction. Bidders are to include a schedule for the design/construct scope of services.

2 Page: 2 Requirements of the Westerly Housing Authority Any bidder which is a corporation not chartered under the laws of the State of Rhode Island must submit an affidavit certifying that said corporation is authorized to do business in the State of Rhode island. Bidders are also required to comply with the requirements of Public law 1975, c.127 which pertaining to Non-Discrimination and Affirmative Action, and Public law 1977, c.33, which requires a Statement of Corporate Ownership. Bidders are also required to comply with Form HUD-537- EZ, General Contract Conditions for Small Construction/Development Contracts. Proposals are to be delivered to the Westerly Housing Authority, 5 Chestnut Street, Westerly, RI, no later than: The Housing Authority reserves the right to reject any or all proposals and to accept the proposal it deems to be in the best interest of the Authority. 3 Page: 3 Insurance Before performing any work, the design/construct contractor (the contractor) shall furnish the Authority with certificates of insurance showing the following insurance is in force and will insure all operations under the contract as follows: Workers Compensation, in accordance with Rhode Island Workers Compensation laws; Commercial General Liability with a combined single limit for bodily injury and property damage or not less $1,000, per occurrence to protect the Contractor and the Authority against claims fopr bodily injury or death and damage to the property of others; This shall cover the use of all equipment, hoist and vehicles on the site(s) not covered by Automobile Insurance noted below; If a contractor has a claims-made policy, then the following additional requirements apply: the policy must provide a retro-active date which must be on or before the execution date of the Contract, and the extended reporting period may not be less than 5 years following the completion date of the Contract; and, Automobile Liability on owned and non-owned motor vehicles used on the site(s) or in connection therewith for a combined single limit for bodily injury and property damage of not less than $500, per occurrence. Scope of Work: The Design/Construct Contract scope of services to be quoted includes the following: Survey and design the dimensions for the retaining wall; Design the details of the proposed retaining wall including all elements described in The Project, herein; Procure any and all municipal or state regulatory permits; Remove the existing damaged retaining wall and remove any soils and debris in preparation for the retaining wall s foundation;

4 To: Design/Construct Contractor Page: 4 Construct the foundation for the retaining wall; Construct the retaining wall in accordance with the approved design; and, Construct any finish landscaping and/or reconstruct any damage to the area during the construction. Please provide an itemized quote for each service. Planned construction period is: November, December, 2018 and January, February, Required trades and activities include the following (check CSI specification, as the list may not be all inclusive): Division One General Requirements: 1905 Insurance and bonding Construction manger Engineering. Division Two Site Construction 2100 Site preparation Excavation support and shoring systems Soil stabilization Landscaping and irrigation. Division Four - Masonry: 4200 Masonry Stone Special masonry installations.
