3 Minerva Park Dam Improvements Village of Minerva Park Ohio Minerva Park, Ohio GCI Project No.: 17-F Field Representative: M. McGannon Date: 10/02/17 Work Shift: Days WEATHER Conditions: Light AM Rain Temperature: AM: 50 PM: 70 Precipitation: Amount: CONTRACTORS / PERSONNEL ON SITE Environmental Remediation Contractors (ERC) - Marty, Shane CONSTRUCTION ACTIVITES ERC continued work on the lake drainage system plan. Pipe was mortared in-place in both the lake drain and gate structures. Excavations began to further widen the temporary spillway in order to cut a trench leading to the outlet headwall. (Reference Pages 8/21, 9/21, 17/21, 18/21, and 19/21 of approved plans dated 12/12/16, and issued by EMH&T). See photos on the EMH&T shared project website. 4 Minerva Park Dam Improvements Village of Minerva Park Ohio Minerva Park, Ohio GCI Project No.: 17-F Field Representative: M. McGannon Date: 10/03/17 Work Shift: Days WEATHER Conditions: Temperature: AM: 61 PM: 86 Precipitation: Amount: CONTRACTORS / PERSONNEL ON SITE Environmental Remediation Contractors (ERC)-Marty, Shane CONSTRUCTION ACTIVITES ERC continued work on the lake drainage system plan. EMH&T survey was on-site in the morning to set locations for the headwall, as well as, future excavation at the dam. ERC excavated from the gate structure to the headwall location. The headwall was set in-place and (5) pieces of 14 foot PVC pipe were placed between the gate structure and the headwall. (Reference Pages 8/21, 9/21, 17/21, 18/21 and 19/21 of approved plans dated 12/12/16, and issued by EMH&T). See photos on the EMH&T shared project website. 5 Minerva Park Dam Improvements Village of Minerva Park Ohio Minerva Park, Ohio GCI Project No.: 17-F Field Representative: M. McGannon Date: 10/04/17 Work Shift: Days WEATHER Conditions: Temperature: AM: 60 PM: 83 Precipitation: Amount: CONTRACTORS / PERSONNEL ON SITE Environmental Remediation Contractors (ERC)-Marty, Shane CONSTRUCTION ACTIVITES ERC de-watered the pipe trench between the gate structure and headwall. A second riser was placed on the gate structure with con seal between the sections. Forms were placed to contain CDF around the base of the gate structure per requirement. CDF was placed, encasing the PVC pipe from the gate structure risers to the headwall. Work slowed when the pipe leaving the gate structure separated from the structure and CDF leaked into the pipe. The material was immediately flushed from the pipe. CDF placement will be completed at a later date. Approximately 100 tons of Type D stone, and 100 tons of manufactured sand were delivered for future use to close the temporary spillway. (Reference Pages 8/21, 9/21, 17/21, 18/21, and 19/21 of approved plans dated 12/12/16, and issued by EMH&T). See photos on the EMH&T shared project website. 6 Minerva Park Dam Improvements Village of Minerva Park Ohio Minerva Park, Ohio GCI Project No.: 17-F Field Representative: M. McGannon Date: 10/05/17 Work Shift: Days WEATHER Conditions: Rain Temperature: AM: 60 PM: 76 Precipitation: Amount: CONTRACTORS / PERSONNEL ON SITE Environmental Remediation Contractors (ERC)-Marty, Shane CONSTRUCTION ACTIVITES ERC removed large stones holding PVC pipe in place between the gate structure and the headwall. Spaces left in the CDF are to be filled at a later date. With rain starting, the crew constructed an earthen dam to protect the gate structure and control potential water flow from the lake. The 3 inch caps were removed in the lake drain to allow water to flow through the drainage system if the lake level rose to a high level. Work ended as rains became heavy. (Reference Pages 8/21, 9/21, 17/21, 18/21 and 19/21 of approved plans dated 12/12/16, and issued by EMH&T). See photos on the EMH&T shared project website. McGannon Date: 10/06/17 Work Shift: Days WEATHER Conditions: Rain Temperature: AM: 62 PM: 80 Precipitation: Amount: CONTRACTORS / PERSONNEL ON SITE Environmental Remediation Contractors (ERC)-Marty CONSTRUCTION ACTIVITES ERC was on-site to fuel equipment and evaluate site conditions. Work was cancelled on-site due continued rains. See photos on the EMH&T shared project website. MATERIALS DELIVERED ENVIRONMENTAL / ECD's / SWPPP ISSUES NOTED Notifications: Corrective Actions:
