As applicable to consumer commodities: 1) Statement of identity. 2) Name and place of business of manufacturer, packer or distributor. Place of business shall include street address, city, state, and zip code; however, the street address may be omitted if it is listed in a readily accessible, widely published, and publicly available resource, including but not limited to a printed directory, electronic database, or Web site. (Effective Dec. 17, 2015.) 3) Net quantity of the contents in terms of weight, measure, or numerical count. CHICAGO, IL - requirement is for polyethylene bag, other than one used for food products weighing not more than five pounds, intended for household. MASSACHUSETTS & NEW YORK - You must label if: 1) Thickness is less than one mil, and 2) Size of opening has a diameter of seven inches or more, and 3) Length and width when added together totals 25 inches or more. RHODE ISLAND - You must label if: 1) Thickness is less than one mil, and 2) Size of opening is larger than five inches in diameter. VIRGINIA - You must label dry cleaning bags if: 1) Thickness is less than one mil, and 2) Length totals twenty-five inches or more. CALIFORNIA & RHODE ISLAND - Total length and width of: <30 inches = 10 point type minimum = or > 30 inches and < 40 inches = 14 point type minimum = or > 40 inches and < 60 inches = 18 point type minimum = or > 60 inches = 24 point type minimum. MASSACHUSETTS - Total length and width of: = or > 25 inches and < 40 inches = 14 point type minimum = or > 40 inches and < 60 inches = 18 point type minimum = or > 60 inches = 24 point type minimum. Label shall appear on both sides of bag. If total length and total width is more than 40 inches (when added), label shall be repeated at 20 inch intervals. NEW YORK - Total length and width of: = or > 25 inches and < 30 inches = 10 point type minimum = or > 30 inches and < 40 inches = 14 point type minimum = or > 40 inches and < 60 inches = 18 point type minimum = or > 60 inches = 24 point type minimum. VIRGINIA - 36 point type minimum. Plastic bags in California, Massachusetts, New York, Rhode Island, and Virginia require the following warning: "WARNING: To avoid danger of suffocation, keep this plastic bag away from babies and children. Do not use this bag in cribs, beds, carriages or play pens. This bag is not a toy." If the warning is required based on thickness and opening size, but not present, then the test shall be reported as fail. If a warning is present, report the warning listed on the bag. Recommended Testing Performance Attachment Strength: Pull Gauge 1 Locks, Logos, Ornaments, Snaps and Zipper Pulls & Closures: 11 lbs minimum Accessories Cycle Usage Test: 100 Cycles UL-TS-111 Edition: Product shall operate as intended after 100 cycles. Note: If sample includes clips, they must operate normally after 100 cycles of open and close. High Humidity & High Temperature Test UL-TS-071 Edition: After exposure to heat and high humidity for a specified time period, 50C (122F) and 95% Relative Humidity for 24 hours, product shall continue to perform its intended function and remain cosmetically acceptable. Low Temperature Test UL-TS The product shall continue to perform its intended function and shall remain cosmetically acceptable after being placed in the chamber at -18C (0F) for 8 hours. Performance Test UL-TS Product shall perform as intended and meet all verifiable labeled claims. Static Load Test Static Load Test: Hook UL-TS-065 Edition: 13.0 UL-TS-065 Edition: Load sample with labeled claim. If no labeled claim, sample shall withstand a specified load for a specified length of time with no evidence of breaking, cracks, parts separation, instability or obvious permanent deformation. Note: Sample shall withstand a load of 2 lbs for one hour. 1 Load with labeled claim. If no labeled claim, there shall be no evidence of breaking, cracks, parts separation, instability, or permanent deformation when the sample is subjected to the specified load for the specified length of time listed below: Apply kg/cm (0.7 lb/in) load at the hook (using the length of the hook to determine the load) for 1 hour. Note: Samples that include hooks intended to hold product other than vinyl pouch (i.e., intended to hold water, keys, etc.).
