2 REQUEST FOR BIDS Woodlawn Road Culvert & Slide Repair Nelson County Fiscal Court will accept sealed bids for furnishing all labor and equipment to excavate overburden material, remove existing culvert, install a new 72 inch circular corrugated culvert 120 foot long and backfill to existing grade line of Woodlawn Road and repair a road slide on Woodlawn Road. Sealed Bids will be opened on Tuesday, July 31, 2018 at 10:00 A.M. at the Nelson County Courthouse located at One Court Square, Bardstown, Kentucky. Contract bid forms and specification can be obtained at this office or by contacting the Nelson County Road Department, Bradley T. 4 This CONTRACT PROPOSAL/ACCEPTANCE by and between the Nelson County Fiscal Court (herein called the owner) and (hereinafter called the Contractor). The OWNER and the CONTRACTOR shall, in consideration of the mutual covenants hereinafter set forth, agree as follows: 1. WORK The CONTRACTOR shall complete all work as specified or indicated in these CONTRACT DOCUMENTS and SPECIFICATION. The work is generally described as follows: Excavate overburden material and remove the existing cross-drain culvert on Woodlawn Road. Install a 72" circular corrugated culvert 120 feet long and backfill excavation to existing roadway elevation; repair road slide failure per the plans and specifications. 2. CONTRACT DOCUMENTS & PLANS/SPECIFICATIONS The culvert shall be constructed and installed as well as the road slide repaired in accordance with this Contract Proposal and the attached specification entitled "Specification - PROJECT #2018-NC-009, CULVERT REPLACEMENT, WOODLAWN Road". The Contractor shall be responsible for all items in the specifications and in these Contract Proposal Documents 3. CONTRACT TIME: The time period for completion of this contract shall be 60 calendar days from the Notice to Proceed. The Notice to Proceed shall be issued after the contractor has been 1

5 given a Notice of Contract Ward and the contractor has provided all insurance documents. A signed copy of this contract agreement will be mailed to the Contractor when bids are approved by the Nelson County Fiscal court. Reference SPECIAL NOTES in Section 5 of the Contract Specification for actual time allowed from removal of existing culvert to complete installation of new culvert. 4. CONTRACT INSPECTION: Inspection for this contract including submittal of material used, requests for payment, and acceptance of work completed will be performed by the - Bradley T. Spalding, P.E., County Engineer, phone # (502) PAYMENT PROCEDURES. Payment for work completed will be made once per month. Invoices for work completed shall be submitted to the Project Engineer for approval by the Second (2nd) Wednesday of each month. Submit payment request to Nelson County Fiscal Court 820 West Stephen Foster Avenue Bardstown, Kentucky UTILITIES/STORAGE SPACE Storage space at the site for trailers will be the responsibility of the contractor who must coordinate and obtain approval when storing equipment on adjoining property. Equipment may be stored at the site at the contractor's own discretion within the 2

6 construction easement area only. No utilities for use during construction are available at this site. 7. INDEMNIFICATION/INSURANCE: The Contractor shall purchase and maintain at the Contractor's expense for the full contract period such insurance as is appropriate for the work being performed. The insurance shall cover claims which may arise from the Work performed with this contract which includes Claims under Worker's Compensation, disability benefits, and claims for damages because of bodily injury, personal injury, sickness or disease, or death of any of the CONTRACTOR's Employees or for any person other than the CONTRACTOR's Employees who may be hurt/injured/killed as a result of the Contractor's Work on this contract. *** The Contractor shall provide proof of insurance prior to start issuance of the Notice to Proceed with work.*** The Contractor shall indemnify and hold harmless the OWNER and the ENGINEER against all claims, damages, losses and expenses, arising out of or resulting from the performance of work on this contract. 3

BID PROPOSAL I, (name) The duly authorized representative of the CONTRACTOR,(Name),of (address), (Phone) do/does hereby propose to perform all work described as listed in this CONTRACT PROPOSAL, any identified addendum, and the Contract Specification #2018-NC-009, to excavate overburden material and remove the existing cross-drain culvert on Woodlawn Road. Install a 72" circular corrugated culvert 120 feet long and backfill excavation to existing roadway elevation; repair road slide failure per the plans and specifications located on Woodlawn Road for the: Bid Item #1 $ (Culvert Install & Associated Items) Bid Item #2 $ (Slide Repair, Estimated 550 tons) Provide a Cost per Ton of rock for Bid Item #2 $ If additional rock is required or the total estimated 550 tons is not used, the cost per ton of rock shall be added/deducted at the provided tonnage rate. CONTRACT ACCEPTANCE: Having reviewed said proposal from the CONTRACTOR, dated To perform all work described as listed in this CONTRACT PROPOSAL, any identified addendum, and the Contract Specification #2018-NC-009, to excavate overburden material and remove the existing cross-drain culvert on Woodlawn Road. Install a 72" circular corrugated culvert 120 feet long and backfill excavation to existing roadway elevation; repair road slide failure per the plans and specifications located on WOODLAWN Road for the bid amount of: Bid Item #1 $ (Culvert Install & Associated Items) Bid Item #2 $ (Slide Repair, Estimated 550 tons) Provide a Cost per Ton of rock for Bid Item #2 $ If additional rock is required or the total estimated 550 tons is not used, the cost per ton of rock shall be added/deducted at the provided tonnage rate. The OWNER, (the Nelson County Fiscal Court) does hereby accept the CONTRACTOR'S Proposal as the low/best qualifying bid. By the OWNER signing this CONTRACT ACCEPTANCE - The OWNER AND THE CONTRACTOR each binds itself, its, partners, successors, assigns and legal representatives to the other party hereto, its partners, successors, assigns and legal representatives in respect of all covenants, agreements, and obligations contained in these Contract Documents. 11 1. Project Description and Contractor Qualifications: A. Project Description: All interested qualified contractors shall submit a lump sum bid to provide all equipment and manpower to excavate overburden material and remove the existing cross-drain culvert on Woodlawn Road. Install a 72" circular corrugated culvert 120 feet long and backfill excavation to existing roadway elevation; repair road slide failure per the plans and specifications as follows: CULVERT REPLACEMENT - Culvert shall be a 72" circular corrugated culvert. - Culvert length shall be a 120 feet (out to out) - Concrete Headwalls are not required for this project. However, the Contractor shall install Gabion Baskets (3' x 3' x 12'; galvanized filled with Class II Channel Lining) as the drawings and amounts indicated on the plans. - Culvert inlet and outlet inverts shall match the existing culvert inverts and ensure proper drainage of this area. - Culvert shall be installed as per manufacture s installation procedures. Culvert shall be banded with connector bands and backfill around the culvert shall be compacted to manufacture s requirements. - Backfill material above the culvert and the excavated approach area shall be placed in one (1) foot lifts and compacted within 95% of optimum density within 10-inches of existing grade line of the road. Pre-approved aggregate may be used by the Contractor in lieu of excavated material over the culvert. Excess excavation material can be disposed of at the inlet and outlet of the culvert within the valley. - Backfill slopes for the culvert shall be constructed with a maximum 2:1 slope allowing a minimum width of 30 feet for the final grade line of the road. - Contractor shall install 6-inches of compacted DGA over the disturbed roadway to the final grade to match the existing roadway less 4-inches. 1

12 ROAD SLIDE REPAIR - Road slide area consists of an approximate slide 12' depth extending approximately 120' long. - Contractor shall grade the bottom of the slide area to drain to the rock bleeders as shown on the plans. A 4-foot wide existing shoulder shall remain with the contractor installing shot rock backfill so the finished shoulder width is 6-feet from the pavement (see drawings). Shot rock backfill shall be installed per the repair details and plans. - Non-wooven 4-ounce filter fabric shall be installed all along the base and up the hillside to the roadway. - Place 12-inch diameter aggregate (shot rock) in the slide area a width of 12' at the base, 2' at the top. - All bidders shall review the site prior and be thoroughly familiar with the site prior to bidding. B. Contractor Qualifications: All bidders shall be capable of providing the necessary equipment and manpower to complete this project as per the contract and specifications. 2. Project Location/Plans: Project Location and Plans are shown on the attached. The contractor shall coordinate/notify the Nelson County Road Department Personnel prior to any construction work. *** The contractor shall be totally responsible for placement of the culvert. The Contractor shall coordinate this work with the Nelson County Road Department prior to starting any construction. The Contractor shall be responsible for off-loading the culvert at the site.*** 3. Removal of Existing Culvert/Traffic Control: The contractor shall be responsible to set all appropriate traffic control barricades, flashing lights, and road closing signs to warn all motorist this road is closed for the duration of the project as Special Note below. The traffic control plan used shall be approved by the Nelson County Road Department. 2

13 SPECIAL NOTE: WOODLAWN Road shall be closed to thru traffic prior to removal of the existing culvert. The County shall be responsible to notify all agencies prior to beginning work. The Contractor shall provide the County with a two week notice prior to beginning work. The Contractor shall be responsible for placement of signage and physical barriers within the roadway prior to the work site to prevent through traffic. The Time frame from removal of the existing culvert to installation of new culvert and backfill activities shall not exceed 21 days maximum. The contractor shall not remove the existing culvert until the new culvert has been scheduled for delivery.
