1 Five Forks Branch Library MPS Project No May 25, 2016 ADDENDUM NO. 3 The following clarifications, amendments, additions, deletions, revisions, and/or modifications are hereby made a part of the Contract Documents and change the original documents only in the manner and to the extent stated below: GENERAL 1. This form is required to be attached to the Bid Form. 1.3 DESCRIPTION A. The undersigned Bidder proposes the amount below be added to or deducted from the Base Bid if particular alternates are accepted by Owner. Amounts listed for each alternate include costs of related coordination, modification, or adjustment. B. If the alternate does not affect the Contract Sum, the Bidder shall indicate "NO CHANGE." C. If the alternate does not affect the Work of this Contract, the Bidder shall indicate "NOT APPLICABLE." D. The Bidder shall be responsible for determining from the Contract Documents the affects of each alternate on the Contract Time and the Contract Sum. E. Owner reserves the right to accept or reject any alternate, in any order, and to award or amend the Contract accordingly within [60] days of the Notice of Award unless otherwise indicated in the Contract Documents. 5/13/2016 ALTERNATES FORM

Acceptance or non-acceptance of any alternates by the Owner shall have no affect on the Contract Time unless the "Schedule of Alternates" Article below provides a formatted space for the adjustment of the Contract Time. 1.4 SCHEDULE OF ALTERNATES A. Alternate No. 01: Colored Aluminum Storefront Frames 1. Aluminum storefront frames to be custom color selected by architect, Kynar finish for exterior frames S6, S7, S8 x (2), S9, S10, S11, S12, S13, S14, S15, S16, S17 as indicted on sheet A2.12 and powder coated finish for interior frames S18, S19, S20, S21, S22, S23, S24, S25, S26, S27, S28 as indicated on sheet A All other interior storefront aluminum door frames to be powder coated white from manufacturer s standard colors as selected by architect.: b. Dollars B. Alternate No. 02: Textile Wall Covering 1. Textile Wall Covering Direct glue application of 5mm thick. 100% wool textile; cut and installed in the field as indicated on sheets ID2.13 & ID2.14. Basis of Design: Fitz Felt. C. Alternate 03: Fabric Wrapped Panels 1. Fabric Wrapped Panels: Pre-manufactured wool wrapped acoustical panels, installed with Z-clip to gypsum wall board as indicated on sheets ID2.13 & ID2.14. 4 D. Alternate 04: Underfloor Cable Trays 1. Cable Trays for underfloor wire management as indicated on sheet E2.14 E. Alternate 05: Assistive Listening System for Meeting Rooms 1. Provide and install a hearing loop (wireless network for hearing aids) for each meeting room. Installation: scored wet ½ inch deep and 1/10 wide (blade width). Score lines must be clean of all debris and slurry. Loop drive, design & wire, shall be installed by vendor. Contractor shall coordinate marking of score lines, accuracy and testing and termination points at control area prior to cover up with vendor. After installation the score lines must be filled with Ardex feather finish or equivalent so no transition to the carpet exists. Scrape flat and clean prior to application of floor finish. Hearing loop shall be designed to the International Standard IEC Basis of Design American Hearing Loop, F. Alternate 06: Commissioning of the HVAC systems. 1. Commissioning of the HVAC systems will be an Add Alternate. The Commissioning Firm will be contracted/hired by the Owner. The mechanical contractor and his subcontractors will be required to provide assistance to the Commissioning Firm throughout the construction for the various phases of commissioning. A Commissioning Specification will be issued as part of the next Addendum; the purpose of the 5/13/2016 ALTERNATES FORM

5 specification is to provide the mechanical contractor with the scope of the commissioning for which they will be providing assistance. G. Alternate 07: Fully adhered TPO roof system. 1.
