4 Struck-by Fatality A 47-year-old grounds man was killed when he was struck by a falling tree section during a tree trimming job. A crew leader was in the elevated bucket of a truck-mounted aerial lift cutting the top of the trunk of an 80-foot-high oak tree when the grounds man walked onto the landing zone (the area under the tree to which branches and debris will fall). He had been told by the crew leader, who was in charge of the site, to remain on a work break at their pickup truck. The crew leader was unaware that the victim was under his work area. He cut away a tree section, pushed down on it, and it fell onto the victim s head.

5 Electrocution Fatality A 25-year-old tree trimmer was electrocuted when he contacted a 7,600 volt overhead power line while clearing a loose branch from a tree. The incident occurred where the company had just finished removing a tree from the back yard. The crew had completed the job and was beginning to clean the site when they noticed a loose branch hanging from a tree. Using a climbing harness, the victim tied onto the ball of a crane line and was raised up to the hanging branch, which he cut down. As he was being lowered to the ground, the victim noticed a second loose branch hanging on another tree. The victim had started to swing toward the branch while on the crane line when he contacted a primary power line and was electrocuted.

6 Falling Fatality During the "tree felling" activity, a branch from an adjacent tree fell and fatally struck the tree cutter after the tree he was cutting fell over an adjacent tree. Accident was witnessed by a coworker who yelled at the cutter of the falling branch. Victim reacted a bit late and was pinned by the falling branch. Similar incident happened in two different NAVFAC locations within a span of 6 months.

7 Caught in-between Fatality A 33 year-old male tree trimmer died when body was caught in chipper gears while chipping small and medium size tree limbs. The victim was working alone at the brush chipper. Co-workers said they heard a strange noise come from the brush chipper. The supervisor on the job went to investigate, discovered the victim s remains, and immediately called 911. Although the incident was no witness, police concluded that the victim either lost his balance while feeding material into the chipper or was leaning across the feed table pushing trimmings into the feed chute when his gloves were caught by the feed rollers and he was pulled through the chipper blades.

8 Tree Maintenance & Removal A qualified tree worker shall make a visual inspection to determine whether an electrical hazard exists before climbing or before performing any work in or on a tree. 31.A.03.b

9 General Requirements Employees working in the proximity of electrical equipment or conductors shall consider them to be energized. 31.A.03.a

10 General If electrical lines or equipment cannot be safely avoided, arrangements shall be made with the power company to de-energize the power. 31.A.03.b

11 Equipment Employees shall be instructed in the safe and proper use of all equipment provided to them. 31.A.04.b

12 Aerial Platforms & Buckets Tree workers in a bucket or work platform shall use fall protection in accordance with manufacturer s recommendations. 31.A.08.a

13 Tree Climbing Use of Rope Access techniques should only be used where other means of access or undertaking the work such as mechanically operated work platforms or pole saws are not practical. 31.B.01.b

14 Tree Climbing Tree crews, where climbing is required, shall have a secondary climber that could assist in a rescue is necessary or the crew shall be working in proximity to nearby crews with a climber who could assist in a rescue if needed. 31.B.03

15 Tree Climbing A tree worker shall be tied in with an approved type of climbing rope and safety saddle when working above the ground. 31.B.04

16 Tree Climbing The climbing rope shall be passed around the trunk of the tree as high above the ground as possible using branches with a wide crotch to prevent any binding of the safety rope. Exception: Palms and other trees with similar growth characteristics that will not allow a climbing rope to move freely. 31.B.05

17 Tree Climbing A figure-eight knot shall be tied in the end of the rope, particularly when climbing high trees, to prevent pulling the rope accidentally through the taut-line hitch and possibly falling. 31.B.06

18 Tree Climbing The tree worker shall be completely secured with the climbing line before starting operation. 31.B.07

19 Tree Climbing Climbers should use chain saws less than 15 lbs.(6.8kg) and they should be connected to the climber by means of a saw lanyard. 31.B.10

20 Tree Climbing Climbing with tree spurs on live trees is generally not allowed. Tree spurs used on large trunk trees shall have longer gaffs, such as 2 in.(7cm). 31.B.12

21 Felling Prior to felling operations, the employee shall consider: Tree and surrounding area Tree shape, lean of tree, decayed or weak spots Wind force and direction Location of other people Electrical hazards 31.C.01

22 Felling Prior to felling tree operations, the work area shall be cleared to permit safe working conditions and an escape route shall be planned. 31.C.02

23 Felling Each worker shall be instructed as to exactly what he is to do. All workers not directly involved in the operation shall be kept clear of the work area. 31.C.04

24 Felling Before starting to cut, the operator shall be sure of his/her footing and must be clear away from brush, fallen trees, and other materials that might interfere with the cutting operation. 31.C.05

25 Felling A notch and backcut shall be used in felling trees over 5 in. (12.7 cm) in diameter. 31.C.06

26 Felling If sections of the tree are to be removed, sections shall be limited in length to 1/3 the distance to the nearest structure, i.e. if tree is 30 ft from structure, sections shall be no more than 10 ft. 31.C.07

27 Felling If there is danger that the tree being felled may fall in the wrong direction or damage property, wedges, block and tackle, rope or wire cable shall be used. 31.C.10

28 Brush Removal & Chipping Brush and logs shall not be allowed to create a hazard at the work site. 31.D.01

29 Brush Chippers Employees working with a brush chipper shall be trained in its safe operation. Exhaust Chute Quick Stop Feed Chute 31.D.02

30 Chipping All workers feeding brush into chippers shall wear eye protectors. Loose clothing, gauntlettype gloves, rings, and watches shall not be worn by workers feeding the chipper. 31.D.05

31 Chippers Employees shall never place hands, arms, feet, legs, or other part of the body on he feed table when the chipper is in operation or rotor is turning. Push sticks shall be used. t 31.D.06

32 Brush Chippers Brush chippers shall be fed from the side of the centerline, and the operator shall immediately turn away from the feed table. 31.D.07

33 Pruning and Trimming Pole pruners, pole saws, and similar tools shall be equipped with wood or nonmetallic poles. Actuating cords shall be of a nonconducting material. 31.E.01.a

34 Stump Cutting Stump cutters shall be equipped with enclosures or guards that effectively protect the operator. No protection 31.E.03

35 Cabling No more than two persons should be in a tree working at apposite ends during cabling installation 31.E.04.b

36 Topping/Lowering Limbs Workers performing topping operations shall ensure the trees can withstand the strain of topping procedures; if not, some other means of lowering the branches should be used. 31.E.05

37 Power Saws Climbers shall use a saw lanyard to carry the saw. 31.E.07.d

38 Power Saws The engine shall be started and operated only when all co-workers are clear of the saw and then in accordance with the manufacturer s recommendations and instructions. 31.E.07.e

39 Chain Saw PPE Proper PPE for chain saw includes, eye protection, chaps, safety boots, hearing protection, and head protection. 31.E.07.i

40 Chopping Tools All edged tools and blades shall be properly sheathed when not in use. 31.E.08.e

41 Wedges & Chisels Only wood, plastic, or soft metal wedges shall be used with power saws.

42 Wedges & Chisels Wood-handled chisels should be protected with s ferrule on the striking end.
